: evolution :
...Two weeks after birth I underwent the first of many operations that were to come- a ground breaking procedure in which the neurosurgeons opened my back up by a horizontal incision on the small of my back from one side to the other, unraveled a "tumor" full of knotted nerves at the base of my spine and "plugged" each nerve back into my spine... : scar tissue :
...I stood in front of the many mirrors in my bathroom, naked, and studied all of my scars. I looked at them, touched them, saw them. I never do this because it makes things too real, because I can recall every scar, when it was made. If I don't look at the scars, then I can escape from everything and pretend that that is another girl. That's another person, another life, another set of emotions : images :

... I think understanding comes only when you realize that depression is its own living and breathing thing, and that it has its own free will, and there is very little you can do to intercept it or prevent it. The scariest part of it is when you come to terms with those facts. It's so hard when you realize you have nothing in your current life to "blame" it on. Nothing you can fix to make it all better. It's just there, lurking...

Spina bifida
Spina bifida also known as myelomeningocele means cleft spine, which is an incomplete closure in the spinal column. It is a congenital disorder that occurs when one or more vertebrae do not develop properly from the neural tube. Spina bifida is usually an isolated birth defect. Most scientists believe that genetic and environmental factors work together to cause this defect and other Neural Tube Defects. The defect usually occurs within the first month of pregnancy. It affects 0.5 to 1 out of 1,000 infants born in the United States.

Alternate Names : Spina Bifida Definition Myelomeningocele is a congenital disorder (birth defect) where the backbone and spinal canal do not close before birth. This can result in the spinal cord and its covering membranes to protrude out of the infant's back.
Spina Bifida
Spina Bifida (Degrees of Severity) Myelomeningocele is one of the most common birth defects of the central nervous system . It is a neural tube defect in which the bones of the spine do not completely form, and the spinal canal is incomplete. This allows the spinal cord and meninges (the membranes covering the spinal cord) to protrude out of the child's back. Spina bifida includes any congenital defect involving insufficient closure of the spine. Myelomeningocele accounts for about 75% of all cases of spina bifida and may affect as many as 1 out of every 800 infants. The rest of the cases are most commonly spina bifida occulta (where the bones of the spine do not close, the spinal cord and meninges remain in place, and skin usually covers the defect) and meningoceles (where the meninges protrude through the vertebral defect but the spinal cord remains in place).

Fetal Surgery for Spina Bifida November 18, 1999 Spina bifida is a relatively common birth defect which occurs in 1 to 2 of every 1,000 babies born in the United States. "Spina bifida" literally means "split spine"; it is caused by failure of the spinal cord to develop properly. The nervous system develops from embryonic tissue called the ectoderm. The first sign of the developing nervous system is the neural plate which can be seen at about the 16th day of development. Over the next few days, a "trench" is formed in the neural plate - this creates a neural groove. By the 21st day of development, a neural tube is formed when the edges of the neural groove meet. The rostral (front) part of the neural tubes goes on to develop into the brain and the rest of the neural tube develops into the spinal cord. Spina bifida occurs when the neural tube is not formed properly; the bones of spinal column (the vertebrae) and muscle fail surround the spinal cord properly. No one knows exactly what causes spina bifida, but it is thought that both environmental and genetic factors play a role. Development of the Nervous System
Skull and
Vertebrae Failure of the neural tube to develop properly may result in three different conditions that lead to spina bifida:
  • spina bifida occulta - the mildest form of spina bifida in which one or more vertebrae are malformed. These vertebrae are covered by a layer of skin.

    Spina Bifida
Spina (spine) bifida (split or divided) is a "condition resulting when the bones of the spine form abnormally and fail to unite in the midline of the back" (Woodburn, 1975). If this split occurs, the spinal cord and its coverings may bulge out through the opening and cause a lump on the back. This malforming of the spine takes place within the first 28 days of pregnancy and varies in severity from a simple lump on the back to severe malformation of the back and lower limbs. Professor Nicholas Tulp gave spina bifida its name and gave the first formative description of the disability. Although the term and real medical description are somewhat recent, it is estimated that spina bifida occurred almost 12,000 years ago. It is also worth noting that it wasn't until 1761 that a man named Morgagni linked lower-limb deformity and hydrocephalus (an excessive amount of cerebro-spinal fluid within the head) with spina bifida. As a result of this finding, much has been done to find a "cure" for hydrocephalus, one of the major problems associated with spina bifida. One such discovery was in the late 1950s; the Spitz-Holter valve and the ventriculatrial shunt (a bypass or replacement of a major cavity between the brain and spinal cord implanted to control the flow of the cerebro-spinal fluid, so as to prevent pressure on the brain). Because of this find and other developments, an increasing number of children survive the more severe forms of spina bifida. Of these children, about 70 percent have normal intelligence (Menelaus, 1971).

    What causes physical disability?
    Spina Bifida
Spina Bifida is a developmental defect which occurs within the first six weeks of pregnancy, caused possibly by a combination of genetic and environmental factors. Some of the spinal bones which normally protect and cover the delicate nerves of the spinal cord do not close. Consequently the spinal nerve tissue protrudes through this gap and forms an open swelling on the back. This could occur anywhere from the back of the head to the lowest part of the spine. Even though surgery at birth closes the gap, the damage to the spinal nerve tissues can never be repaired. The degree of paralysis will depend on the extent of damage to the spinal cord and the position of the lesion. Usually there is some paralysis and loss of sensation in the legs and lower trunk. This could result in:
    • a need to use a mobility aid such as callipers, crutches, a walking frame, walking sticks or a wheelchair various degrees of bowel and bladder incontinence. There are several methods available to manage this problem.

    http://www.westworld.com/~barbara/index28spinabif
    Spina Bifida
from the National Information Center for Children and Youth with Disabilities (NICHCY) Definition
Spina Bifida means cleft spine, which is an incomplete closure in the spinal column. In general, the three types of spina bifida (from mild to severe) are:
Spina Bifida Occulta: There is an opening in one or two of the vertebrae (bones) of the spinal column without apparent damage to the spinal cord. Meningocele: The meninges, or protective covering around the spinal cord, has pushed out through the opening in the vertebrae in a sac called the "meningocele." However, the spinal cord remains intact. This form can be repaired with little or no damage to the nerve pathways. Myelomeningocele: This is the most severe form of spina bifida, in which a portion of the spinal cord itself protrudes through the back. In some cases, sacs are covered with skin, in others, tissue and nerves are exposed. Generally, people use the terms "spina Bifida" and "myelomeningocele" interchangeably. Incidence
Approximately 40% of all Americans may have spina bifida occulta, but because they experience little or no symptons, very few of them ever know they even have it. The other two types of spina bifida, meningocele and myelomeningocele, are known collectively as "spina bifida manifesta," and occur in approximtely one out of every thousand births. Of these infants born with "spina Bifida manifesta," about 4% have the meningocele form, while about 96% have the myelomeningocele form.
