The definition of sexual dysfunction involves problems involving arousal, lack of desire, fear of intimacy, or overactive sexual activity. Female Sexual Dysfunction: Evaluation and Treatment
NANCY A. PHILLIPS, M.D.
Wellington School of Medicine, University of Otago, Wellington, New Zealand
A patient information handout on sexual dysfunction in women, written by the author of this article, is provided on page 141 S exuality is a complex process, coordinated by the neurologic, vascular and endocrine systems. Individually, sexuality incorporates family, societal and religious beliefs, and is altered with aging, health status and personal experience. In addition, sexual activity incorporates interpersonal relationships, each partner bringing unique attitudes, needs and responses into the coupling. A breakdown in any of these areas may lead to sexual dysfunction. See editorial
on page 52.
Primary care physicians, skilled in the treatment of medical and psychologic disorders, often feel unqualified to treat patients with sexual dysfunction. However, with an understanding of sexual functioning and application of general medical and gynecologic treatments to sexual issues, sexual dysfunction may be effectively approached with the same skills. The latter includes obtaining a complete patient history, conducting a physical examination, application of basic treatment strategies, providing patient education and reassurance, and recommending appropriate referral when indicated. Diagnosis Female sexual dysfunction can be subdivided into desire, arousal, orgasmic and sexual pain disorders. Sexual pain disorders include dyspareunia and vaginismus.

Sexual Disorders
What are sexual disorders?

Sexual dysfunctions are disorders related to a particular phase of the sexual response cycle. For example, sexual dysfunctions include sexual desire disorders, sexual arousal disorders, orgasm disorders, and sexual pain disorders. If a person has difficulty with some phase of the sexual response cycle or a person experiences pain with sexual intercourse, he/she may have a sexual dysfunction.
Examples of sexual dysfunctions include:
  • Hypoactive Sexual Desire Disorder. This disorder may be present when a person has decreased sexual fantasies and a decreased or absent desire for sexual activity. In order to be considered a sexual disorder the decreased desire must cause a problem for the individual. In this situation the person usually does not initiate sexual activity and may be slow to respond to his/her partner's sexual advances. This disorder can be present in adolescents and can persist throughout a person's life. Many times, however, the lowered sexual desire occurs during adulthood, often times following a period of stress.
  • Sexual Aversion Disorder.

In addition, some types of medications are increasing incidence of sexual dysfunction in both males and females. For example, approximately 60% of the estimated 150 million people who are receiving anti-depressant therapy, are treated with selective serotonin re-uptake inhibitors (SSRIs), and about 30-40% of these patients experience some form of sexual dysfunction as a side effect of the treatment. The most conservative estimates suggest that over 30 million men in the U. S., over 30 million men
in the E. U., and over 150 million men worldwide are affected with erectile dysfunction (ED). More liberal estimates estimate that over half of all men 40 years of age and older have some form of erectile dysfunction. However, it appears that only one in 20 affected men seeks medical treatment.
Incidence of sexual dysfunction, primarily erectile dysfunction, resulting from nerve damage, is particularly high in patients with prostate cancer, following prostatectomy, affecting 60 to 85 percent

    What is Impotence
Impotence is a consistent inability to sustain an erection sufficient for sexual intercourse. Medical professionals often use the term "erectile dysfunction" to describe this disorder and to differentiate it from other problems that interfere with sexual intercourse, such as lack of sexual desire and problems with ejaculation and orgasm. This fact sheet focuses on impotence defined as erectile dysfunction. Impotence can be a total inability to achieve erection, an inconsistent ability to do so, or a tendency to sustain only brief erections. These variations make defining impotence and estimating its incidence difficult. Experts believe impotence affects between 10 and 15 million American men. In 1985, the National Ambulatory Medical Care Survey counted 525,000 doctor-office visits for erectile dysfunction.

Disturbances in the sexual response cycle or pain associated with sexual arousal or intercourse. (See also Chs. 220 and The sexual response cycle is mediated by a delicate, balanced interplay between the sympathetic and parasympathetic nervous systems. Vasocongestion is largely mediated by parasympathetic (cholinergic) outflow; orgasm is predominantly sympathetic (adrenergic). Ejaculation is almost entirely sympathetic; emission involves sympathetic and parasympathetic stimulation. These responses are easily inhibited by cortical influences or by impaired hormonal, neural, or vascular mechanisms. - and -adrenergic blockers may desynchronize emission, ejaculation, and perineal muscle contractions during orgasm, and serotonin agonists frequently interfere with desire and orgasm. Disorders of sexual response may involve one or more of the cycle's phases. Generally, both the subjective components of desire, arousal, and pleasure and the objective components of performance, vasocongestion, and orgasm are disturbed, although any may be affected independently.

Information Return to top Sexual difficulties can begin early in a person's sex life or they may develop after an individual has previously experienced enjoyable and satisfying sex. A problem may develop gradually over time, or may occur suddenly as a total or partial inability to participate in one or more stages of the sexual act. The causes of sexual difficulties can be physical, psychological, or both.
Emotional factors affecting sex include both interpersonal problems (such as marital/relationship problems, or lack of trust and open communication between partners) and psychological problems within the individual ( depression , sexual fears or guilt, past sexual trauma, and so on).
