The phenomenon of somatization, which results in unexplained physical complaints, is ubiquitous in primary care settings although it often goes unrecognized. Medical training emphasizes the identification and treatment of organic problems and may leave physicians unprepared to recognize and address somatoform complaints. As a process, somatization ranges from mild stress-related symptoms to severe debilitation. Patients at the low end of the spectrum often respond to simple reassurance, but patients who are more impaired require interventions specifically designed to avoid unnecessary exposure to dangerous, costly and frustrating diagnostic procedures and treatments. (Am Fam Physician 2000;61:1073-8.) I n patients with somatoform disorders, emotional distress or difficult life situations are experienced as physical symptoms. Patients who somatize present with persistent physical complaints for which a physiologic explanation cannot be found. Failure to recognize this condition and manage it appropriately may lead to frustrating, costly and potentially dangerous interventions that generally fail to identify occult disease and do not reduce suffering. In patients with somatoform disorders, emotional distress is experienced as a physical symptom.

Previous Next POEM Care Recommendation Letter for Somatization Disorder Clinical Question: Does a care recommendation letter improve outcomes for patients with somatization disorder? Setting: Outpatient (primary care) Study Design: Randomized controlled trial (nonblinded) Synopsis: Somatizing patients have many unexplained symptoms; evaluation of these symptoms consumes significant health care resources and is frustrating for the patient and the physician. In this study, 2,902 consecutive primary care patients were screened for the presence of at least three unexplained physical symptoms with an 11-item questionnaire derived from the Diagnostic and Statistical Manual of Mental Disorders , 3d ed., rev. (DSM-III-R) criteria for somatization disorder. A more detailed face-to-face interview was used to classify the patients (based on DSM-III-R criteria) into somatoform disorder (13 or more lifetime unexplained physical symptoms; n = 88), abridged somatization disorder (at least four unexplained lifetime physical symptoms in men or six in women; n = 183), or multisomatoform disorder (three or more current unexplained symptoms from a list of 15 in a patient with a two-year history of somatization; n = 111). The diagnostic classifications of somatoform disorder, abridged somatization disorder, and multisomatoform disorder were not mutually exclusive; 188 patients (6.4 percent) of the 2,902 patients were classified in at least one of these groups. Thus, on a typical day of outpatient care seeing 24 patients, a physician might see one or two of these patients, so this classification has face validity.

Somatization Disorder
DSM IV Criteria Many physical complaints resulting in treatment being sought or significant functional impairment. Onset is before the age of 30. Physical Complaints History of pain related to at least 4 sites or functions. Two GI symptoms. One sexual symptom. One symptom suggestive of a neurological condition (pseudoneurological). Symptoms can not be explained by organic etiology or symptoms are in excess of what is expected from the medical evaluation. Symptoms are not intentionally produced. Clinical Features of Somatization Disorder Somatization disorder is chronic problem, and it often causes frequent seeking of medical treatment or multiple concurrent treatments. Patients undergo multiple procedures, surgeries, and hospitalizations. The disorder often begins during adolescence. Frequently encountered symptoms include nausea, vomiting, extremity pain, shortness of breath, and pregnancy or menstruation associated complaints. The frequency and severity of symptoms may vary with level of stress.

Definition: Somatization disorder is a chronic condition in which there are numerous physical complaints lasting for years and resulting in substantial impairment that are caused by psychological problems.

Causes, incidence, and risk factors: The disorder is marked by multiple physical complaints that persist for years involving any body system. Most frequently, the complaints involve chronic pain and problems with the digestive system, the nervous system, and the reproductive system. The disorder usually begins before the age of 30 and occurs more often in females. Somatization disorder is highly stigmatizated and patients are often dismissed by their physicians as having problems that are "all in your head." However, as researchers study the connections between the brain, the digestive system, and the immune system, these disorders are becoming better understood and should not be seen as "faked" conditions which the patient could end if he or she chose to do so. The symptoms are severe enough to lead the person to visit the doctor or take medication, and the symptoms interfere with work and relationships. A lifelong history of sickliness is often present, though no specific disease is ever identified to account for the symptoms. A greater intensity of symptoms often occurs with stress.

Mental Health Disorders Somatoform Disorders Somatization Disorder A very specific set of symptoms defines this somatoform disorder. The individual must have a history of physical complaints beginning before he or she is 30 years old that has caused significant distress or impairment. Among the complaints there must have been at least four pain symptoms, two symptoms involving the digestive tract, one involving sexual functioning, and one symptom involving the nervous system. The symptoms could have occurred at any time prior to the examination. The pain can have occurred at any place, during any activity; similarly, there are no further restrictions on the place or time of the other three classes of symptoms. The second requirement is that the symptoms cannot be adequately explained by any observable medical condition or by the effects of a drug. Finally, it must appear that the symptoms are not produced deliberately by the examinee. Somatization Disorder (SD) is relatively rare, and probably more common in women than in men. No more than 1 man in 500, and perhaps 1 woman in 200, has SD. People with the disorder may consult multiple physicians and receive many tests, and even operations, that turn out to be unnecessary. The disorder is likely to be lifelong; patients who seek or are referred to mental health professionals are difficult to convince that their symptoms have little physical basis, given that the patient did not voluntarily produce the symptoms. Denying the validity of the symptoms simply makes the patient seek another professional who acknowledges the genuineness of the symptoms. Patients' colorful, though often inconsistent, descriptions of their symptoms may initially be convincing to the physicians that they so often consult. Maxmen and Ward

Yet how to deal with hypochondria, a disorder that afflicts one of every 20 Americans who visit doctors, has been one of the most stubborn puzzles in medicine. Where the patient sees physical illness, the doctor sees a psychological problem, and frustration rules on both sides of the examining room. Recently, however, there has been a break in the impasse. New treatment strategies are offering the first hope since the ancient Greeks recognized hypochondria 24 centuries ago. Cognitive therapy, researchers reported last week, helps hypochondriacal patients evaluate and change their distorted thoughts about illness. After six 90-minute therapy sessions, the study found, 55 percent of the 102 participants were better able to do errands, drive and engage in social activities. Antidepressant medications, other studies indicate, are also proving effective. The biggest challenge is sorting out their hypochondriacal complaints from real disease, because even a hypochondriac can get sick. I once had a patient who came to me after having 17 negative HIV tests. He had not had any sexual relations in over 6 months, yet thought he needed another test.
