Short bowel syndrome (SBS) occurs whenever the functions of the small bowel have been removed from the body by disease or surgery. The normal small bowel varies in length between 10 and 28 feet and is divided into three sections. The first section is the duodenum and is located just after the stomach. The duodenum is relatively short, measuring only about 10-12 inches. The next section is the jejunum, which is the largest but not the longest section. The jejunum comprises only 2/5 of the rest of the small intestine but is the largest section because its many folds give it an increased diameter. The final 3/5 of the small bowel is the ileum.
The level of difficulty experienced by a patient with SBS is often determined by which sections are affected. One half of the small bowel can be removed without significant disability resulting, especially if the ileum is not involved. However, the more intestine removed, the greater the disability a patient will experience. The entire jejunum can be removed without much disability except for lactose intolerance. The ileum can compensate for all the losses of the jejunum except the lactose deficiency. Removal of the ileum accounts for most of the problems seen in short bowel syndrome since it is responsible for the absorption of fats, bile salts, and B12.

I have been on TPN now for 20 months. I have asked what criteria is used to decrease TPN and theres been little discussion about this. I currently cycle TPN 12h/d. Weight stable 118lbs. @ 5"3'.

Short Bowel Syndrome (SBS) The short bowel syndrome occurs from massive small intestinal resection leaving less than 200 cm (6.5 feet) of small bowel. In children, however, a length which is less than 30% of the normal length for age is the generally accepted definition of SBS. Etiologies range from a loss due to congenital anomalies, loss from necrotizing enterocolitis , resections due to strangulation of the intestine (volvulus) or from extensive resections for inflammatory bowel disease . Approximately 10 to 20,000 patients are cared for each year with SBS in the United States. Severity of the loss of intestine has generally been associated with a loss of excess bowel length or loss of the ileocecal valve. Over the past decade, it has been better appreciated that the severity of SBS is most correlated with the absolute function of the intestine. Many individuals with 40 cm of small bowel may eventually adapt and transition off TPN. More important is the need for normal motility of the stomach and small intestine, as well as the overall injury that the remaining intestine incurred. Figure above shows the various etiologies, and approximate frequencies, of SBS in infants and children.

What is going on in the body?
The small bowel carries food from the stomach to the large bowel, which attaches to the rectum and anus. It plays an important role in digestion. Vitamins, minerals, salts, and important fats are absorbed through the small bowel.
Removal of part of the small bowel decreases the area to absorb these nutrients. Many people can tolerate removal of up to 50% to 60% of the small bowel without having problems. However, when more than this amount or a key part of small bowel is removed, symptoms often occur. These symptoms make up what is called the short bowel syndrome.
What are the signs and symptoms of the condition?
The main symptoms of this condition are abdominal pain, or distress , and diarrhea . Malabsorption, or poor absoption, of important nutrients may lead to weight loss , low blood counts including anemia , and vitamin deficiencies.

Serono Serostim Additional Studies For Short Bowel Syndrome Indication Needed, Cmte. Says
Serono Serostim requires more study prior to approval for short bowel syndrome, FDA’s Gastrointestinal Drugs Advisory Committee said June 25. "Efficacy has not been shown in a generalizable fashion," Committee Chair Michael Wolfe, MD, Boston University, said. The committee voted six to three that the study submitted by Serono was not sufficient to demonstrate the efficacy of the growth hormone product for short bowel syndrome patients. One of the main concerns of the committee, and FDA, was that in the 41-patient study conducted by Serono, 38 patients were enrolled from one center. The committee voted seven to two that the results were not generalizable to other patients with the condition. The committee did find that the study results, however, were "clinically meaningful" in a six-to-three vote, responding to a question that FDA had posed about study results and primary endpoint.

Serono Serostim For Short Bowel Syndrome To Receive Review At June 25 Meeting
Serono’s Serostim NDA (21-597) (somatotropin) for the treatment of short bowel syndrome in patients receiving specialized nutritional support will be reviewed by FDA’s Gastrointestinal Drugs Advisory Committee at its June 25 meeting. Serono reported in July 2002 that a Phase III study demonstrated that Serostim in combination with glutamine reduced the requirements for intravenous parenteral nutrition in patients with short bowel syndrome. Short bowel syndrome is a condition which follows extensive surgical removal of the small intestine as a treatment for various conditions, including severe inflammatory bowel disease, trauma or blockage of a blood vessel supplying the bowel. The committee will meet on June 26 to review Axcan's Photofrin (porfimer sodium) for treatment of dysplasia in Barrett's esophagus.

What causes intestinal tissue to grow back after part of the bowel has been removed? And how can scientists use that knowledge to give newborns hope for a normal life? Those are the questions being studied by Brad Warner, MD , and his colleague Christopher Erwin, PhD , in the Cincinnati Children's Division of Pediatric General and Thoracic Surgery . Their research focuses on short bowel syndrome, most often the result of a large portion of the intestine being removed to treat necrotizing enterocolitis in preterm infants. The remnant bowel tries to grow back in length and girth to compensate for the missing portion a process known as the intestinal adaptation response. If the intestine grows back fully, the child can tolerate oral feedings completely. However, if the response is incomplete, the child will need nutrition by vein, which eventually can result in chronic liver failure. Since 1994, Drs. Warner and Erwin have researched how various growth factors, particularly epidermal growth factor (EGF), can enhance the intestinal adaptation response. They have established that the EGF receptor in the intestine is critical, through studies in mice that both inhibited and increased the amount of EGF, thus correspondingly suppressing or amplifying the adaptation response. Their laboratory also pioneered small bowel resection procedures in mice.

When a significant length of the intestinal tract is removed, short bowel syndrome is the result. SBS is characterized by inefficient absorption of nutrients and fluids. The severity of the condition is determined by the amount of bowel removed and the type and length of the remaining bowel, as well as the condition of the remaining bowel and digestive organs, and the presence or absence of the ileocecal valve. The ileum is better able to take over the functions of the jejunum than when the situation is reversed. This is because the important digestive function of absorption of fats, bile salts, and vitamin B12 takes place in the ileum. After bowel surgery, a process called intestinal adaptation occurs in which the remaining bowel improves its ability to absorb nutrients by an increase in its surface area and in the absorptive capacity of individual cells in the intestine and colon. This process begins immediately after intestinal surgery and continues for up to two years. This information is provided as a general educational service
