General Discussion
Symptoms
Infants with Smith-Magenis Syndrome typically have unusual facial features that may include a wider than normal nose, a flat area across the middle of the face, and a prominent forehead and/or jaw. The head appears short and flat. Affected children may have a raspy or hoarse voice, speech delays and a loss of hearing. They may also have short, wide fingers and toes.
Growth delay and mental retardation as well as hyperactivity may occur in children with Smith-Magenis Syndrome. Self-destructive behaviors may include head-banging, wrist-biting, the insertion of foreign objects into the nose and ears (polyembolokoilamania), and pulling out the nails of the fingers and/or toes (onychotillomania). Extreme nearsightedness (myopia) and crossed eyes (strabismus) frequently occur in affected children. Other visual difficulties may include detachment of the retina of the eyes.

Smith-Magenis Syndrome is a rare chromosomal disorder characterized by abnormalities of the head and facial (craniofacial) area, delays in the acquisition of skills requiring the coordination of mental and muscular activities (psychomotor retardation), mental retardation, speech delays, and/or behavioral abnormalities. Characteristic craniofacial abnormalities may include an abnormally short, broad head (brachycephaly); an abnormally broad, flat midface; a broad nasal bridge; an unusually prominent jaw (prognathism); incomplete closure of the roof of the mouth (cleft palate); and/or malformed, abnormally positioned ears. In addition, most affected individuals experience speech delays that may occur in association with hearing impairment and have an abnormally hoarse, deep voice.

Smith-Magenis syndrome is associated with a distinct phenotype of brachycephaly with midface hypoplasia, short, broad hands, mental retardation, sleep disturbances and self-abusive behavior. Reasons for Referral:
Patients with clinical features suggestive of Smith-Magenis Syndrome may be tested for deletions of 17p11.2 by FISH simultaneously with high-resolution chromosomal analysis (if not previously performed). Prenatal diagnosis may be performed if an affected family member has been studied in our laboratory and shown to have a deletion detectable by FISH. Please call regarding all prenatal samples. Testing Methodology:
FISH is the application of fluorescently labeled DNA molecules to metaphase chromosomes and interphase nuclei for the detection of chromosome abnormalities and alterations. It is a rapid, reliable and direct approach for identifying patients with microdeletions or microduplications. FISH analysis for Smith-Magenis syndrome is performed on metaphase chromosomes. Bridging Past and Future Horizons: Understanding the SMS Puzzle September 2000 An educational conference on Smith-Magenis Syndrome (SMS) was held on Sept 21-24, 2000 at the Key Bridge Marriott, Arlington, VA. The conference was co-sponsored by the National Human Genome Research Institute (NHGRI) and the National Institutes of Health (NIH) Office of Rare Diseases (ORD), in conjunction with the parents' support group Parents and Researchers Interested in Smith-Magenis Syndrome (PRISMS ). The conference series brought together parents, researchers and professionals caring for persons with SMS to exchange information, ideas, scientific data and family experiences. Attendees hear from medical experts, geneticists,and parents about the latest information on the medical, social, developmental and behavioral aspects of SMS, as well as current research efforts into the cause, management and effective treatment of SMS. The format of invited speakers and a series of smaller, more hands-on workshops is designed to maximize the exchange of information between parents and professionals from a variety of specialties. and Charles I. Scott, Jr. We describe a 14-year-old boy with both physical and behavioral manifestations of the Smith-Magenis syndrome. Low level mosaicism (11%) for deletion 17p11.2 was found in peripheral blood lymphocytes. The deletion was also observed in 100% of metaphases examined from skin fibroblast cultures. We confirm that the Smith-Magenis syndrome is associated with a highly recognizable phenotype. Because evidence of the abnormal cell line may be minimal or absent in peripheral blood, fibroblast studies are indicated for patients in whom mosaicism for deletion 17p11.2 is suspected clinically.
