Somatoform disorders
Definition
The somatoform disorders are a group of mental disturbances placed in a common category in the fourth (1980) edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) on the basis of their external symptoms. These disorders are characterized by physical complaints that appear to be medical in origin but that cannot be explained in terms of a physical disease, the results of substance abuse, or by another mental disorder. In order to meet DSM-IV 's criteria for a somatoform disorder, the physical symptoms must be serious enough to interfere with the patient's employment or relationships, and must be symptoms that are not under the patient's voluntary control. It is helpful to understand that the present classification of these disorders reflects recent historical changes in the practice of medicine and psychiatry. When psychiatry first became a separate branch of medicine at the end of the nineteenth century, the term hysteria was commonly used to describe mental disorders characterized by altered states of consciousness (for example, sleepwalking or trance states) or physical symptoms (for example, a "paralyzed" arm or leg with no neurologic cause) that could not be fully explained by a medical disease.     Conversion Disorder
    • At least one symptom or deficit of sensory or voluntary motor function suggests a neurological or general medical condition. It is not limited to pain or sexual dysfunction. Appropriate investigation does not identify a neurological or general medical condition or the direct effects of substance use that can fully explain it. Conflicts or other stressors that precede the onset or worsening of this symptom suggest that psychological factors are related to it. The patient doesn't consciously feign the symptoms for material gain (Malingering) or to occupy the sick role (Factitious Disorder). It is not a culturally sanctioned behavior or experience. It is serious enough to produce at least 1 of:

    A. Psychiatric Morbidity
    Psychiatric morbidity due to service during the Gulf War was predicted to be low for several reasons: the short duration of the conflict, the relatively low casualties sustained by American forces, and the positive support for the war at home. Examination of records of evacuation during the conflict is one approach to examining the extent of psychiatric morbidity: the Army rate of evacuation for psychiatric reasons translated to only 2.7 per 1,000 evacuations per year.[ ] This very low rate of psychiatric evacuations is in contrast to prior wars in which evacuations for psychiatric disorders in comparison to total evacuations were: 23% in World War II, 10% for Korea, and 7% from Vietnam[ ]. Of the roughly 250 Army personnel evacuated from the Gulf for psychiatric reasons, approximately fifty (20%) were later determined to be disabled for further military service; levels of Post Traumatic Stress Disorder (PTSD2) were found to be very low, with only four of these 50 carrying a diagnosis of PTSD [ ]. Another approach to assessing psychiatric morbidity possibly relating to service in the Gulf is to examine the numbers of service members referred for disability determination due to psychiatric disorders. As of March 1994, approximately 294 soldiers with psychiatric-related diagnoses were referred for disability determinations. Of these, 112 carried the diagnosis of PTSD.[

Children and adolescents with physical illness have a progressive course, with deteriorating school performance, visual loss, fatigue, posture abnormalities, and a variety of symptoms that suggest a medical disorder. Psychogenic disorders, on the other hand, usually have a rapid onset, with a family history of similar presentations, a clear discrepancy between the physical findings and the patient's complaint, and some conscious or unconscious motivation for the maintenance of these symptoms (see Table 1 ). Patients have an inappropriate level of disability and distress and increased utilization of health care services.

    Somatization is where mental factors such as stress cause physical symptoms. Somatoform disorders are a severe form of somatization where physical symptoms can cause great distress, often long-term. However, people with somatoform disorders are usually convinced that their symptoms have a physical cause.
    Somatization What is somatization?
    When physical symptoms are caused by mental or emotional stress it is called 'somatization'. For example, many people have occasional headaches caused by mental stress. But, stress and other mental health problems can cause many other physical symptoms such as: chest pains, tiredness, dizziness, back pain, feeling sick, diarrhoea, period problems, etc. The term 'psychosomatic' means something similar to somatization, but includes other things. See separate leaflet called 'Psychosomatic Disorders'. How can the mind cause physical symptoms
    The relationship between the mind and body is complex and not fully understood. When we 'somatize', somehow the mental or emotional problem is expressed partly, or mainly, as one or more physical symptoms. However, the symptoms are real and are not imagined. You feel the pain, have the diarrhoea, etc. How common is somatization?

Included in this group are somatization disorder, conversion disorder, and body dysmorphic disorder. Hypochondriasis and chronic pain disorder (somatoform pain disorder) are also classified as somatoform disorders, but are covered in separate topics.
    • Somatization disorder: multiple physical symptoms that recur over a period of several years with no medical basis. The patient has usually consulted numerous doctors for treatment and medications. This disorder was previous called hysteria. The symptoms often involve neurological problems (double vision), gynecological problems (painful menstruation) or gastrointestinal problems (abdominal pain).
    • Conversion disorder: loss or change of a physical function (paralysis of an arm, blindness, seizures). The symptom is not intentionally produced or faked.
    • Body dysmorphic disorder: preoccupation with an imagined defect in a normal-appearing person. The defect may be very minor, but the concern is blown out of proportion. The focus is often on skin wrinkles and blemishes, facial hair, shape of the nose, mouth or jaw.

    Psychosomatic Illness (Somatoform Disorder) Description
    Sometimes a child or adolescent continually complains of a discomfort or a pain for which a physician cannot find a cause. The pain or the discomfort, however, is very real to the youngster. The child or adolescent may be suffering from a psychosomatic illness - (Somatoform Disorder) - and may be expressing emotional conflicts through physical complaints. Somatoform Disorders is the relatively new term used to describe a group of involuntary disorders characterized by recurrent and clinically significant physical symptoms that cannot be fully explained by a neurological or generalized medical ("organic") condition. The actual diagnosis in children is rare because the criteria for Somatoform Disorders were established for adults and certain symptoms are not yet experienced by children. Somatoform Disorder must be distinguished from two other diagnoses: Malingering and Factitious Disorders, both characterized by consciously or purposely produced symptoms.
