Sleep Apnea in Renal Failure Sleep apnea is common in renal failure patients, and may be associated with coronary artery disease and stroke, arrhythmias and both systemic and pulmonary hypertension. It could contribute to the high mortality from cardiovascular disease in the dialysis population, but these answers, as well as the specific causes of sleep apnea in ESRD are unknown. The following links not only validate the frequency of sleep apnea in ESRD patients, but also offer a screening questionaire, diagnostic criteria and management guidelines. Twenty-nine consecutive patients with ESRD reporting disrupted sleep or daytime sleepiness underwent all-night polysomnography. The PLMS index and the total number of arrousals per hour predicted mortality. An index of greater than 20 was associated with a 20 month survival rate of 50%, compared with a 90% survival rate among those with a PLMSI less than 20.

Sleep apnea is a serious sleep problem. If you have it, you stop breathing for more than 10 seconds at a time many times while you sleep. Another term for this problem is obstructive sleep apnea. Sleep apnea affects between 2 and 10% of people. It is more common in men than in women. It is also more common in people who are overweight, but there are many people with normal weight who have sleep apnea.

What is obstructive sleep apnea?
Obstructive sleep apnea syndrome (OSAS) is a debilitating sleep and breathing disorder defined as the cessation of breathing for 10 seconds or more (an apnea) at least five times per hour of sleep. During sleep, the body's muscles relax, which can cause excess tissue to collapse into the upper airway (back of the mouth, nose and throat) and block breathing. When breathing is interrupted by an obstruction in the airway, the body reacts by waking enough to start breathing again. These arousals may occur hundreds of times each night but do not fully awaken the patient, who remains unaware of the loud snoring, choking and gasping for air that are typically associated with obstructive sleep apnea. WHAT IS SLEEP APNEA?
Sleep apnea is a serious, potentially life-threatening condition that is far more common than generally understood. First described in 1965, sleep apnea is a breathing disorder characterized by brief interruptions of breathing during sleep. It owes its name to a Greek word, apnea, meaning "want of breath." There are two types of sleep apnea: central and obstructive. Central sleep apnea, which is less common, occurs when the brain fails to send the appropriate signals to the breathing muscles to initiate respirations. Obstructive sleep apnea is far more common and occurs when air cannot flow into or out of the person's nose or mouth although efforts to breathe continue. In a given night, the number of involuntary breathing pauses or "apneic events" may be as high as 20 to 30 or more per hour. These breathing pauses are almost always accompanied by snoring between apnea episodes, although not everyone who snores has this condition. Sleep apnea can also be characterized by choking sensations. The frequent interruptions of deep, restorative sleep often lead to early morning headaches and excessive daytime sleepiness.

But the condition can afflict both men or women at any age. People with this disorder actually may stop breathing while asleep-even hundreds of timeswithout being aware of the problem. During an apnea attack, the snorer may seem to gasp for breath, and the oxygen level in the blood may become abnormally low. In severe cases, a sleep apnea victim may actually spend more time not breathing than breathing and may be at risk for death. In the most common form of the condition, obstructive apnea (also called upper airway apnea), air stops flowing through the nose and mouth, but throat and abdominal breathing efforts are uninterrupted. The snoring that results is produced when the upper rear of the mouth (the soft palate and the cone-shaped tissuethe uvulathat descends from it) relaxes and vibrates as air passes in and out. This sets up an air current between the palate and the base of the tongue, resulting in snoring. Typically, the individual will wake up, emit a vigorous snort or grunt while gasping for air, then immediately fall back to sleep, only to repeat the cycle.
