Vertebra and Spinal Nerves Spinal cord trauma can be caused by any number of injuries to the spine that can result from motor vehicle accidents, falls, sports injuries (particularly diving into shallow water), industrial accidents, gunshot wounds, assault, and others. A seemingly minor injury can cause spinal cord trauma if the spine is weakened (such as from rheumatoid arthritis or osteoporosis
Direct injury, such as cuts, can occur to the spinal cord, particularly if the bones or the discs have been damaged. Fragments of bone (from fractured vertebrae, for example) or fragments of metal (such as from a traffic accident) can cut or damage the spinal cord. Direct damage can also occur if the spinal cord is pulled, pressed sideways, or compressed. This may occur if the head, neck, or back are twisted abnormally during an accident or injury.

Basic Principles of Spinal Cord Compression Joane Parent Canada Spinal cord diseases are frequent in the dog. To review the basic principles of lesion localization as it relates to the spinal cord, intervertebral disc disease has been used as model. In IVDD, there is spinal cord compression without instability. The spinal cord is composed of an outer layer of white matter and a core of gray matter. The white matter is made of axons while the gray matter is composed of cell bodies. Clinically, lesions that are limited to the gray matter behave similarly to lesions of the peripheral nervous system. It is the involvement of the white matter that leads to clinical signs of spinal cord disease. Spincal cord compression
Cord compression by spinal tumour deposits is a serious occurrence and urgent action is required to prevent the development of paraplegia. Although generally an uncommon symptom, spinal cord compression can be a common complication in metastatic prostate cancer. Symptoms include the sudden onset of back pain or exacerbation of existing back pain together with leg weakness, altered or painless loss of sensation, leg pain and difficulty passing urine. Any patient with suspected spinal cord compression should be admitted as an emergency for investigation and treatment. Once paralysed, only 5% walk again. 30% of patients survive to 1 year. Action should be taken, irrespective of neurological findings. Dexamethasone 4mg q.d.s. should be started immediately to reduce oedema around the tumour and the patient sent for urgent magnetic resonance imaging of the spine. Treatment options are surgical decompression or radiotherapy. *Prostate cancer patients treated with LHRH agonists (buserelin, goserelin, luprorelin or triptorelin) should be carefully monitored for signs of spinal cord compression during the first month of treatment. Use of an anti-androgen for 3 days prior to and 3 weeks after the commencement of LHRH treatment is essential for all patients.

Spinal Cord Compression The symptoms and signs of spinal cord compression depend on the length of time for which it has been present. Patients with metastatic disease should be encouraged to report ant suspicious symptoms as early as possible in order to prevent irreversible damage.
  • Early : vertebral pain Late : sensory changes, motor weakness Very late : sphincter disturbance.
Any patient suspected of developing spinal cord compression should be referred as an emergency for investigation ( MRI is investigation of choice) and treatment. Therapeutic options include With early treatment, 70% of patients retain the ability to walk. Once paralysed, only 5% walk again. The management of paraplegia is more complex. Rapid onset complete paraplegia (over 24-36 hours) has a poor prognosis. It is often caused by infarction of the spinal cord secondary to tumour compression and thrombosis of a spinal artery. It can also be as a result of infiltration of the dura by tumour. The prognosis is better in patients who have not received hormone treatment.

Researchers at the University of Maryland have been working with a group of 101 patients who have cancer that causes compression of the spinal cord. Some of the patients received radiotherapy alone, while others had surgery to relieve the spinal compression and remove as much tumor as possible. Sixteen patients in each of the sub-groups were unable to walk at the start of the study. More of those in the surgery plus radiotherapy group regained the ability to walk compared to those receiving radiotherapy alone. Those receiving surgery also tended to survive for longer. Mobility is important to such patients - they may not, always, have much longer to live. Being able to walk keeps them relatively independent and improves quality of life. This study suggests that surgery is a worthwhile way of helping them. Source
