Q: Rash - Stevens-Johnson Syndrome... My husband has been taken into hospital after developing a strange rash which spread to his mouth and caused masses of blisters in his mouth, eyes and genitals. The doctors have said that they think he has had a severe reaction to some antibiotics he was taking for a chronic chest problem (the antibiotics were called co-trimoxazole, I think). Is this an allergy and how quickly will it settle? Maisie Dr Trisha Macnair responds It sounds as though your husband has a condition known as Stevens Johnson syndrome, which is a severe form of a rash known as erythema multiforme. The cause is what is known as a cell-mediated hypersensitivity reaction to a distinct immunological stimuli, which in simple terms is a form of allergy which often takes some days or weeks to develop. Although the illness can be very severe, especially at first, it usually settles over a period of about a month. Sometimes the rash comes back again at a later date. Drugs may trigger it Stevens-Johnson Syndrome can be triggered by various drugs, especially some types of antibiotics, such as those containing penicillin or, as is the case with co-trimoxazole, sulphonamides. Salicylates (such as aspirin) and barbiturate drugs can also cause it, as can viral infections (Herpes simplex, the cold sore virus is a notorious trigger), cancers and other general illness. It's important to try to identify the trigger so that it may be avoided in future but often it is never pinned down.

General Discussion
Stevens-Johnson Syndrome is a rare disorder characterized by inflammation of the mucous membranes of the mouth, throat, anogenital region, intestinal tract and membrane lining the eyelids (conjunctiva). Affected individuals may have abnormalities (lesions) of the skin and mucous membranes that are purplish or red in color. The abnormalities may be flat (macules) or small and raised (papules). In some cases, the lesions may develop raised fluid-filled centers (bullae or blisters). Affected individuals may also have blisters and/or bleeding in the mucous membranes of the lips, eyes, mouth, nasal passage, and genitals. In addition, abnormalities of the eyes may develop as a result of the lesions caused by Stevens-Johnson Syndrome (ocular sequelae). Such abnormalities may include infection of the delicate membrane of the eye and eyelids (conjunctiva) and inflammation associated with an abnormal discharge from the conjunctiva (purulent conjunctivitis).

Stevens-Johnson syndrome (SJS) is a severe and potentially life-threatening (15% of cases) disease , it is a hypersensitivity complex affecting the skin and the mucous membranes, a severe expression of erythema multiforme (and so SJS is also called erythema multiforme major SJS is characterized by fever, sore throat, and headache leading to the sudden development of circular mucocutaneous lesions that can cover the majority of the skin. These lesions begin as macules and can develop into papules, vesicles, bullae, or urticarial plaques. The most extreme cases are termed Toxic Epidermal Necrolysis Syndrome (TENS), in these cases the entire skin is affected. Treatment is initially similar to that of patients with thermal burns, and continued care can only be supportive and symptomatic, there is no specific drug treatment (2002). The cause of SJS is either infections (usually following viral infections such as herpes simplex virus influenza mumps , cat-scratch fever, histoplasmosis Epstein-Barr virus , or similar), drug-induced (penicillins, barbiturate , sulfas, phenytoin ), malignancy-related (carcinomas and lymphomas), or idiopathic (50% of the time)

Stevens Johnson Syndrome / Toxic Epidermal Necrolysis
Caused By Drugs Among the most feared complications of drug therapy are the exfoliative skin disorderserythema multiforme (EM), Stevens Johnson Syndrome (SJS) and toxic epidermal necrolysis (TEN). Both diseases, Stevens Johnson Syndrome (SJS) and TEN, are primarily, if not solely, caused by drugs among adults, and approximately 50% are drug-related among all ages. The most common drugs associated with Stevens Johnson Syndrome (SJS) and TEN are antibiotics, nonsteroidal anti-inflammatory drugs (NSAIDs) and anti-convulsants. Prescription NSAIDs, such as Bextra (valdecoxib) and Daypro (oxaprozin), are one of the leading classes of drugs that cause Stevens Johnson Syndrome (SJS) and TEN reactions. In many countries, increasing rates of skin eruptions are attributed to (NSAIDs). NSAIDs have played in increasing role in the etiology or cause of TEN, and it has been reported in the medical literature that some NSAIDs that have a long half-life carry a higher risk of Stevens Johnson Syndrome (SJS) or TEN. History In 1922, Stevens and Johnson described two children with fever and stomatitis (mouth ulcers), severe disseminated conjunctivitis (eye inflammation) and cutaneous (skin) eruptions. It soon became evident that severe forms of Stevens Johnson Syndrome (SJS) could lead to toxic epidermal necrolysis (TEN) and that both diseases could be induced by the same drugs.

C utaneous reactions are among the most common adverse drug events reported. Rashes are typically thought of as benign hypersensitivity reactions that resolve merely by discontinuing the offending agent. In most cases, this is true. However, there are some skin disorders that are dangerous and sometimes even fatal. One such disorder is Stevens-Johnson syndrome (SJS). There is great controversy over the true definitions of and distinctions between erythema multiforme, SJS, and toxic epidermal necrolysis (TEN). The terms are sometimes incorrectly used interchangeably. Erythema multiforme is most typically experienced subsequent to an infection, specifically herpes simplex virus (HSV) and mycoplasma. The skin lesions have a regular shape with a well-defined border and occur mostly on the extremities.

Erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis are mucocutaneous disorders that are thought to be related, constituting a spectrum of reactions. Erythema multiforme (EM) has the most benign presentation, and is followed in severity by Stevens-Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN). Additionally, EM is generally a postinfectious process, while SJS and TEN are drug-induced reactions. Legal Compensation for Arava Damages On March 28th, 2002 Public Citizen, a consumer protection group started by Ralph Nader, urged regulators to immediately ban the rheumatoid arthritis drug Arava, saying it had linked the product to 12 fatal cases of liver damage. According to the petition, there have been at least 130 cases of severe liver toxicity reported to the FDA, including 56 hospitalizations and 22 deaths. Liver toxicity induced by Arava is a suspected cause in 12 of these deaths.

My doctor wants me to try Lamictal. I'm afraid of Stevens-johnson syndrome. I'm a 40 year old female. Intial dose would be 25MG twice a day. Is it worth the risk?

In my experience, once I started using one quarter of that dose as the starting place (instead of the dose you cite, although that's what's in the manufacturer's info (or was; they are probably onto this by now, as most of my colleagues seem to have arrived at the same conclusion)) after that, I haven't seen hardly a single case of rash if my patient actually followed the instructions for ultra-slow dose increases. And I understand that some leading figures on this topic, like Dr. Laura Marangell (per a recent conference presentation) are now so comfortable with the rash risk that they don't even stop the medication if a person gets a bumpy, red, itchy spot on their body only if it's around the mouth or inside; if there are blood test changes; or if a person is feeling sick as well as having the rash. Otherwise, they just pull back on the dose for a bit and wait for the rash to go away. In other words, the risk of S-J syndrome is much lower now that we're using much slower dose increases. I saw 1/3,000 for the severe rash frequency recently, down from 1/1,000, for example. That doesn't make it less scary, perhaps, but it at least is less common than it was when we started this.
