In other words, they may talk at home but may not talk at school or unfamiliar settings. It is unknown why this happens. Sometimes children with develop Selective Mutism following some sort of traumatic experience. In others, the disorder may develop as a result of social anxiety. A rare disorder, Selective Mutism is found in less than 1% of the population and typically occurs before the age of five.     What is selective mutism?
    The most common feature of selective mutism is when your child does not speak in social situations, including school, even though he can speak and understand spoken language. It used to be called elective mutism.
    What are the characteristics of selective mutism?
    You may notice your child is overly shy when she enters preschool or kindergarten and seems unable to talk to others in the new class. You may have noticed that your child had a limited group of people she spoke to before starting school. Then, after your child is in school for several months, you find out that your child is not talking there. Some characteristics of selective mutism are:
    • Continues to not speak in social situations where talking is expected (such as school, Sunday school, play groups, around parents' friends), even though the child can speak in other situations (such as to family members). The situations of "nontalking" begin to interfere with the child's school work and social development.

    A Family and Genetic Study of Selective Mutism:
    Relationship to Social Anxiety Disorder and Serotonin Genes This SM research study is a collaboration between SMG~CAN , Dr Elisa Shipon-Blum and The University of California, San Diego ( Dr. Murray Stein)
    Study Objectives:
    The objective of this study is to interview families in order to confirm that social anxiety disorder segregates within families of Selectively Mute children. A secondary objective is to determine the genetic make-up of parents, siblings, and the SM child in terms of an association with specific genes, such as the serotonin transporter (SLC6A4) genes and subtypes which has been linked to anxiety disorders. Findings from this study will improve our understanding of the hereditary nature of Selective Mutism, it's relationship to DSM-IV anxiety disorders, and it's possible neurobiological association.
    Study Design:
    50 families with SM children will participate in this study. Dr. Shipon-Blum will evaluate and either diagnose or confirm a previous diagnosis of SM to screen for the interview portion of the study. Upon acceptance into this study, if each SM family would TRY to provide us with another family (without SM) to serve as a control for the study.

    What is Selective Mutism?
    Selective Mutism (SM) is a complex childhood anxiety disorder characterized by a child’s inability to speak in select social settings, such as school.  These children are able to talk normally in settings where they are comfortable, secure and relaxed. Over 90% of children with Selective Mutism also have social phobia or social anxiety a persistent fear of social or performance situations. SM is quite debilitating and painful to the child.  Children and adolescents with Selective Mutism have an actual fear of speaking, often standing motionless with fear when they are confronted with specific social situations where there is an expectation to talk. This can be quite heart wrenching for both child and parents; these children are so anxious they literally freeze, are expressionless, unemotional and, often, socially isolated.  Although it is difficult to determine prevalence due to problems in diagnosis and misdiagnosis, recent research estimates that at least seven of every one thousand children have SM.

    Children who suffer in silence.
    Children with selective mutism are still, far too often, misdiagnosed. The inability to speak is attributed to shyness, developmental delay, learning disabilities or autism. Worse, children are accused of being defiant, oppositional, manipulative or insolent. They are alternately punished and bribed. Their school life becomes a nightmare as they are made to repeat grades or attend inappropriate special education classes. And their parents undergo suspicion, or sometimes even charges, of child abuse. And, we’ll point out right here and now, that there is absolutely no evidence that the selective mutism is caused by abuse or neglect by parents.
