      Introduction
      The problem of an inadequate erection is probably one of the biggest issues a man confronts. Most men experience erectile problems on occasion, but impotence, also called erectile dysfunction, is defined as "the persistent failure to develop and maintain erections of sufficient rigidity for penetrative sexual intercourse." Of course, men have other kinds of sexual problems including lack of desire and problems with ejaculation, yet impotence is the most common and troubling. The best way for men to begin solving erectile problems is by reading about men's sexual system - anatomy, physiology, diseases, drugs, diagnosis and treatments. Some problems may be solved simply and others may require a visit to your family doctor or a urologist. In either case, we encourage you to become an educated health care consumer, which should help you regardless of the cause or cure for your problem.

Common Sexual Dysfunctions
In diagnosis and history taking, it is important to remember that each dysfunction covered in this course can be lifelong (has always been present), acquired (has not always been present), situational (occurs in some situations and not others), or generalized (occurs regardless of the situation). The following list, while far from exhaustive, describes common sexual dysfunctions, their possible causes, and treatments. Note:
Inhibited sexual desire (ISD)
Sexual desire changes over the course of our lives, and occasional loss of desire in either sex is not uncommon. In ISD, however, there is persistent loss of desire that disrupts sexual relationships. It is characterized by diminished sexual attraction, decreased sexual activity, few or no sexual dreams or fantasies, and diminished attention to erotic material by one or both partners. ISD is the most common presenting sexual dysfunction in women and is less commonly reported in men. Female sexual arousal disorder (FSAD) is the name for persistent or recurrent inability to achieve or maintain an adequate lubrication-swelling response. (A woman with FSAD may or may not find enjoyment in physical contact.)

Purpose of This Module
Most women and men at one time or another experience some alteration in sexual response, and an occasional problem in this regard is not an indication of dysfunction. Even a single episode of altered response or failure to perform sexually can cause unnecessary concern and may even perpetuate the problem, since anxiety about sexual performance can diminish sexual functioning. Differentiating between natural alterations in sexual response and actual sexual dysfunction can help the client avoid becoming drawn into a cycle of anxiety and can help providers diagnose and treat the causes of true sexual dysfunction. Female sexual dysfunction has basically been neglected because no therapy was available. With the advent of Viagra by Pfizer Pharmaceuticals , effective therapy for some forms of female sexual dysfunction may become available to the general public. Female sexual dysfunction can be classified into five basic areas including: 1) Desire problems 2) Arousal difficulties 3) Lubrication factors 4) Pelvic congestion 5) Orgasmic difficulties. For males, desire problems are minimal and usually related to hormonal difficulties. For females, desire problems occur in more than 33 percent of dysfunction cases. This is probably related to the more complex psychological aspects of female sexuality. On the other hand, many of these patients can be helped. In males, desire problems make up only five percent of all sexual dysfunction. Arousal, lubrication, and pelvic congestion issues together probably represent about half of all female sexual problems and, fortunately, is the area in which pharmacology in the near future appears to offer the greatest hope.

Over 40% of women may have a sexual problem at sometime in their lives. While it is natural to look for medical diseases or hormonal deficiencies as the sole cause, most sexual dysfunction is a combination of both physiological and psychological factors. In a recent continuing medical education article, Petok WD: A practical approach to evaluating female sexual dysfunction. OBG Management 1999;11:68-77, outline the six most commonly seen female sexual dysfunctions.
What are the most commonly seen sexual problems that women have? Petok lists six commonly seen sexual problems that present to doctors and sex therapists. They are:
  • Hypoactive sexual desire disorder (HSDD) - lack of sexual desire with a sort of passive apathy, not seeking out sex but not actively resisting when a partner initiates sex.
  • Sexual aversion disorder (SAD) - an active aversion to all genital sexual contact which may manifest itself as revulsion, fear or anger.
  • Female sexual arousal disorder (FSAD) - rare, if ever, thoughts about sex or getting "turned on" except in perhaps a theoretical way. Lay persons would term this frigidity.

The article, written by Ray Moynihan of the Australian Financial Review , says drug companies are sponsoring medical research into invented medical disorders such as "female sexual dysfunction" in order to cash in on the lucrative post-Viagra market. The article begins with the question: "Is a new disorder being identified to meet unmet needs or to build markets for new medications?" Moynihan suggests the latter, that drug companies are sponsoring medical research that will create medical disorders that can be labelled "dysfunctions" so that a drug can be found to cure it. Drug companies contacted Friday disclaim any attempts to "invent" medical dysfunctions for the sole purpose of profiting by it. They suggest women with sexual difficulties could benefit from a Viagra-type drug if it can be shown it improves or enhances sexual performance. Pfizer Inc. launched Viagra in 1998 and since then more than 17 million men have obtained prescriptions for the drug as a treatment for erectile difficulties.     Sexual dysfunction takes different forms in men and women. A dysfunction can be life-long and always present, acquired, situational, or generalized, occurring despite the situation. A man may have a sexual problem if he:
    • ejaculates before he or his partner desires
    • does not ejaculate, or experiences delayed ejaculation
    • is unable to have an erection sufficient for pleasurable intercourse feels pain during intercourse
