One Step Laser Surgery for Snoring
By: Otolaryngology Associates Dr. R. I. Dickson and Dr. D. R. Mintz Snoring, a problem which afflicts millions of people and their partners, occurs when there is obstruction of the free flow of air through the passages of the back of the mouth and nose. This is most often caused by excessive tissue in the uvula and soft palate. Since snoring can be a sign of a more serious condition such as sleep apnea, patients should be fully evaluated by a qualified physician prior to any treatment. Until now the only surgical alternative to Laser Assisted Uvulopalatoplasty (LAUP) was the more extensive and painful procedure (Uvulopalatopharyngoplasty - UPPP) performed under general anaesthetic in the operating room. The hospital stay could be one to three days, with a recuperative period of at least one or two weeks. Some patients have been reluctant to undergo UPPP solely to treat snoring because it involves significant pain and it is their partner and not them who suffers. Standard LAUP treatment done in most centers requires three to five treatments to slowly shrink the floppy palate. No tissue specimen is actually removed; the palate is slowly scarred and tightened. Our one step method involves removal of the same amount of tissue as with standard UPPP. The amount of the palate left for normal function is determined by a palatal dimple easily seen in every patient. This dimple corresponds to the lower level of the palatal muscle elevators which are needed for complete closure on swallowing. Preserving these muscles prevents nasal speech or liquid back flow. Our procedure is done in one session in over 95% of cases. We can remove more tissue with no increased pain because we also use a cold laser (not available in the US) and inject cortisone with local anaesthetic.

When you inhale during sleep, your soft palate, uvula, or both, may vibrate against the back of the throat or the base of your tongue, producing the vibration noise we refer to as snoring. The vibration is caused by the fluttering of loose tissue as you breathe, making inhalation more difficult. If you snore, it is a clear sign that your breathing passage is constantly blocked, resulting in abnormal breathing. Depending on the amount of air passing through the breathing passage and the speed at which the throat is vibrating, snoring may be extremely loud, soft as a whisper, or somewhere in between. Text: We all know what snoring sounds like. But some people - especially overweight men over the age of 40 - have a different kind of snore. Now and again the steady rhythm of their snoring is interrupted by a few seconds of silence, followed by a few extra loud snores.
The reason for this is that the person's tongue, as well as part of their throat, has become so relaxed during sleep that they block the airway, making it hard to breathe. The loud snores following the silence mean the person is waking up and struggling for breath.
Called sleep apnoea, the condition affects about twice as many men as women - doctors think that about nine percent of men over 40 have sleep apnoea. For many of them it's severe enough to be a health problem. Although the person with sleep apnoea doesn't realise, he or she is waking up frequently during the night and not getting enough sleep. This can cause irritability and unusual drowsiness during the day - a serious problem for anyone who drives a car. Research in the US, for instance, has found that drivers with this condition cause more than twice as many road accidents as other drivers.

Introduction
Doctors who are interested in sleep related breathing disorders and run sleep clinics can see a very wide variety of abnormalities ranging from life threatening problems to simple bad habits! In this article we will consider normal sleep patterns together with snoring, sleep related breathing disorders and the condition called obstructive sleep apnoea. We shall see how our normal sleep patterns may be disturbed by jet lag and shift work. Finally, advice will be given about how best to obtain help.
The natural rhythm of sleep.
Nature tends to follow certain rhythms. The spinning of the earth around its own axis dictates a daily rhythm, the moon has a monthly influence and the rotation of the earth around the sun determines the seasons. In common with all other animals we have our own in-built natural rhythms. For example, we have a spontaneous circadian daily rhythm and the reproductive cycle has a monthly rhythm. Curiously, our daily rhythm is approximately 25 hours and is reset to 24 hours by the time clues given by our environment. During this 24 hour cycle there are two periods when the ability to fall asleep is greatly enhanced. This is maximal at about 2 p.m. and between 11 p.m. and 5 a.m. These periods have been described as sleep gates and are shown clearly in the figure below.

CONSIDERING SURGERY FOR SNORING? Before determining whether surgery is appropriate, consider the cause of the snoring. Pure snoring is the vibration of tissues in the airway, whereas with obstructive sleep apnea, blockages occur repeatedly somewhere in the airway. Of course, not everyone who snores has sleep apneaand not everyone who has untreated sleep apnea snoresbut snoring is a common symptom of sleep apnea. Before undergoing surgery for snoring, it is wise to consider if sleep apnea is present. Untreated sleep apnea can be harmful to your health, and eliminating the snoring does not necessarily eliminate the apneas. Some people snore only in certain situations, for example, when they have nasal congestion and cannot breathe through their mouth (people who breathe through their mouth are more prone to snore). Likewise, people who have deviated nasal septums or blocked nasal passages from other causes are more likely to snore. Sleeping flat on the back or drinking alcoholic beverages close to bedtime also induces snoring. (Such situations may induce apneas as well.) A thorough evaluation and sleep study can determine whether the snoring is associated with apneas and, if so, the severity of the sleep apnea. Generally deemed cosmetic, surgery for snoring is rarely covered by insurance policies and may be a significant expense. Most surgical procedures for snoring can be conducted in a physician's office under local anesthetic. (People with sleep apnea must be cautious about general anesthesiano matter for what medical condition the surgery is donebecause of the effects anesthesia has on the airway.
