Male and female sexual dysfunction: Blockbuster indication for multiple pharmacological targets
LeadDiscovery

Publication date: September 2002
"Indication switching", or the redirecting of a drug or a pharmacological target from one disease to another is attractive because it can speed products towards the market for limited additional cost. The launch of sildenafil revolutionized the erectile dysfunction market overnight while at the same time offering one of the most well known examples of "indication switching". Originally developed as a cardiovascular drug Pfizer researchers discovered serendipitously that sildenafil, and PDE5 inhibition in general, was an effective treatment of erectile dysfunction. The consequent upsurge in sexual dysfunction research has resulted in an explosion of data describing the potential of both novel targets and more developed targets for this group of indications.

In women, sex hormones that change across the life cycle, menstrual cycle, and diurnally have direct effects on sexual functioning, and indirect effects via modulation of neurotransmitter systems. These complex neuroendocrine effects lead to the sexual dysfunction seen with antidepressants. Strategies to manage these effects have had some success. They include switching to antidepressants with minimal sexual side effects, addition of hormones and/or antidotes, and lowering the dose of medication. Emerging data on the pathophysiology of sexual function and dysfunction, and new treatment options may lead to improved quality of life for women diagnosed with depression. Introduction ]. In contrast to men, whose erectile dysfunction increases with age, women are much less likely to have age-related sexual dysfunction, according to a paper by an expert on sexual relationships at the University of Chicago and based on a new global study to be presented today, October 10. The survey, the Global Study of Sexual Attitudes and Behaviors , funded by Pfizer, Inc., is based on in- person and telephone interviews in 2001 with 27,500 men and women aged 40-80 in 30 countries worldwide. Researchers looked at equal numbers of men and women equally distributed across four decades. The survey asked about a range of health conditions such as hysterectomies, depression and other illnesses, including diabetes, heart disease, and hypertension. The researchers assessed whether aging increased the risk of sexual dysfunction, even when these health conditions were taken into account. The research team examined sexual dysfunction among men and women as well as other topics related to aging and sexuality. The cross-country differences of sexual dysfunction were reported for persons experiencing the problem for two months or more. Among men, the team found increasing problems with erectile dysfunction (ED) as men age. Among women, the researchers looked at these factors: lack of interest in sex, inability to experience orgasm, pain during intercourse, lack of sexual pleasure and lubrication difficulties. The pooled global survey found that 31 percent of women lacked interest in sex, 22 percent were unable to orgasm, 21 percent found sex unpleasurable, 20 percent had trouble lubricating, and 14 percent experienced pain with sex.

Causes of Sexual Dysfunctions
Lauren Welsh
"Sexual dysfunctions are an important public health concern, to which general health and emotional problems contribute" . What is a sexual dysfunction? A sexual dysfunction is any condition that inhibits someone's ability to enjoy sex. Some common sexual dysfunctions are: hypoactive desire disorder (low sex drive), hyperactive sexual disorder (high sex drive), sexual aversion disorder, lack of lubrication (females), impotence (male erectile disorder), premature ejaculation, vaginismus (prolonged contractions of the vaginal wall that cause painful intercourse), or failure to orgasm during sex . In a study of happily married couples, 14% of men and 15% of women reported that they were either not very satisfied or not at all satisfied with their sex lives. In another study by Laumann in 1992, it was found that the prevalence of sexual dysfunction is 43% in women and 31% in men

by Heather Smith Definition Causes Risk Factors ... Organizations
Definition
Female sexual dysfunction is a chronic disturbance in the sexual response cycle.
Causes
The sexual response cycle consists of three stages: desire, arousal, and orgasm. It relies on mental stimulation, nerve signals, hormones, and blood flow. Sexual dysfunction can arise if any of these processes are disturbed. Sexual dysfunctions include: Hypoactive sexual desire disorder - low sexual desire. Causes include conditions or medications that interfere with nerve signals, such as:
  • Hormone deficiencies Neuropsychiatric disorders
Sexual aversion disorder - strong desire to avoid sexual activity. Caused primarily by psychological factors stemming from traumatic experiences, such as:
  • Childhood sexual abuse Painful sexual experiences Feelings of shame and guilt
Female sexual arousal disorder - inability to attain or maintain a physical response to sexual arousal. Physical causes include conditions and medications that interfere with nerve signals or blood flow, such as:
  • Diabetes Arteriosclerosis Medications, including many blood pressure and psychiatric drugs

Diabetic-Lifestyle offers recipes, menus, medical updates, entertaining, travel - practical information to enhance life while managing diabetes on a daily basis. - Home Female Sexual Dysfunction, Diabetes, and Viagra Last year we shared with our readers information about diabetes and sexual dysfunction. At that time Pfizer had brought out Viagra and jokes about this medication, Viagra web sites, and copy cat drugs were rampant. Now, however, we are receiving questions about women, sexual dysfunction and Viagra. We hope that this article will help you by once again fulfilling our promise to you of making you informed consumers when it comes to living the most complete lives you can. First, allow me to share that Pfizer has updated its Viagra labeling with new warnings in response to postmarketing reports of serious adverse events. Revised in consultation with the FDA, the new labeling is intended to make sure that consumers and doctors are fully informed about the benefits and risks of using Viagra, know that consideration must be given to the cardiovascular status of patients prior to prescribing Viagra, and know how to safely use the drug. The label has always warned against taking the medication with nitrate medication because the combination can drop blood pressure to levels that can have fatal consequences. The revised label warns specifically those in whom the drug had not yet been studied, such as men who have had a heart attack, stroke or life-threatening arrhythmia in the past six months, those with significant hypotension or hypertension, those with unstable angina and those with retinitis pigmentosa.

Soulwork Life Coach Training
Common partnership problems include male impotence and female frigidity. These terms commonly describe an inability to participate in normal sexual activity, or to enjoy orgasm. These sexual or psychosexual dysfunctions often have physical, relationship or emotional causes. Some sex dysfunctions can be managed in the short term by drugs such as Viagra , and can be treated for the long term with therapeutic coaching . If left untreated, these problems can increase the likelihood of a partner's sexual affairs or divorce The common emotional causes of sexual dysfunction are interrelated and include:
  • emotional incest ( son with mother - guilt) emotional incest ( daughter with father - guilt) consequences of abortions (guilt) promiscuous behavior (multiple sexual partners - guilt) abandonment or betrayal of past partners (guilt) power games between partners (anger / fear) victim games between partners (anger) sexually transmitted diseases (fear / guilt) sexual abuse between close family members (anger / guilt)

80. Female Sexual Function
sexual dysfunctions are treatable. Education, information and counseling are often very effective in relieving sexual dysfunctions.
http://www.mckinley.uiuc.edu/Handouts/femalefunctiondysfunction.html
Female Sexual Function/Dysfunction There are several stages that outline the body's response to sexual stimulation. These stages are fluid, and very individual. Although most females will progress through the stages in order, the amount of time spent in each stage can vary dramatically. FUNCTION Stage One - Excitement
  • Blood begins to build up (vasocongestion) in the blood vessels of the genital region. A slippery fluid seeps through the lining of the vagina, functioning as a lubricant. The clitoris begins to swell somewhat and may elongate slightly. The nipples become harder and erect and there is a darkening of the skin through the neck, breasts and upper abdomen during sexual excitement; this is termed the "sex flush." General muscular tension builds in the body as heart rate and blood pressure increase.
Stage Two - Plateau
  • The outer third of the vaginal wall becomes swollen with blood, narrowing the space within the vagina slightly. Swelling seems to create the tension that is an important precursor of orgasms, and is termed the "orgasmic platform." The clitoris retracts under its foreskin so that it no longer receives any direct stimulation. The breasts have usually expanded by this time and nipple erection may be maintained. Muscular tension continues to increase, along with the heart rate, blood pressure and respiration rate. The heart rate usually increases to between 110-175 beats per minute.
