See also sleep disorders Causes, incidence, and risk factors Return to top In normal conditions, the muscles of the upper part of the throat keep this passage open to allow air to flow into the lungs. These muscles usually relax during sleep, but the passage remains open enough to permit the flow of air. Some individuals have a narrower passage, and during sleep, relaxation of these muscles causes the passage to close, and air cannot get into the lungs. Loud snoring and labored breathing occur. When complete blockage of the airway occurs, air cannot reach the lungs.

Snoring is an age-old problem with social consequences, including marital discord , separate bedrooms, and even separation and divorce . Snoring occurs due to vibration of the tissue in the throat, usually while the patient is breathing through the mouth. The site(s) - or point(s) - of obstruction must be diagnosed before treatment can begin.

Apnea (literally, "without breath") is the term used when someone stops breathing for very short periods of time, usually 10 to 20 seconds. It's termed "obstructive" when respiratory efforts continue, such as movements of the chest. It's termed "central" when all respiratory effort stops. There is also a mixed version. In children, sleep apnea is almost always obstructive. During the apneic episode, the child will have decreased oxygenation of the blood. Children with Down syndrome (DS) are certainly at risk for OSA. In 1991, one study showed 45% had OSA. This can be caused by several different factors present in DS: the flattened midface, narrowed nasopharyngeal area, low tone of the muscles of the upper airway and enlarged adenoids and/or tonsils.
Why is this important? Well, first, there's the obvious problem of the child not getting enough quality sleep and the behavioral effects that brings. Second, I've mentioned above that during sleep apnea, the oxygenation of the blood decreases. It has been shown that in children with DS and heart disease this low oxygenation causes an increase in the blood pressure in the lungs as the body tries to get more oxygen. This "pulmonary hypertension" can cause the right side of the heart to become enlarged and other cardiac complications can follow. The incidence of death due to OSA is unknown.

Chronic Obstructive Airways Disease
There can be large falls in arterial oxygen saturation during sleep in patients with chronic obstructive pulmonary disease. In general, these falls can be predicted from daytime oxygen levels and are not due to classical OSA syndrome, but to a number of factors which combine to produce worsening hypoxaemia during sleep. These patients are often at the limits of their respiratory capacity, and the loss of the intercostal and accessory muscle contribution to breathing, and the increase in upper airways resistance associated with REM sleep, cause marked falls in ventilation. In addition, patients may be hypoxic while awake and small falls in ventilation and PaO2 lead to large falls in arterial oxygen saturation during sleep. The hypoxic episodes demonstrated by overnight oximetry in a patient with chronic obstructive pulmonary disease differ in character from those seen in patients with OSA syndrome (figure 10). There is some evidence that increases in upper airway narrowing during sleep, short of apnoeas and even snoring, may contribute to the development of CO2 retention in these patients. However classical sleep apnoea is not significantly more common in unselected COPD patients with raised CO2 levels, and excessive nocturnal hypoxaemia is not one of the predictors of long- term survival in patients with COPD.
