Shy-Drager syndrome, also known as multiple system atrophy, is a rare neurodegenerative disorder characterized by progressive failure of the autonomic nervous system, affecting movement, blood pressure regulation and other functions of the body. The autonomic nervous system is the branch of the nervous system that is responsible for a variety of vital involuntary body functions, such as heart rate, blood pressure, sweating, and bowel and bladder control. Symptoms of Shy-Drager syndrome include unusually low blood pressure upon standing (orthostatic hypotension), loss of bladder and bowel control, and lack of sweating (anhidrosis).

Shy-Drager syndrome
Shy-Drager syndrome (named after Dr. Milton Shy and Dr. Glenn Drager, who identified this syndrome in 1960) is a rare, progressively degenerative disease of the autonomic nervous system . Its cause is not known, and there is no cure at this time. Symptoms of this syndrome can include: dizziness or fainting when standing up (known as postural or orthostatic hypotension ); incontinence; muscular tremors; impotence; dry mouth and skin; vision problems; trouble regulating body temperature due to abnormal sweating; breathing difficulties; Parkinson-like tremors. Most people with this syndrome are disabled within 7-8 years of the initial onset of symptoms; few live longer than 15 years afterwards.

http://miso.wwa.com:80/~claudec/shydrager/ History and Description The Shy-Drager syndrome is a baffling condition that affects one in 10,000 people, mainly those between ages 50 and 70. The victims have abnormally low blood pressure which can cause dizziness and fainting - but only when standing up. Blood pressure may actually be high when the patient lies down. While severely affected patients may be unable to stand up at all, mildly affected patients may be misdiagnosed as having anxiety or even hypertension. Many over-the-counter drugs like cold preparations and diet capsules can cause extraordinary elevations of blood pressure even in very low dosages in such individuals. The patients are thus at risk for stroke and hemorrhage if they take the "recommended" amounts of these drugs. In the Shy-Drager syndrome (multiple system atrophy), autonomic failure is widespread and is associated with impairment in other neurologic systems. The autonomic dysfunction in the Shy-Drager syndrome can be viewed predominantly as a central defect with an inability to engage a generally healthy peripheral autonomic system. Patients with the Shy-Drager syndrome typically have extensive autonomic failure with impairment linked to several different neurological systems. In these patients, chronic orthostatic hypotension may be a presenting symptom; in other cases, extrapyramidal symptoms or cerebellar symptoms may predominate early in the course. In patients with cerebellar involvement tremor is worsened by nicotine and Shy-Drager patients generally discontinue smoking at the onset of their disease. Idiopathic orthostatic hypotension, multiple system atrophy, multiple system atrophy with autonomic failure, orthostatic hypotension, orthostatic hypotension-neurologic syndrome, orthostatic hypotensive-dysautonomic-dyskinetic syndrome, orthostatic hypotension variant syndrome, primary neurogenic orthostatic hypotension, progressive autonomic failure of central origin.
Associated persons: Glenn Albert Drager George Milton Shy Description: A progressive disorder of the autonomic nervous system in which chronic orthostatic hypotension is associated with bladder and bowel incontinence, Parkinsonism, cerebellar ataxia, impotence, visual troubles, tremor and disturbances of sweating, and muscle wasting leading to severe invalidism. Central nervous system manifestations may be identical to those of Parkinson's. Autonomic neurones are degenerated and there may be atrophic areas in the basal ganglia, midbrain and brainstem. Onset usually takes place in the sixth or seventh decades, most persons becoming disabled within 5 to 7 years. More males than females are affected. Frequency is 1 in 10.000 persons. The aetiology is unknown.

Definition
Shy-Drager syndrome is a degenerative disorder of the nervous system. Multiple areas of the nervous system can be affected, such as the parts that control motor and involuntary functions. The motor system controls movement and balance. The involuntary, or autonomic, nervous system controls body functions (for example, blood pressure and bowel and bladder function). Shy-Drager syndrome may cause postural hypotension. Postural hypotension is a drop in blood pressure after changing position.
Causes
It is unknown what causes the degeneration or nerve loss in Shy-Drager syndrome.
Risk Factors
A risk factor is something that increases your chances of getting a disease or condition. Risk factors for Shy-Drager syndrome include:
