Semantic-pragmatic disorders Semantic refers to the meanings of words and phrases Pragmatic refers to the use of language in a social context (knowing what to say and when and how to say it to other people) Children with semantic difficulties struggle to understand the meaning of sentences or words. Words which refer to abstract concepts such as feelings (sad, puzzled, jealous) or status (important, official) can be especially hard for these children to understand. They misinterpret and take literally phrases such as "Cut it out!", "Stop it" or "He's a push-over". Often it can be difficult for children with a semantic disorder to work out the topic or key information in a sentence. Children with pragmatic difficulties struggle to use language socially.

Semantic-Pragmatic Disorder
Semantic-pragmatic disorder is very similar to Asperger Syndrome and a common characteristic of people with nonverbal learning disabilities and hyperlexia . The condition involves deficits in social and nonverbal communication such as the give and take of communication as well as interpreting cues from body language and expression. While Semantic-Pragmatic Disorder itself is almost always milder than Asperger's, the vast majority of people with Asperger Syndrome have pragmatic language deficits and could benefit from the techniques for dealing with this disorder. Semantic-Pragmatic Disorder may be a variant PDD or it may be just a descriptive term for the language impairment in autistic spectrum disorders. The following is the summary from SEMANTIC PRAGMATIC IMPAIRMENTS by Dr. Jane Shields:
  • "Semantic Pragmatic Disorder" is a term which has been used, mostly in the UK and by speech and language therapists, for the last 10 - 15 years. The term was originally used to describe a developmental language disorder, affecting two aspects of communication: "semantics" and "pragmatics".

Hi! My almost 5-year-old son has always had speech problems. He was late, had a kind of fantasy language (with few sounds), two-word sentences for a long time.
Pronounciation was bad (is better now). Now he has a rather limited vocabulary, doesn't use prepositions or other small words (or missuses them). And... he doesn't understand as much as he should. Has started stuttering like his older brother (who has Tourettes Syndrom, and who never had any language problems apart from the stutter. On the contrary: very verbal).
Speech therapist made a test. She just told me a little, we have another appointment in June: he's not just delayed, but there are some things he doesn't understand or doesn't use when he speaks - abstract things (like prepositions and other words that don't mean much). He doesn't seem to understand negative statements either (but can use them himself. It's not just the difficult things he can't use or understand, he can do some of the difficult tasks and misses on some of the easy ones. I had the impression THAT HE WAS LISTENING TO THE TONE OF HER VOICE, WHAT SHE PUT THE STRESS ON, NOT TO THE CONTENT OF WHAT SHE SAID.

HISTORY Semantic-Pragmatic Disorder was originally defined in the literature on Language Disorder in 1983, by Rapin and Allen, although at that time it was classified as a syndrome. They referred to a group of children who presented with mild Autistic features and specific semantic pragmatic language problems. In babyhood, parents often described them as model babies or by contrast babies who seemed to cry too much. Many of these children babbled little or very late and went on using 'jargon' speech much longer than other children of the same age. Their first words were late and learning language was a hard slog. Some had other speech disorders too. Problems were usually first identified between 18 months and 2 years when the child had few if any real words.
