The skeletal trunk that part of the body made up of the spine (back and neck), shoulders, and pelvis acts as the major support system for the rest of the body. Sometimes called the trunk or the torso, the skeletal trunk is home to weight-bearing bones and joints, muscles, tendons, ligaments, and nerve centers that make movement possible and allow flexibility for the rest of the body. Because of the complexity of its many tasks, the skeletal trunk is vulnerable to many disorders. Problems such as degenerative arthritis, nerve impingement, fractures, tumors disorders that can also cause pain and disability in other parts of the body are common to the skeletal trunk.

NIELSEN, M.D. The orthopedic surgeons' reliance and emphasis on x-ray procedures and surgery by nature makes him think of the spine as a focally or regionally injured organ. Actually, it's rare for a spine injured by sudden deceleration in an auto accident, lift or fall to receive its injury in one area. The x-ray changes suggesting that simply show the peak of injury, not the breadth. The patient frequently senses this better than the surgeon as he/she notes pain in remote areas where x-rays appear normal. The Prolotherapist like the Chiropractor and Osteopath sees the spine as an integral functioning unit with varying degrees of injury. The surgeon during informed consent cautions the patient that surgical fusion at one level may cause the adjacent disc space to degenerate. This is because the ligaments in the next segment are already injured and will now be put under even more movement and stress. When a patient undergoes disc removal and fusion (PILF) for leg and back pain, the surgeon cautions that he may relieve the leg pain and aggravate the back pain for leave it unchanged. This is because the surgery has done nothing or weakened the back ligaments.

CONGENITAL spine disorders, Congenital spinal disorders are due to a development defect in the mesenchymal anlage during the 4th to 6th week of gestation.

PAEDIATRIC SPINAL DISORDERS There are many variations. These have been divided and grouped into eight separate categories as shown below: IDIOPATHIC SCOLIOSIS This manifests as a lateral deviation and rotation of the spine with no identifiable cause, but it is usually related to a brain stem, hormonal or proprioception disorder. The adolescent form is most common but there is an Infantific form also a Juvenile form. Right thoracic curves, with apex T7 or T8 are the most common, followed by double major, left lumbar and right lumbar, in that order. Afflicted children are first noticed because of a trunk shift, shoulder or pelvic asymmetry, asymmetric umbilical reflex, spinal curvature, asymmetric rib hump and/or limb length inequality. Inclusion of the iliac crest on radiographs will reveal the degree of skeletal maturity. A lateral radiograph to check for spondylolisthesis is also recommended. An M.R.I. scan is needed if there is excessive Kyphosis, rapid progression, neurologic symptoms, onset before 11 years, of left thoracic/thoracolumbar curves. Treatment includes careful observation, some for bracing and if needed surgery.

Bursitis What is a bursa?
A bursa is a fluid-filled sac that functions as a cushion and gliding surface to reduce friction between tissues of the body. The major bursae are located adjacent to the tendons near the large joints, such as the shoulders, elbows, hips, and knees. When the bursae becomes inflamed, the condition is known as bursitis Bursitis is usually a temporary condition. It may restrain motion, but generally does not cause deformity. What causes bursitis?
The most common causes of bursitis are injury or overuse, although infection may also be a cause. Bursitis is also associated with other diseases, such as arthritis, thyroid disease, and diabetes. What are the symptoms of bursitis?
Bursitis can cause pain, localized tenderness and limited motion. Swelling and redness may occur if the inflamed bursa is close to the surface (superficial). Chronic bursitis may involve repeated attacks of pain, swelling, and tenderness, which may lead to the deterioration of muscles and limited range of motion.

Introduction
At one time or another, most people have experienced neck pain. In the vast majority of cases, this is a benign, self-limited complaint. Symptoms are commonly described as a soreness or stiffness of the neck, which may or may not be associated with a minor injury. Patients often attribute this to a "cold wind" or "sleeping wrong" that may or may not be a factor. Most commonly, degenerative disorders of the spine are responsible. Degenerative "disease" is the changes associated with spinal wear and tear or age. Though among the most frequent causes of neck pain, it is the sometimes the most difficult to treat. Other possible disorders that can cause neck pain are rheumatoid arthritis (room-ah-toyed arth-rye-tis), infection, or cancer. It is rare that such conditions cause only neck pain, as they are usually associated with other warning signs such as profound or unintentional weight loss, fevers, or pain in other joints such as the hip or knee. The focus of this chapter is a discussion of degenerative disorders of the cervical spine. These affect adult patients of any age, with a tendency for particular disorders to affect certain age groups. Cervical disk herniations are more characteristic in the young (less than forty-years old), while cervical spondylosis (spon-dee-low-sis) and stenosis (sten-oh-sis) are typically found in older patients. Treatments vary from observation, medications, and therapy to injections or operative intervention.

    Abnormal Curvature of the Spine
    Poor posture, a congenital abnormality, injury, infection, and other diseases, such as cerebral palsy, can cause abnormal curvature of the spine. The most common kinds of abnormal curvature are lordosis, flat back, kyphosis and scoliosis. All these disorders can be treated with therapy, exercise, and, in some cases, surgery. Normal Spine Lordosis Flat back Kyphosis Scoliosis Lordosis , the inward curvature of the spine, is normal to a slight degree in the neck and lower back, but it can become exaggerated by poor posture or by kyphosis. Excessive lordosis in the lower back often occurs in someone who is overweight and has weak abdominal muscles. Once excessive lordosis has developed, it can become a permanent condition and may contribute to the development of osteoarthritis of the spine. Flat back , the absence of normal spinal curves, is the result of abnormal development in infancy or injury.
