Everyone has brief pauses in their breathing pattern called apnea - even your child. Usually these brief stops in breathing are completely normal. Sometimes, though, apnea or other sleep-related problems can be a cause for concern. Read on to find out what it means when your child's doctor mentions apnea. Types of Apnea
The word apnea comes from the Greek word meaning "without wind." Although it's perfectly normal for everyone to experience occasional pauses in their breathing, apnea can be a problem when breathing stops for 20 seconds or longer. There are three types of apnea: obstructive, central, and mixed. Obstructive Apnea
This type of apnea is caused by an obstruction of the airway (such as enlarged tonsils and adenoids ), and it is a common type of apnea in children. As many as 1% to 3% of otherwise healthy preschool-age children have obstructive apnea. The most common symptom is snoring; others include labored breathing while sleeping, gasping for air, sleeping in unusual positions, and changes in color. Because obstructive sleep apnea may disturb sleep patterns, these children may also show continued sleepiness after awakening in the morning, and tiredness and attention problems throughout the day. Sometimes this can affect school performance. One recent study suggests that some children diagnosed with AD/HD actually have attention problems in school because of disrupted sleep patterns caused by obstructive sleep apnea.

Breathing disorders during sleep can lead to disturbances as commonplace as snoring. In more extreme cases, they may cause daytime hypersomnolence - the inability to stay awake during the day. The term apnea is used to describe a pause in breathing of 10 or more seconds. Sleep apnea is defined as having 30 or more apneic episodes per 8 hours of sleep. Sleep Apnea Syndrome (SAS) is characterized by numerous apneic episodes along with loud snoring and daytime sleeping. Daytime sleeping is a real problem on the job or while driving. Falling asleep at the wheel is a common problem for SAS patients and the cause of numerous accidents. Falling asleep at work frequently results in injury and/or job loss. Believe it or not, SAS can also lead to memory problems as well as difficulty concentrating and thinking. Anxiety and/or depression are frequent symptoms, as is irritable and aggressive behavior. Physically, a person with SAS may experience early morning headaches, sexual dysfunction, chronic tiredness, high blood pressure, and swelling of the extremities.
Treatment
There are numerous treatment possibilities available for people experiencing sleep apnea. Obese patients must lose weight. There is very little risk associated with medically supervised weight-loss programs, and the weight loss may offer additional physical benefits.

Most of us spend 5-8 hours sleeping every night and take it for granted that we will fall asleep easily and wake up refreshed in the morning. However, most of us will have some problem with sleep at least once in our lives and 1 in 5 have a chronic problem with sleep. We all breath at a rate of 10-20 times a minute without conscious effort. However, 1 in 5 individuals will have some problem with breathing due to smoking, occupational exposures, or conditions such as asthma (which affects 1 in 20). Sleep Apnea
Sleep apnea is described as the cessation of breathing during sleep. What is actually occurring is the intermittent and repetitive closure of the airway druing sleep. This begins a vicious circle of events. The patient falls asleep, the airway closes. Then in response to closure and the need to breathe, the brain wakes up briefly, the patient breathes; the brain retruns to sleep and the circle begins again. These events may occur hundreds of times across the night resulting in severely fragmented sleep, poor oxygenation and daytime sleepiness. The corrective treatment most often used is nasal CPAP. This is a tight fitting mask, connected via a flexible tubing to an air pump. This allows a positive pressure of air to splint the airway open. The patient breathes well, sleeps well and shortly is no longer sleepy. Other treatment options include: ENT surgery, weight loss, position changes, and dental appliances. If you choose surgery or dental appliance, it is important to note that the symptom of snoring may be corrected but apnea may remain. Always see your sleep specialist for followup which may include another sleep study.

What Is Sleep Apnea? Sleep apnea is a serious, potentially life-threatening condition that is far more common than generally understood. First described in 1965, sleep apnea is a breathing disorder characterized by brief interruptions of breathing during sleep. It owes its name to a Greek word, apnea, meaning "want of breath." There are two types of sleep apnea: central and obstructive. Central sleep apnea, which is less common, occurs when the brain fails to send the appropriate signals to the breathing muscles to initiate respirations. Obstructive sleep apnea is far more common and occurs when air cannot flow into or out of the person's nose or mouth although efforts to breathe continue. In a given night, the number of involuntary breathing pauses or "apneic events" may be as high as 20 to 30 or more per hour. These breathing pauses are almost always accompanied by snoring between apnea episodes, although not everyone who snores has this condition. Sleep apnea can also be characterized by choking sensations. The frequent interruptions of deep, restorative sleep often lead to early morning headaches and excessive daytime sleepiness.

What causes sleep apnea?
  • Some children have facial structures that narrow the airway and cause sleep apnea. Sleep apnea can also happen when the throat and tongue relax during sleep and partly block the airway. Sleep apnea can occur in overweight children if they have too much tissue in their airway, making it narrow.
Who can get sleep apnea?
  • Both adults and children of all ages can have sleep apnea. It is most common between the ages of 3 and 6 years old. Some children are at higher risk. Overweight children, children with Down's syndrome, and children who have large tonsils are at higher risk.

DANIEL WAGNER, MD: "Apnea" means "without breath." And sleep apnea is a type of "without breath" that occurs only during sleep. The majority of cases are obstructive, where the throat back behind your tongue literally gets sucked closed by the chest as it's trying to pull air through it. That's because the muscles of the throat have relaxed enough when you're asleep to allow that to happen. It would never happen while you were awake you would choke and know it immediately. That goes on for 10, 20, 30, sometimes 60 seconds. The efforts of the chest finally wake the person up briefly, usually without their knowing it. They breathe a few times and get their oxygen levels back up, and then as they fall back into light sleep, the whole business happens again.

Why do I snore? Medical experts estimate that more than 30 million American adults snore. Snoring or gasping sounds during sleep is caused by the vibration of relaxed, floppy tissues that line the upper airway (or throat). When you sleep, muscle tone decreases throughout your body and your throat muscles relax, causing the soft palate and the uvula (fleshy structure that dangles from the roof of your mouth) to vibrate as you breathe in and out. Is snoring harmful to my health?
The severity of this disorder varies: It can be a mild nuisance that disturbs a restful night's sleep or a symptom of the more serious, progressive sleep apnea syndrome. A history of snoring may precede development of more serious sleep disorder symptoms, including excessive daytime sleepiness, memory impairment, morning headache, poor work performance and loss of sexual drive. While snoring by itself is not life threatening, it can interfere with a good night's sleep for you, the snorer, and also affect your partner's ability to get quality sleep. Scientific research has found that partners of snorers lose up to an hour of sleep a night because of the nuisance. How can I minimize my snoring?

General Description of Sleep Apnea and Snoring
Sleep apnea is a disorder in which a person stops breathing during the night, perhaps hundreds of times, usually for periods of 10 seconds or longer. In most cases the person is unaware of it, although sometimes they awaken and gasp for breath. It is usually accompanied by snoring. People who have sleep apnea may not even be aware of the condition, but it inevitably causes daytime sleepiness.
Sleep apnea is generally categorized as obstructive, central, or mixed. A less severe form of obstructed breathing called upper airway resistance syndrome (UARS) is also worth mentioning.
Obstructive Sleep Apnea
Obstructive sleep apnea (OSA), the most common form of apnea, occurs when tissues in the upper throat (or airway) collapse at intervals during sleep, thereby blocking the passage of air. In general, OSA occurs as follows:
  • On its way to the lungs, air passes through the nose, mouth, and throat (known as the upper airway).
  • Under normal conditions, the back of the throat is soft and pliant and tends to collapse inward as a person breathes.
