Definition
Seborrheic dermatitis is a skin condition that causes a scaly, itchy rash. It can appear as a mild, white scaling or a yellowish or reddish scaly rash. Most commonly affected areas are along the hairline, in and behind the ears, and on the eyebrows, nose, and chest. Dandruff is a type of seborrheic dermatitis. It is the scaling of the skin on the scalp. It can be dry or oily and sometimes itchy. Seborrheic dermatitis involves swelling and redness of the underlying skin (inflammation), while dandruff is just a mild, white scaling of the scalp (no inflammation). Causes
The exact cause of seborrheic dermatitis is unknown, though common skin yeast organisms called Malassezia may play a role in some persons. Seborrheic dermatitis seems to run in families and to affect people with oily skin or hair. Seborrheic dermatitis is a common chronic skin condition affecting the scalp and areas of skin where oil glands are abundant. In its mildest variety, called "dandruff", it affects the scalp and shows mainly shedding of scales with itching. More severe varieties include a troublesome oiliness of the hair and scalp, sores, greasy scales, crusts, and pimples in the scalp. Other areas of the skin can be and are often affected. Here we see the condition on areas of the forehead near the hairline, the eyebrows and the eyelid margins, the sides of the nose, in and around the ears, the central chest, belly button, and genitals. The cause of seborrheic dermatitis is not well understood. It is not contagious nor due to any infectious agents. There is some relationship between this condition and a skin disease called psoriasis. In fact, some individuals with severe cases of seborrheic dermatitis are later found to have psoriasis. The production of flakes of skin which shed from the scalp is a normal condition, as is the production of oil from the oil glands. For some reason, the over production of either or both of these materials is found in most cases of this condition.

Diagnosis/Definition
  • Chronic superficial inflammatory disease of the skin. Predilection for the scalp, eyebrows, nasolabial creases, ears, sternal area, groin and gluteal crease. Characterized by scant, loose, dry, moist or greasy scales and by crusted, pinkish or yellowish patches of various shapes and sizes; by remissions and exacerbations; and more or less by itching. On the scalp manifests as dry powdery dandruff or an oily type with erythema and thick crusts. Frequently spreads beyond the hairy scalp. On and in the ears, seborrheic dermatitis is frequently mistaken for otitis externa. There is scaling in the aural canals, around the auditory meatus and in the postauricular region or under the lobe. In these areas the skin often becomes red, fissured and swollen.
Initial Diagnosis and Management
  • Evaluation: A focused history and physical examination. Areas to be examined should include the scalp, ears and mid-face.
