Sore throat
Sore throat refers to the inflammation of the pharynx or back of the throat .It occurs frequently when a person has a cold or an attack of influenza. This inflammation may also involve the tonsils and adenoids. Symptoms:
In the case of an acute sore throat the patient complains of pain, irritation, and inflammation in the throat, followed by chillis, fever, and some hoarseness or laryngitis. The lymph gland along the side of the neck may become swollen and tender. The back of the throat may become very red and even covered with a grayish white membrane .the patient may find difficulty in swallowing, especially during the acute stage. There may also be some postnatal discharge if the inflammation has spread to the nasal passage. The main causes of a sore throat are common cold and influenza. Other causes include sinisistis, measles, diphtheria and even leukemia in rare cases.

There are many causes for a sore throat ( medical term is pharyngitis ) Most cases of pharyngitis will resolve on their own with simple, conservative measures. Most cases DO NOT require antibiotics. CAUSES
  • Post nasal drainage from a cold or allergy can cause an irritated throat, or mouth breathing from nasal congestion may cause dryness, which makes the throat feel sore
  • Environmental causes include smoke, overuse or sexually transmitted diseases.
  • Streptococci bacteria causing STREP THROAT can be confirmed by a throat culture, and treatment will require antibiotics a very red throat with exudate and fever can accompany strep throat.
  • MONONUCLEOSIS may cause a sore, swollen throat which may be accompanied by a fever, fatigue or swollen lymph nodes.
TREATMENT:
  • Gargles of warm salt water (1/2 teaspoon of salt in 8 ounces of warm water) frequently (every 1-2 hours) can help remove drainage and ease pain.

LENEXA, Kan., Dec. 15, 1998 Take it from a sore throat lozenge maker, there are times when you need more than relief of symptoms, such as the one case in four when a sore throat may be caused by strep bacteria. "A strep throat usually comes on more quickly and the symptoms are more severe than in the garden variety sore throat," says Borchers, whose company manufacturers Cough-X(R) lozenges, which are good for either type of sore throat. "Also, in the case of strep, lymph nodes in the neck, below the ears, and under the jaw may be significantly enlarged and tender to the touch. There may also be a white or yellow coating at the back of the throat and a fever of 101 degrees or higher." The best course of action if you suspect strep is to see your doctor for a throat culture. Strep is usually treated with antibiotics. If the throat culture turns up no bacteria, you probably have a viral sore throat, and treatment can be limited to relieving symptoms. For pain, gargle with a weak salt-water solution and suck on lozenges such as Cough-X that contain a topical anesthetic.

Sore throat (pharyngitis) is very common. It is usually caused by a virus infection. Soreness in the throat may be the only symptom. In addition, you may also have a hoarse voice, mild cough, fever, headache, feel sick, feel tired, and the glands in your neck may swell. It may be painful to swallow. Symptoms typically get worse over 2-3 days and then gradually go, usually within a week. You may also develop a sore throat if you have a cold or flu-like illness. Tonsillitis is an infection of the tonsils at the back of the mouth. Symptoms are similar to a sore throat, but may be more severe. In particular, fever and generally feeling unwell tend to be worse. You may be able to see some pus which looks like white spots on the enlarged red tonsils. What is the treatment for sore throat and tonsillitis?
  • Not treating is an option as many throat infections are mild and soon get better.

These lozenges release a weak topical anesthetic as they dissolve. The effect of this is that the anesthetic will sometimes numb everything in your mouth, especially your tongue. Examples of this type of medication include:

    The Medical Post, May 21, 1996 New York It's not all in their heads. Parents who suspected their child's strep throat caused subsequent development of obsessive compulsive (OC) or tic disorders can rest easy now that research finally confirms this relationship. The mounting evidence pointing to a link between group A Beta-hemolytic streptococci and these disorders could eventually lead to a means of identifying children at risk and the use of preventive therapy. The research is so convincing that Dr. Susan Swedo, acting scientific director at the National Institute of Mental Health (NIMH) and head of the NIMH's section on behavioral pediatrics, said she believes a throat culture is in order for both the child and family members when a child presents with acute onset or exacerbation of OC or tic symptoms. "Our studies and others on obsessive compulsive disorder are truly proof that these are neurobiological illnesses, that what was previously thought to be due to punitive toilet training is now known to be associated with changes in your brain chemicals, patterns of responsiveness of glucose metabolism, and now perhaps to be triggered by an autoimmune reaction." The evidence shows in susceptible children, the strep infection triggers the autoimmune response, which affects the basal ganglia and can lead to symptoms of OC or tic disorder including

    Tonsillectomy for sore throats
    Tonsillectomy is a common operation, with over 70,000 being carried out every year. The main reason for doing it is to prevent recurrent throat infection - either tonsillitis or pharyngitis. How well does tonsillectomy work?
    Review
    A systematic review [1] searched widely and obtained some unpublished information. The entry criteria were participants suffering from recurrent throat infections, the operation was tonsillectomy or adeno-tonsillectomy, and studies needed to be randomised.
    Results
    Five studies were found, two of which had strict entry criteria related to frequency of throat infections and defining features of a throat infection. The other three had less explicit criteria. No trial included children over 15 years. The main outcome was that of incidence of throat infection during two years of follow up. For all throat infections the results for years 1 and 2 are seen in the Figure. Over the two years, tonsillectomy meant avoiding 2.3 to 3.6 throat infections. Of these infections, perhaps 10-30% would be likely to be moderate or severe. More infections occurred in control groups in the first than in the second year. One study had a three year follow up, by which time the benefits of tonsillectomy had been lost. There was no statistical significance in school days lost between treated children and controls.

    A Word about Fevers
    ... Sulphur It is every parent's worst nightmare. Your child is in pain and you suspect an infection. Acute earaches and sore throats can hurt a lot. The younger the child and the more severe the pain the more upsetting it can be for parents and child.
    This is when your nurturing skills will be called upon, when your ability to be calm and reassuring and comforting may be stretched to the limit. Maybe at a time when you feel frightened and powerless, especially if this is new to you — if this is your baby's first illness — or if your child is very distressed.
    Many parents who turn to their doctors for help are prescribed antibiotics for the infection and Tylenol for the pain and to bring the fever down. Unfortunately, antibiotics will only 'work' for bacterial infections and since so many colds, coughs, earaches and sore throats are viral in origin, this type of treatment is not going to be effective.
    Homeopaths are concerned about the effects of repeated courses of antibiotics, especially on a child's developing immune system. Because many cases of infection will subside within 24 hours, especially earache (whatever treatment is given) some doctors are now adopting a wait-and-see policy for children with minor, acute infections and are willing to support parents who choose TLC and alternative medicine over antibiotics.     SORE THROAT
    Is the patient under 6 years old?
    Have there been symptoms for more than 2 days without improvement?
    Is there a persistent fever of over 102 degrees Fahrenheit?
    Are there "pus pockets" on the tonsils?
    Are there tender, enlarged lymph nodes ("glands") in the neck?
    Are you coughing up stuff from your chest?
    • If yes to any of the above, the patient should be examined by a physician promptly.
    • If no , then follow the instructions below.
    ACTIVITIES: Antibiotics are only helpful if you have a strep throat . It is impossible to make the diagnosis of strep throat without a lab test. Most sore throats are caused by viruses; the illness will just have to run its course. You may use over-the-counter medications, such as Tylenol Chloroseptic DIET:
    CONTACT YOUR DOCTOR FOR ANY OF THE FOLLOWING:
    1. The temperature is over 103 degrees Fahrenheit.

    Sore Throat
    While most parents are worried about strep throat when their child has a throat infection, their are also many viruses that cause infections that look very similar to strep. If your child has a sore throat with fever and a red, swollen throat or tonsils with white pus on them, then he should be seen by his physician so that he can be tested for strep throat. If the tests for strep are negative, then your child's throat infection is caused by a virus and antibiotics will not work. Viral infections of the throat usually improve in two to three days without treatment.
    • Strep throat : caused by the bacteria group A streptococci. It is most common in children over three years old and begins with a fever, a red, swollen throat and tonsils that can have a white coating of pus, swollen glands, decreased appetite and energy level. If strep throat is suspected as the cause of tonsillitis in your child, your doctor will do a throat swab to confirm that there is infection with strep bacteria. This infection is easily treated with antibiotics, usually penicillin. Your child will no longer be contagious after being on an antibiotic for twenty four hours. It is important to take a complete course of antibiotics to prevent your child from getting rheumatic fever.

    What causes a sore throat?
    Many things can cause a sore throat such as infections with viruses or bacteria, or sinus drainage and allergies, among others. You should see your provider right away if you have a sore throat with a high fever, if you have problems breathing or swallowing, or if you feel very faint. If you have a sore throat and a fever, but you just feel mildly ill, you should visit your provider within the next day or two. If you have a cold with sinus drainage, you may use over-the-counter medicines, like Sudafed.
    How does the provider decide if I need antibiotics?
    The decision to prescribe antibiotics might be based only on your history and physical exam. Antibiotics usually are prescribed only for patients who might have "strep throat," an infection caused by a bacteria called Streptococcus. A patient with strep throat might have a sore throat with fever that starts suddenly, without a cough or cold symptoms. The exam might show a red throat, with pus on the tonsils and swollen neck glands. If you have these signs, your provider may do other tests to see if you need an antibiotic.
