Selective Mutism Selective mutism is characterized by a consistent failure to speak in specific social situations in which there is an expectation for speaking. Children with selective mutism have the ability to both speak and understand language, but fail to use this ability as a result of what we would consider to be social anxiety in adults. Most children with selective mutism function normally in other areas of their lives. Selective mutism is not a communications disorder and is not part of a developmental disorder. By definition, selective mutism does not include children with conduct disorders, oppositional/defiant behavior, and/or attention-deficit hyperactivity disorder. T he principal problem in children with selective mutism appears to be anxiety . This anxiety (which causes avoidance) seems closest to the definition of social anxiety disorder (social phobia). Indeed, most adults with social anxiety disorder relate strongly and can fully understand selective mutism, even though they themselves were not necessarily children with selective mutism. It also appears that cognitive-behavioral therapy, with the emphasis being on the behavioral component (depending on the age of the child) is the therapy of choice.

Selective mutism has been mistakenly associated ín the past with childhood abuse or trauma, charges that researchers say are not supported by scientific evidence. Until recently, ít was called “elective mutism,” but doctors changed the name because ít implied a willful stubbornness of the child that “we’ve found ís really not the case,” says Anne Marie Albano, director of the anxiety disorders program at the New York University Child Study Center. A child’s inability to speak ín public ís not only frustrating for parents, ít can also be frightening. When Samantha missed the bus home from school one day, she was unable to tell school officials that she needed to call home for a ride. Instead she began the 2½-mile trek home, until her mother, ín a frantic search of the neighborhood, spotted her. “I worry about her safety,” her mother says. “I especially worry that she won’t be able to ask for help íf she needs ít.” How Shy is Too Shy?

This psychiatric disorder most commonly found in children, characterized by a persistent failure to speak, so as to be heard in select settings which continues for a reasonable period of time, usually a month or two. These children understand language use and have the ability to speak. In typical cases, they speak to their parents and a few select others. Most do not speak in school or in other social situations. Generally, most function normally in other ways, although some may have additional disabilities. Most learn age appropriate skills and academics. Currently, SM is a condition of severe anxiety or phobia. SM may be associated to a variety of things, but the exact cause is unknown. Heredity has also become a possibility, many cases being noted by mothers and other family members that they had the same difficulties. but years ago even less was known about Selective Mutism and it was generally excused as being overly shy. With this new year we all have our work cut out for us. Inspiration by the success stories shared by those that wrote in and the firm belief that, if we work hard enough, we can make a difference in our children's quality of life coping with Selective Mutism.

    It was over ten years ago when I first heard of Selective Mutism. I was a newly hired public school Speech-Language Pathologist. Fresh from graduate school, I took my position as a Communication Disorders Specialist very seriously. It is only with the experience of time (or is it age) that I can be humbled to now admit that I didn’t know as much as I thought I did. It is with this frame of mind that I’d like to share with you what I have learned, but to do that, I have to also share with you my mistakes. The old adage, we learn from our mistakes could not be more true.

An Integrated Treatment Approach
by Robert L. Schum Selective mutism is a uniquely challenging childhood communication disorder that demands a coordinated treatment approach. Speech-language pathologists are in an excellent position to coordinate intervention among family, classroom teachers, and other clinicians. The Problem Selective mutism is defined as a failure to speak in specific s ocial situations despite speaking in other situations, and it is typically a symptom of an underlying anxiety disorder. Children with selective mutism can speak normally in certain settings, such as within their home or when they are alone with their parents. However, they fail to speak in other social settings, such as at school or at other places outside their home. Other symptoms associated with selective mutism can include excessive shyness, withdrawal, dependency upon parents, and oppositional behavior. Most cases of selective mutism are not the result of a single traumatic event, but rather are the manifestation of a chronic pattern of anxiety. Mutism is not passive-aggressive behavior. Mute children report that they want to speak in social settings, but are afraid to do so.

References and related books Introduction Those that have worked with selectively mute children have encountered wide variations in their social actions. Some children enjoy contact with others and will play easily, but remain silent. Some have a close friend who often speaks for them by interpreting gestures. Others find all aspects of social situations uncomfortable and do not participate at all. Whatever form the condition takes, it can persist. There are children in the 2nd, 3rd, and 4th grades who have never spoken in school. There are students in high school who have not uttered any or no more than a few words in a school setting. As you can imagine, the condition can have dramatically negative effects on social functioning. Back to top.
