Polycystic ovarian disease (PCO), also known by the name Stein-Leventhal syndrome, is a hormonal problem that causes women to have a variety of symptoms including:
  • Irregular or no periods
  • Acne
  • Obesity , and
  • Excess hair growth.
  • How is PCO diagnosed?
    The diagnosis of PCO is generally made on the basis of clinical signs and symptoms as discussed above. The doctor will want to exclude other illnesses that have similar features, such as low thyroid hormone blood levels ( hypothyroidism ) or elevated levels of a milk-producing hormone (prolactin). Also, tumors of the ovary or adrenal glands can produce elevated male hormone (androgen) blood levels that cause acne or excess hair growth, mimicking symptoms of PCO. Other laboratory tests can be helpful in making the diagnosis of PCO. Serum levels of male hormones (DHEA and testosterone) may be elevated. However, levels of testosterone that are highly elevated are not unusual with PCO and call for additional evaluation. Additionally, levels of a hormone released by the brain (LH) is elevated.

    Home Stein-Leventhal syndrome DESCRIPTION Stein-Leventhal syndrome is an accumulation of incompletely developed follicles in the ovaries. The condition may be characterized by irregular menstrual cycles, absent menses, multiple cysts on the ovaries, and infertility. Abnormal ovarian function sometimes causes incompletely developed follicles (ova) to accumulate in the ovaries. These ova fail to mature and ovulate, therefore causing them to accumulate as cysts in the ovary and contributing to infertility in the woman. Polycystic ovaries are two to five times as large as normal ovaries, and they have a white, thick, very tough outer covering. This condition is commonly called Stein-Leventhal syndrome, and usually develops shortly after puberty. A woman with polycystic ovaries will stop menstruating (she may not have ever started) or will menstruate erratically. She will gain weight, eventually becoming obese, and may develop excessive amounts of facial or body hair (hirsutism). Some women exhibit virilization.

Of these, an estimated 33% have polycystic ovarian syndrome, generally defined in the UK as polycystic ovaries together with one or more characteristic features (hirsutism, acne, male-pattern baldness, amenorrhoea or oligomenorrhoea, or raised serum concentrations of testosterone and/or luetinising hormone) (2,3). In polycystic ovarian syndrome the associated metabolic abnormalities (abnormal serum lipid concentrations and insulin resistance) also put some women at an increased risk of developing diabetes mellitus (4).

    What is polycystic ovary syndrome?
    Polycystic ovary syndrome (PCOS) is a disorder in which many benign cysts form on the ovaries under a thick, white covering. It is most common in women under 30 years old. The ovaries are glands located on either side of the uterus (womb) in a woman's lower abdomen. The ovaries produce the female hormones estrogen and progesterone. Before menopause, they also produce eggs. Ovarian cysts are fluid-filled sacs that result from ovulation cycles. Many ovarian cysts go away without treatment. The most common cysts are just enlargements of normal egg follicles. This disorder has also been called Stein-Leventhal syndrome.
    How does it occur?
    Polycystic ovary syndrome is caused by an abnormal production of two hormones by the pituitary gland in the brain. These two hormones are LH (luteinizing hormone) and FSH (follicle-stimulating hormone). Imbalance of these hormones prevents the ovaries from releasing an egg each month. The ovaries produce more of the male hormone testosterone. They continue to produce estrogen but not progesterone.

    Treatment: Medications used to treat the symptoms of Stein-Leventhal syndrome include oral contraceptives, spironolactone, flutamide, and clomiphene citrate. Treatment with clomiphene induces the pituitary gland to produce more FSH, which in turn stimulates maturity and release of the ova. Occasionally, more potent ovulation induction agents (fertility drugs, human menopausal gonadotropins) are needed for pregnancy. A "wedge resection" of the ovaries has been used in the past to remove cysts. Finally, weight reduction , which may be very difficult, is also very important. Maintaining general good health and eliminating the complications of obesity are helpful.
    Expectations (prognosis): Pregnancy may be achieved with appropriate medical intervention.

Causes, incidence, and risk factors: Abnormal ovarian function sometimes causes incompletely developed follicles (ova) to accumulate in the ovaries. These ova fail to mature and ovulate, therefore causing them to accumulate as cysts in the ovary and contributing to infertility in the woman. Polycystic ovaries are two to five times as large as normal ovaries, and they have a white, thick, very tough outer covering. This condition is commonly called Stein-Leventhal syndrome and usually develops shortly after puberty. A woman with polycystic ovaries will stop menstruating (she may not have ever started) or will menstruate erratically. She will gain weight, eventually becoming obese , and may develop excessive amounts of facial or body hair ( hirsutism ). Some women exhibit
