Somatization Disorder and Medicine Side-Effects
Posted by Jubei on May 5, 2004, at 19:28:39 I would like to say that I have a history of somatization disorder and that I believe that I have been treated unfairly by many doctors. Because of the nature of somatization disorder and its ability to manifest physically that which is held in the mind, the dissonance of what is in the mind and a "real" symptom are made. I have had side-effects with drugs and they are not taken as seriously because doctors often believe I am making them up in my mind. However, this is quite mean especially when the symptom experienced is a common one many people have on the given drug. I think doctors should be more responsible and give the benefit of the doubt to the patient, at least as true to being in human nature as possible and giving action to grant a peace of mind to the patient.

Posted by Racer on May 5, 2004, at 19:39:57 In reply to Somatization Disorder and Medicine Side-Effects , posted by Jubei on May 5, 2004, at 19:28:39 Don't you know? "Blaming the patient is a time honored tradition in medicine." I read that on Dr Bob's site in a discussion amongst doctors of weight gain from anti-depressants. I read something else, somewhere else, which I can't link to because I don't know where it is, but it was in a discussion about how a lack of response to medications leaves patients feeling at fault for their failure to respond to meds even though it is often the doctor's failure to take the patient's complaints seriously. If you can hang on a few more decades, the medical profession may catch up and be reasonable. Until then, you're right: it's a crap shoot whether a doctor dismisses your side effects as 'all in your mind' or real no matter how real they are to you. I'm in that same boat right now, if it helps, so maybe we can row together.

http://www.gesher.org/Neurocare/Medical Education/Psychosomatics.html
neurocareusa.com home neurocare home neuropsychiatric education Introduction In the medical-legal arena, medical specialists, physical therapists, and psychiatrists are often called upon to make an assessment that renders an opinion as to the whether a patient's complaints or efforts are organically, psychiatrically, or pain-based or whether they are consciously feigned, rendering the diagnosis of malingering. Various tests have been designed to assess the so-called "truthfulness" of a patient's pain or the degree of effoft they put forth as a measure of the validity of the test. One of main theoretical difficulties practicioners must overcome in these patients is having a diagnostic grid that is too small to understand properly the scope of the clinical material that one may encounter. It seems to me that the problem lay in the seeming dichotomy between mind and body. Modern medicine has become filled with splitters those who for the sake of clear diagnostic criteria have arbitrarily decided that some conditions are physical (or use the term organic) while others are mental (also called functional). Of course there is truth to this, however, clinicians must face the fact that the body and psyche are irrevocably linked. This has been repeatedly proven scientifically and experimentally.

Background: Somatoform disorders represent a group of disorders characterized by physical symptoms suggesting a medical disorder. However, somatoform disorders represent a psychiatric condition because the physical symptoms present in the disorder cannot be fully explained by a medical disorder, substance use, or another mental disorder. These somatoform disorder physical complaints challenge medical providers who must distinguish between a physical and psychiatric source for the patient’s complaints. Often, the medical symptoms patients experience may be from both medical and a psychiatric illnesses. Anxiety disorders and mood disorders commonly produce physical symptoms. These physical symptoms can dramatically improve with successful treatment of the anxiety or mood disorder. The Diagnostic and Statistical Manual for Mental Disorders, Fourth Edition (DSM-IV)

Somatization and Somatoform Disorders
Somatization is where mental factors such as stress cause physical symptoms. Somatoform disorders are a severe form of somatization where physical symptoms can cause great distress, often long-term. However, people with somatoform disorders are usually convinced that their symptoms have a physical cause.
Somatization What is somatization?
When physical symptoms are caused by mental or emotional stress it is called 'somatization'. For example, many people have occasional headaches caused by mental stress. But, stress and other mental health problems can cause many other physical symptoms such as: chest pains, tiredness, dizziness, back pain, feeling sick, diarrhoea, period problems, etc. The term 'psychosomatic' means something similar to somatization, but includes other things. See separate leaflet called 'Psychosomatic Disorders'. How can the mind cause physical symptoms
The relationship between the mind and body is complex and not fully understood. When we 'somatize', somehow the mental or emotional problem is expressed partly, or mainly, as one or more physical symptoms. However, the symptoms are real and are not imagined. You feel the pain, have the diarrhoea, etc. How common is somatization?

These people are not "lying" about or making up their illness; they believe that they are truly sick. Causes and Risks There is no specific cause, but stress or conflict may underlie the illness. The pain may have a special significance or may serve as a way to receive attention. People with this disorder may receive attention, sympathy or relief from responsibilities from others, or financial compensation from disability that may sustain the pain and behavior.
