Sore throats can be painful and annoying. Fortunately, most sore throats are caused by a minor illness and will go away without medical treatment. Home treatment often gives relief. Many things can cause a sore throat. A mild sore throat is often caused by:

“Cheering at the game was great, but my voice was hoarse and my throat was sore the next couple of days.” Chris B., Duke University Sore throats are common complaints of college students. The soreness can range from a mere scratch to severe pain.
Soreness or pain in the throat, especially when you talk or swallow Swollen neck glands The back of the throat and/or the tonsils look bright red or have pus deposits or white spots. Sores on the roof of the mouth You may have other symptoms with the sore throat, too. These include: Fatigue, fever, postnasal drip, bad breath, headache, and/or earache.
Causes
A bacterial or viral infection, such as strep throat, tonsillitis, or mononucleosis Shouting for long periods of time, such as from cheering at a sporting event Tobacco or marijuana smoke Air pollution, dry air and/or allergies Postnasal drip Self-induced vomiting Acquiring an infection from oral sex with an infected partner
Treatment
If an infection is suspected, your health care provider may take a throat culture. If streptococcus or any other type of bacteria is present, he or she may prescribe an antibiotic. Be sure you take all of the antibiotic, even if you are feeling better. If strep throat is inadequately treated, other conditions, such as rheumatic fever can occur.

Does your child's sore throat mean a dose of antibiotics? Only if it's a proven case of strep say the experts.
Does your child have a sore throat? While viruses cause most sore throats, the streptococcus bacteria, otherwise know as strep, usually cause those that are accompanied by headache and fever. Thankfully, in this day and age of antibiotics, strep throat is not nearly the dreaded illness that it once was, says pediatrician, Dr. Diane Sacks. "In the past, 50 or 60 years ago, it was a very serious infection that was feared because it led to serious heart problems, rheumatic fever or problems in the kidney. Fortunately, penicillin came along and it killed strep very effectively."
Dr. Sacks adds that strep can still lead to scarlet and rheumatic fever if left untreated, so if your child has a very severe sore throat and swollen glands, the doctor needs to take a throat swab, and if it turns out positive, medicine must be taken for its full coarse of treatment to prevent some of the secondary side effects."
Still, don't assume that your child will be prescribed antibiotics even when it appears to be strep. Overuse of antibiotics has been blamed for the development of some strains of antibiotic resistant bacteria. Therefore, most doctors won't prescribe them unless the signs of strep are very clear and the swab is positive, says pediatrician Dr. William Feldman, editor of "The 3AM Handbook". He adds "people are developing guidelines that predict strep throat a little better. Now if the child with a sore throat, has a runny nose, no fever and there's nothing to see in the throat, then the chances of this being strep throat are quite remote."

  • There are few good-quality studies of the effectiveness of antibiotic treatment of proven group A streptococcal (GAS) pharyngitis in children; available data suggest that antibiotics may reduce symptom duration. While there is limited justification for antibiotic treatment of GAS pharyngitis to prevent acute rheumatic fever in non-Indigenous Australians, there is no justification for routine antibiotic treatment of all patients with sore throat. Two strategies are open to clinicians: not to treat GAS pharyngitis with antibiotics, in which case no investigations should be done; or to treat cases of sore throat with clinical features that suggest GAS, in which case diagnosis should be confirmed with a throat swab, and penicillin started while awaiting the result. Penicillin should be discontinued if the swab is negative, or continued for 10 days if it is positive for GAS. Surveillance of GAS infections and acute rheumatic fever is needed in Australia, as are further studies of effectiveness (including cost-effectiveness) of antibiotic treatment of proven GAS pharyngitis.

It may start as a scratchy dryness. Or perhaps you wake up raw, swollen, barely able to swallow. Sore throats are so common that we often end up just popping a lozenge and trying to ignore them. Yet all too frequently, that itchy, burning sensation lingers or returns every time life gets a little stressful. Why? Many sore throat sufferers overlook the real reason for their pain. You wont recover until you pinpoint the exact cause of yours. The Stress Sore Throat
WHAT IT FEELS LIKE: You seem to get a sore throat monthly, accompanied by a low-grade fever, congestion and general malaise.
WHAT'S REALLY GOING ON: Stress may not cause a sore throat, but it sure makes you more vulnerable to common viruses. If you are in school, you're getting hit doubly hard because you're continually trading germs with classmates and roommates. Colds cause throat pain by infecting your throat tissues and by producing congestion that drips down your throat.
HOW TO TREAT IT: You've heard it a hundred timesget some rest. But who can really afford to? In lieu of eight hours a night, make sure you wash your hands frequently to reduce your exposure to hitchhiking germs. For temporary pain reliefs, gargle with a mixture of one teaspoon of salt in one cup of warm water, drink steaming liquids such as tea with honey, take acetaminophen as needed, and try an herbal tea containing a throat-soothing ingredient like slippery elm, lavender, or comfrey. If your throat pain lingers after the cold is gone, you may have a bacterial infection, which requires antibiotics.

Background
    Other organisms can cause pharyngitis. Of the beta hemolytic streptococci, group C represents the second most important cause. Few other bacteria frequently cause pharyngitis; however, viral infections are a very common cause. In order to decrease unnecessary antibiotic use, we would like to quickly and appropriately make decisions about patients with acute pharyngitis. Two recent systematic reviews of this topic have been published, one in the Rational Clinical Examination Series and one in the Clinical Practice Guideline Series by the American College of Physicians-American Society of Internal Medicine. The rational clinical examination series examined a number of articles and looked at the likelihood of strep throat infections and the presence of individual clinical findings. They also looked at and recommended several clinical prediction rules as having both good accuracy and good validation. Their analysis suggests that well validated clinical prediction rules can help physicians make informed use of both rapid antigen tests and throat cultures.

LONDON (Reuters Health) - A potentially deadly disease that begins as a sore throat is making a comeback in the UK, possibly because doctors are being told not to prescribe antibiotics for throat infections, scientists said on Monday.
Lemierre's disease was common in the early 20th century but largely disappeared with the advent of antibiotics. The symptoms are a sore throat followed by infection in the lungs and liver and septicaemia (blood poisoning). The disease is fatal if not treated.
Scientists with the Public Health Laboratory Service (PHLS) see an average of about 20 infections a year in England and Wales. But 37 cases were identified 1999, and higher numbers were seen in 2001, researchers said at the service's annual conference on Monday.
Dr. Jon Brazier, from the PHLS Anaerobe Reference Unit in Cardiff, said guidelines issued in 1999 urging general practitioners (GPs) not to use antibiotics for sore throats might be partly responsible.
"We cannot be sure, but it may be that this recent increase is due to a reluctance to prescribe antibiotics," Brazier said.

    Children less than 2 years of age usually don't know how to complain about a sore throat. A young child who refuses previously enjoyed foods or begins to cry during feedings may have a sore throat. Their symptoms are captured by the DRINKING FLUIDS, DECREASED guideline.
Cause
  • Most sore throats are part of a cold. Of severe sore throats, about 20% are caused by the Strep bacteria. Only these need an antibiotic.
