Alternate Names : Briquet's Syndrome Definition Somatization disorder is a chronic condition in which there are numerous physical complaints lasting for years and resulting in substantial impairment that are caused by psychological problems. The disorder is marked by multiple physical complaints that persist for years involving any body system. Most frequently, the complaints involve chronic pain and problems with the digestive system, the nervous system, and the reproductive system. The disorder usually begins before the age of 30 and occurs more often in females. Somatization disorder is highly stigmatizated and patients are often dismissed by their physicians as having problems that are "all in your head." However, as researchers study the connections between the brain, the digestive system, and the immune system, these disorders are becoming better understood and should not be seen as "faked" conditions which the patient could end if he or she chose to do so.

Diagnostic criteria for Somatization Disorder
A history of many physical complaints beginning before age 30 years that occur over a period of several years and result in treatment being sought or significant impairment in social, occupational, or other important areas of functioning.
Each of the following criteria must have been met, with individual symptoms occurring at any time during the course of the disturbance:
Four pain symptoms : a history of pain related to at least four different sites or functions (e.g., Head, abdomen, back, joints, extremities, chest, rectum, during menstruation, during sexual intercourse, or during urination)
Two gastrointestinal symptoms : a history of at least two gastrointestinal symptoms other than pain (e.g., Nausea, bloating, vomiting other than during pregnancy, diarrhea, or intolerance of several different foods)
One sexual symptom : a history of at least one sexual or reproductive symptom other than pain (e.g., Sexual indifference, erectile or ejaculatory dysfunction, irregular menses, excessive menstrual bleeding, vomiting throughout pregnancy)
One pseudoneurological symptom : a history of at least one symptom or deficit suggesting a neurological condition not limited to pain (conversion symptoms such as impaired coordination or balance, paralysis or localized weakness, difficulty swallowing or lump in throat, alphonia, urinary retention, hallucinations, loss of touch or pain sensation, double vision, blindness, deafness, seizures; dissociative symptoms such as amnesia; or loss of consciousness other than fainting)

What is somatization disorder?
Somatization disorder is a psychological disorder (listing 300.81 in the DSM-IV, the Diagnostic and Statistical Manual of Mental Disorders). It is a variety of somatoform disorder, a condition in which a patient reports physical symptoms which cannot be explained by a known general medical disorder . These symptoms must be significant enough to cause distress and impair the person's functioning.
The diagnostic criteria

I would like to know how a neurologist would differentiate temporal lobe epilepsy from somatization disorder. I have a lesion in the right temporal lobe and mildly abnormal EEGs evidencing "sharp"waves with some bilateral dysfunction. The only type of seizure I have had is periods of amnesia when on certain medications, anti-depressants and anti-psychotics. I have read about "Pure Amnesic Syndrome" and it sounds like what happens to me. Recently I started taking Dexatrim to lose weight and had another blackout. I had been told by neurologists in the past that I had a lower seizure threshold. Would OTCs such as Dexatrim cause a ?seizure? Or would this be characteristic of "disassociative amnesia" the DSM -IV lists under Somatization Disorder. If a person has somatization disorder, where neurologic complaints, including amnesia are listed in the diagnostic criteria, would abnormal EEGs be expected or is this an actual physical problem?

Q. Can you tell me more about treatment techniques and theories behind somatization disorder? A. Somatization disordersometimes called Briquet's syndromeis one of the somatoform disorders, which also include conversion disorder, pain disorder, hypochondriasis and body dysmorphic disorder. The SDs have in common this feature: the patient presents with some physical complaint, sign or symptom that cannot be explained by any known medical/neurological illness. All the SDs are thought to be related to some psychological precipitant or factor, such as recent trauma or learned behavior, and are not a form of malingering (i.e., the patient is not faking or intentionally producing the symptom). Treatment of these disorders is difficult and sometimes frustrating. The details of treatment would take an entire chapterbut essentially involve forms of behavior modification and sometimes the use of antidepressant medication (many of these SD patients are also depressed and some consider SDs to be an atypical presentation of depression). The basic goal is to reinforce appropriate, nondependent behaviors and to discourage inappropriate, dependent behaviors. Sometimes, uncovering or psychoanalytic therapy may be helpful.
