S leep apnea is a cessation of breathing during sleep that is caused by repetitive partial or complete obstruction of the airway by pharyngeal structures. Patients with obstructive sleep apnea often are overweight, snore loudly, and complain of daytime fatigue and sleepiness. A sleep laboratory may be used to confirm the diagnosis. Family physicians should be able to diagnose and manage this relatively common disorder. Deciding When to Treat The risk of long-term mortality increases when patients have 20 or more respiratory events per hour during sleep testing. The initial step in the management of sleep apnea is deciding which patients need treatment. The severity of the condition is measured by the number of apneas (cessations of airflow) or hypopneas (reductions in airflow) that cause sleep arousal. The number of these events per hour is commonly called the "apnea-hypopnea index" or the "respiratory disturbance index." An often quoted study recommended treatment if patients had 20 or more respiratory events per hour, because long-term mortality increases above that level.

What is sleep apnea?
Sleep apnea (sleep-disordered breathing) is a serious and common sleep disorder affecting about 12 million Americans, according to the National Institutes of Health (NIH). Its name comes from a Greek word, apnea , meaning "without breath." People with sleep apnea stop breathing briefly many times during the night. The breathing pauses last at least 10 seconds, and there may be 20 to 30 or more pauses per hour. The main symptoms of sleep apnea are persistent loud snoring at night and daytime sleepiness. Another symptom is frequent long pauses in breathing during sleep, followed by choking and gasping for breath. People with sleep apnea don't get enough restful sleep, and their daytime performance is often seriously affected. Sleep apnea may also lead to high blood pressure, heart disease, heart attack, and stroke. However, it can be diagnosed and treated.

    One Step Laser Surgery for Snoring
    By: Otolaryngology Associates Dr. R. I. Dickson and Dr. D. R. Mintz Snoring, a problem which afflicts millions of people and their partners, occurs when there is obstruction of the free flow of air through the passages of the back of the mouth and nose. This is most often caused by excessive tissue in the uvula and soft palate. Since snoring can be a sign of a more serious condition such as sleep apnea, patients should be fully evaluated by a qualified physician prior to any treatment. Until now the only surgical alternative to Laser Assisted Uvulopalatoplasty (LAUP) was the more extensive and painful procedure (Uvulopalatopharyngoplasty - UPPP) performed under general anaesthetic in the operating room. The hospital stay could be one to three days, with a recuperative period of at least one or two weeks. Some patients have been reluctant to undergo UPPP solely to treat snoring because it involves significant pain and it is their partner and not them who suffers. Standard LAUP treatment done in most centers requires three to five treatments to slowly shrink the floppy palate. No tissue specimen is actually removed; the palate is slowly scarred and tightened. Our one step method involves removal of the same amount of tissue as with standard UPPP. The amount of the palate left for normal function is determined by a palatal dimple easily seen in every patient. This dimple corresponds to the lower level of the palatal muscle elevators which are needed for complete closure on swallowing. Preserving these muscles prevents nasal speech or liquid back flow. Our procedure is done in one session in over 95% of cases. We can remove more tissue with no increased pain because we also use a cold laser (not available in the US) and inject cortisone with local anaesthetic.

General Description of Sleep Apnea and Snoring Sleep apnea is a disorder in which a person stops breathing during the night, perhaps hundreds of times, usually for periods of 10 seconds or longer. In most cases the person is unaware of it, although sometimes they awaken and gasp for breath. It is usually accompanied by snoring. People who have sleep apnea may not even be aware of the condition, but it inevitably causes daytime sleepiness.
    Sleep apnea is generally categorized as obstructive, central, or mixed. A less severe form of obstructed breathing called upper airway resistance syndrome (UARS) is also worth mentioning. Obstructive Sleep Apnea Obstructive sleep apnea (OSA), the most common form of apnea, occurs when tissues in the upper throat (or airway) collapse at intervals during sleep, thereby blocking the passage of air. In general, OSA occurs as follows:
    • On its way to the lungs, air passes through the nose, mouth, and throat (known as the upper airway).
    • Under normal conditions, the back of the throat is soft and pliant and tends to collapse inward as a person breathes.

Obstructive sleep apnea is the most common form of sleep apnea, a disorder marked by repeated stops and starts in a person's breathing during sleep, sometimes hundreds of times during the night. These breathing disruptions make for poor-quality sleep and daytime symptoms such as drowsiness, concentration problems and dulled reaction times potential hazards on the road. The new study suggests that of an estimated 1,400 traffic deaths related to obstructive sleep apnea in 2000, 980 could have been prevented if all the drivers had been treated with continuous positive airway pressure (CPAP). The estimates, based on data from the National Safety Council and published medical studies, also indicate such treatment could save Americans $11 billion a year in collision costs. Dr. Alex Sassani of the University of California San Diego and his colleagues report the findings in the journal Sleep.
