Shy-Drager Syndrome
Multiple system atrophy (MSA) with postural hypotension, also called Shy-Drager syndrome, is a progressive disorder of the central and sympathetic nervous systems. The disorder is characterized by postural (or orthostatic) hypotension-an excessive drop in blood pressure when the patient stands up, which causes dizziness or momentary blackouts. MSA has been classified clinically into three types, olivopontocerebellar atrophy (OPCA), which primarily affects balance, coordination, and speech; a parkinsonian form (striatonigral degeneration), which can resemble Parkinson's disease because of slow movement and stiff muscles; and a mixed cerebellar and parkinsonian form. In all three forms of MSA, the patient can have orthostatic hypotension. Orthostatic hypotension and symptoms of autonomic failure such as constipation, impotence in men, and urinary incontinence usually predominate early in the course of the disease. Constipation may be unrelenting and hard to manage. Shy-Drager syndrome may be difficult to diagnose in the early stages. For the majority of patients, blood pressure is low when the patients stand up and high when the patients lie down. Other symptoms that may develop include impaired speech, difficulties with breathing and swallowing, and inability to sweat.

Shy-Drager syndrome
Shy-Drager syndrome (named after Dr. Milton Shy and Dr. Glenn Drager, who identified this syndrome in 1960) is a rare, progressively degenerative disease of the autonomic nervous system . Its cause is not known, and there is no cure at this time. Symptoms of this syndrome can include: dizziness or fainting when standing up (known as postural or orthostatic hypotension ); incontinence; muscular tremors; impotence; dry mouth and skin; vision problems; trouble regulating body temperature due to abnormal sweating; breathing difficulties; Parkinson-like tremors. Most people with this syndrome are disabled within 7-8 years of the initial onset of symptoms; few live longer than 15 years afterwards.

Description
Shy-Drager Syndrome (Multiple System Atrophy) (SDS)(MSA) is a a rare degenerative condition resulting from degeneration of certain nerve cells in the brain and spinal cord. Body functions controlled by these areas of the brain and spinal cord do not function normally in sufferers. These functions include the autonomic or involuntary nervous system (which controls blood pressure, heart rate, and bladder function) and the motor system (which controls balance and muscle movement). The syndrome is named after the two doctors who first identified it in 1960. The cause is unknown. It affects one in 10,000 people and is most often diagnosed in men older than 60. It is two or three times more common among men than among women. Depending upon which part of the brain is affected first, MSA may appear in different ways. Sometimes it presents with low blood pressure on standing, urinary bladder problems, or difficulties with balance and movement that resemble Parkinson's disease. Early symptoms often include impotence and urinary incontinence. Most patients complain of severe constipation and later develop rectal incontinence.
Orthostatic hypotension – an excessive drop in blood pressure when the patient stands up causing light-headedness or dizziness - is a universal feature of SDS. It is worse after meals and early in the day. When orthostatic hypotension becomes severe (i.e. blood pressure of less than 70/40) ‘blackouts’ may result. When the patient falls to a prone position, the blood pressure returns toward normal and the patient will regain consciousness.

Definition
Shy-Drager syndrome (SDS) is a rare condition that causes progressive damage to the autonomic nervous system. The autonomic nervous system controls vital involuntary body functions such as heart rate, breathing, and intestinal, urinary, and sexual functions. The autonomic nervous system also controls skin and body temperature, and how the body responds to stress. Shy-Drager syndrome leads to dizziness or fainting when standing up, urinary incontinence, impotence, and muscle tremors.
Description
SDS was named for neurologists Milton Shy, M.D., from the National Institutes of Health, and Glenn Drager, M.D., from the Baylor College of Medicine, who first described the condition in 1960. It affects one of every 10,000 people, typically those between ages 50-70. It affects more men than women. In severe cases, the person cannot even stand up. Symptoms can be mild as well. Sometimes, people with mild cases are misdiagnosed as having anxiety or hypertension. Many nonprescription drugs, such as cold medicines and diet capsules, can trigger extremely high blood pressure spikes in patients with SDS, even in very low doses. Therefore, these patients are at risk for strokes and excessive bleeding (hemorrhage) if they take even the recommended dosage of these drugs.
