The Story of a Sleep Apnea Victim
My husband, Norman, often fell asleep watching television, and the kids snickered. "Dad's getting old," my daughter would whisper. But Dad wasn't really old. In his early fifties, he was much too young to be sleeping his life away. As the malady became worse, he'd fall asleep a dozen times a day, even in the middle of a sentence. He often fell asleep while driving, creating a danger to himself and to others. If I were there, I'd nudge him with my elbow or shout to wake him up. Several times, however, he was alone and he landed in the ditch.

The impact of sleep apnea can be devastating: not only upon those afflicted, but also, upon those close to them.
This extremely common and potentially devastating disorder is easily detected in most cases. Also, highly effective treatments are available. Tragically, though, in many cases, sleep apnea is ignored until it is too late. All too often, snoring is still regarded as humorous, rather than as an indicator of serious medical problems. Denial on the part of the person with the problem is commonplace. The most common kind of sleep apnea is called Obstructive Sleep Apnea Syndrome. Sleep apnea means "cessation of breath." It is characterized by repetitive episodes of upper airway obstruction that occur during sleep, usually associated with a reduction in blood oxygen saturation. In other words, the airway becomes obstructed at several possible sites. The upper airway can be obstructed by excess tissue in the airway, large tonsils, a large tongue and usually includes the airway muscles relaxing and collapsing when asleep. Another site of obstruction can be the nasal passages. Sometimes the structure of the jaw and airway can be a factor in sleep apnea. What are the symptoms?
  • excessive daytime sleepiness
  • frequent episodes of obstructed breathing during sleep. (The patient may be unaware of this symptom usually the bed partner is extremely aware of this).
Associated features may include:
  • loud snoring
  • morning headaches
  • unrefreshing sleep
  • a dry mouth upon awakening
  • chest retraction during sleep in young children (chest pulls in)

  • change in personality

    CANPAP is a multi-centred randomized trial designed to explore a new non-pharmacologic therapy (Continuous Positive Airway Pressure - CPAP) for patients with Congestive Heart Failure and Central Sleep Apnea.
    The CANPAP trial is currently recruiting in many large metropolitan cities in Canada. The trial began in January 1999 at six clinical sites in Canada. An additional four sites have since joined the trial. The clinical sites participating in the CANPAP trial are as follows:

    Sleep Apnea is a disorder of breathing during sleep. Typically it is accompanied by loud snoring. Apnea during sleep consist of brief periods throughout the night in which breathing stops. People with sleep apnea do not get enough oxygen during sleep. There are 2 major types.
    Obstructive Sleep Apnea is the most common type and is due to an obstruction in the throat during sleep. Bed partners notice pauses approx. 10 to 60 seconds between loud snores. The narrowing of the upper airway can be a result of several factors including inherent physical characteristics, excess weight, and alcohol consumption before sleep. Central Sleep Apnea - caused by a delay in the signal form the brain to breath . With both obstructive and central apnea you must wake up briefly to breathe, sometimes hundreds of times during the night. Usually there is no memory of these brief awakenings.

    Sleep apnea is a disorder that commonly afflicts more than 12 million people in the United States. It takes its name from the Greek word apnea, which means "without breath." People with sleep apnea literally stop breathing repeatedly during their sleep, often for a minute or longer and as many as hundreds of times during a single night. Sleep apnea can be caused by either complete obstruction of the airway (obstructive apnea) or partial obstruction (obstructive hypopnea hypopnea is slow, shallow breathing), both of which can wake one up. There are three types of sleep apnea — obstructive, central, and mixed. Of these, obstructive sleep apnea (OSA) is the most common. OSA occurs in approximately 2 percent of women and 4 percent of men over the age of 35. Causes The exact cause of OSA remains unclear. The site of obstruction in most patients is the soft palate, extending to the region at the base of the tongue. There are no rigid structures, such as cartilage or bone, in this area to hold the airway open. During the day, muscles in the region keep the passage wide open. But as a person with OSA falls asleep, these muscles relax to a point where the airway collapses and becomes obstructed. When the airway closes, breathing stops, and the sleeper awakens to open the airway. The arousal from sleep usually lasts only a few seconds, but brief arousals disrupt continuous sleep and prevent the person from reaching the deep stages of slumber, such as rapid eye movement (REM) sleep, which the body needs in order to rest and replenish its strength. Once normal breathing is restored, the person falls asleep only to repeat the cycle throughout the night.

    O bstructive sleep apnea is a disorder in which complete or partial obstruction of the airway during sleep causes loud snoring, oxyhemoglobin desaturations and frequent arousals. As a result, affected persons have unrestful sleep and excessive daytime sleepiness. The disorder is associated with hypertension, impotence and emotional problems. Because obstructive sleep apnea often occurs in obese persons with comorbid conditions, its individual contribution to health problems is difficult to discern. The disorder has, however, been linked to angina, nocturnal cardiac arrhythmias, myocardial infarction, stroke and motor vehicle crashes. Even though obstructive sleep apnea is fairly common, it often remains undiagnosed in primary care practice. Because the disorder is associated with significant morbidity and even some mortality, family physicians need to be familiar with its clinical presentation and treatment. Pathophysiology Obstructive sleep apnea is caused by repetitive upper airway obstruction during sleep as a result of narrowing of the respiratory passages.
