In order to treat pain, it is important to attempt to define it. Webster's Dictionary defines pain as a sensation of hurting or strong discomfort in some part of the body caused by injury, disease, or functional disorder and transmitted through the functional nervous system. Furthermore, it is the sensation that one feels when hurt mentally or physically, especially distress, great anxiety, anguish, grief, etc. The International Society for the Study of Pain Management defines pain as an unpleasant sensory and an emotional experience associated with actual or potential tissue damage or described in terms of such damage. Chronic pain is defined as pain which lasts for more than three months and is associated with significant impairment of activities of daily living and/or work activities. Thus, pain is not merely to be identified with nociception. Patients have affective responses to pain. Ongoing pain may result in different problems to different people. Functional impairments may vary depending upon the patient's individual physical and emotional response to pain as well as the responsibilities involved in their life and vocational activities. Goals in pain management certainly involve the complete remediation of the nociceptive etiology whenever possible. However, there are times when the focus is on managing the pain. Functional goals must be addressed. This may include activities of daily living, homemaking, recreation, sexuality, childcare and vocational issues. Treatment must be done with appropriate utilization of medical services. Appropriate medication usage is urged. Psychological support must be delivered when needed. At times, marital support may be required. The patient must be educated about the nature of their problem. Good biomechanics and ergonomics as well as a therapeutic exercise program is encouraged.

Alan Gardner MB BS FRCS(Eng), Consultant Orthopaedic Surgeon Spinal disability, particularly back pain, is the leading cause of long-term absenteeism from work in the UK and 1% of the adult population is permanently disabled by it. At least five million adults consult their general practitioner annually concerning back pain. The overall cost in health care in 1998 Was estimated to be £1.6 billion, to which must be added that of informal care and loss of work which totals £10.6 billion . This amounts to a back tax of approximately £205 per year for every individual in the UK. The scale of the problem is similar in every industrialised country.
How can this best be managed?
Most patients with low back pain can be treated by their general practitioner with the help of a physical therapist. Studies on the management of back pain at this level" have led to recommendations for a more aggressive policy in regard to treatment, with advice to stay active with suitable medication in the acute state'followed by a structured approach employing back schools, exercise therapy, the use of nonsteroidal anti -inflammatory drugs and a multidisciplinary programme for those with chronic pain who are well motivated, to encourage a return to work of some sort.

While recent advances in emergency care and rehabilitation allow many SCI patients to survive, methods for reducing the extent of injury and for restoring function are still limited. Immediate treatment for acute SCI includes techniques to relieve cord compression, prompt (within 8 hours of the injury) drug therapy with corticosteroids such as methylprednisolone to minimize cell damage, and stabilization of the vertebrae of the spine to prevent further injury. Sciatica: Pain radiating (travelling) into the buttock, back of the thigh, and often into the calf and foot. This is usually caused by irritation of a nerve root of the sciatic nerve, often from compression by a disc or degenerative disease. Radiculopathy: Dysfunction of a nerve root by any cause. The motor manifestations are weakness, while the sensory manifestations may consist of pain (sciatica), numbness, or tingling (paresthesias), or a combination of these. Mechanical Back Pain: Also called "back strain" or "musculoskeletal back pain." This diagnosis excludes anatomic sources of pain, such as disc herniation, spondylosis, etc. The causes may be multifactorial, including strain of the paraspinous muscles (the muscles along the spine), strain the the ligaments of the spine, degenerative facet joint disease (the joints between the bones of the spine), or others. Herniated Disc: Also called herniated nucleus pulposus (HNP), disc rupture, disc prolapse. This condition and its treatment are discussed later in this section. Spinal Stenosis (Spondylosis): Narrowing of the spinal canal, typically in the lumbar or cervical spine (lower back and neck, respectively). This is a complex problem requiring an individualized approach for each patient, by an experienced neurosurgeon or neurologist.
