The Southwest Georgia Regional Spine Center was created when Dr. Edward Hellman joined the orthopedic surgical practice of Southwest Georgia Orthopedics. Dr. Hellman is the first fellowship trained spine surgery specialist to begin practice in Albany. He is also a member of the North American Spine Society. The purpose in creating the center is to provide a high level of specialized spinal care to the residents of South Georgia and the Southeast. Dr. Hellman has been trained to treat disorders of the cervical, thoracic and lumbar spine in the pediatric through adult patient. His special interests include the treatment of spinal trauma, spinal deformity/scoliosis, and the use of minimally invasive techniques in spinal surgery in the pediatric through adult patient. He has also been trained in some of the newest advances in spinal surgery, including vertebroplasty for osteoporotic vertebral compression fractures, IDET for the treatment of degenerative disc disease, and percutaneous LASER disc surgery for contained disc herniations. SOUTHWEST GEORGIA REGIONAL SPINE CENTER DISC DISORDERS OF THE Intervertebral discs are the "cushions" of cartilage between the bony vertebral bodies in the cervical, thoracic, and lumbar spine. There existence allows the body to accept and dissipate load across multiple levels in the spine and still allow for the flexibility required for performing normal activities of daily living. As the body twists, bends, flexes and extends; the intervertebral discs are constantly changing their shape. The discs themselves are composed of a tough, outer fibrous layer (the annulus fibrosis) and a soft, almost gelatinous inner layer (the nucleus pulposis). As a part of the aging process, the discs begin to lose their high water content and their ability to dissipate force as efficiently (SO CALLED DEGENERATIVE DISC DISEASE). By the age of 35, approximately 30% of people will show evidence of disc degeneration at one or more levels. By the age of 60, greater than 90% of people will show evidence of disc degeneration at one or more levels on MRI. In some patients, this disc degeneration can be nearly asymptomatic; in others, disc degeneration can lead to intractable back pain. As the discs deteriorate, the spine can initially become less stable. Bony spurs can develop as a result of this instability and can cause pressure on nearby nerves leading to leg or arm pain. Narrowing of the neural canal by these bony spurs is known as degenerative spinal stenosis. The outer layer of the discs themselves can also tear. When this occurs, the inner, gelatinous layer can herniate out ( A "HERNIATED" OR "RUPTURED" DISC) and also cause pressure on an adjacent nerve. If the herniation occurs in the neck and causes pressure there, this can cause pain that radiates into the shoulder and arm; if it occurs in the back, the pain produced can radiate down into the hip and leg.

Webster’s Dictionary defines pain as a sensation of hurting or strong discomfort in some part of the body caused by injury, disease, or functional disorder and transmitted through the functional nervous system.Furthermore, it is the sensation that one feels when hurt mentally or physically, especially distress, great anxiety, anguish, grief, etc.The International Society for the Study of Pain Management defines pain as an unpleasant sensory and an emotional experience associated with actual or potential tissue damage or described in terms of such damage. Chronic pain is defined as pain which lasts for more than three months and is associated with significant impairment of activities of daily living and/or work activities.Thus, pain is not merely to be identified with nociception.Patients have affective responses to pain.Ongoing pain may result in different problems to different people.Functional impairments may vary depending upon the patient’s individual physical and emotional response to pain as well as the responsibilities involved in their life and vocational activities. Goals in pain management certainly involve the complete re-mediation of the nociceptive etiology whenever possible. However, there are times when the focus is on managing the pain.Functional goals must be addressed. This may include activities of daily living, homemaking, recreation, sexuality, childcare and vocational issues.Treatment must be done with appropriate utilization of medical services.Appropriate medication usage is urged.Psychological support must be delivered when needed.At times, marital support may be required.The patient must be educated about the nature of their problem.Good biomechanics and ergonomics as well as a therapeutic exercise program is encouraged.

There is no simple cause for back or neck pain. It is sometimes possible to trace your pain to an accident or wear and tear that has happened over time. The exact cause of a patient's lower back pain is often difficult for physicians to pinpoint because of the complex composition of the human spine. Bone, discs, muscles, ligaments, tendons and various other tissues are arranged like a three-dimensional puzzle to make up the spine. The complex make up can easily mask the exact cause of lower back pain. It can originate from identified muscle trauma, or an unknown non-traumatic event. Lower back pain can also begin in other regions of the body, such as the nerves or nervous system, and eventually attack the muscles or other structures in the lower back. Other causes of lower back pain include post-neural difficulties, congenital disorders, trauma, infections, degenerative disorders, inflammatory diseases, circulatory disorders, or a wide range of other causes. Neck pain and symptoms caused by a cervical (neck) spinal disorder are a common problem for many adult Americans. The cervical spine is composed of many different anatomic structures, including muscles, bones, ligaments, and joints. Each of these structures has nerve endings that can detect painful problems when they occur. The different parts of the cervical spine are normally well balanced and able to handle all the movements, stresses, and strains of the body gracefully. However, when the different parts of the cervical spine are injured or start to wear out, the neck can become a significant source of pain and discomfort.

Is There Such A Thing As Cervicogenic Headache ?
by Amanda J. O'Connor Can cervical spine disorders cause headache? The role of the cervical spine in the aetiology of headache remains controversial despite the fact that there is a substantial body of literature which supports a cervicogenic origin of headache. The notion that disease or dysfunction of structures in the neck may give rise to headaches is however, not new. Lance (1982) quotes a lovely passage by John Hilton written between 1860 and 1862: This description suggests a referred pain pattern from the upper cervical nerves (C2) which supply the scalp at the back of the head to the vertex, and the trigeminal nerve (the 5th nerve), which supplies the face and anterior scalp up to the vertex. For cervical spine disorders to cause head pain there must be i) pain sensitive structures in the neck ii) changes in the neck structures sufficient to cause stimulus to the pain receptors. iii) identifiable neurological pathways from the cervical spine to the head.
