Definition
Scurvy is a condition caused by a lack of vitamin C (ascorbic acid) in the diet. Signs of scurvy include tiredness, muscle weakness, joint and muscle aches, a rash on the legs, and bleeding gums. In the past, scurvy was common among sailors and other people deprived of fresh fruits and vegetables for long periods of time. Description
Scurvy is very rare in countries where fresh fruits and vegetables are readily available and where processed foods have vitamin C added. Vitamin C is an important antioxidant vitamin involved in the development of connective tissues, lipid and vitamin metabolism, biosynthesis of neurotransmitters, immune function, and wound healing. It is found in fruits, especially citrus fruits like oranges, lemons, and grapefruit, and in green leafy vegetables like broccoli and spinach. In adults, it may take several months of vitamin C deficiency before symptoms of scurvy develop. Currently, the recommended daily allowance (RDA) for vitamin C is 50-60 mg/day for adults; 35 mg/day for infants; 40-45 mg/day for children 1-14; 70 mg/day during pregnancy ; and 90-95 mg/day during lactation . The body's need for vitamin C increases when a person is under stress , smoking, or taking certain medications.

Vitamin C has an important role in the synthesis of collagen and manifestations of the disease reflect deficient collagen; thus blood vessels are excessively permeable and rupture easily and the bone shows defective formation and lacks tensile strength. Clinically children with scurvy appear malnourished and irritable with generalized weakness, petechial haemorrhages and bruising, poor wound healing and tender, painful extremities. Radiographs show generalized osteopenia, loss of the normal trabecular pattern and thin cortices ( Fig.1 ). Metaphyses show a characteristic transverse lucent band (Trummerfeld's zone) representing the primary spongiosa and relative increased density of the adjacent zone of provisional calcification (white line of Frankel). Fragility of the zone of provisional calcification may cause irregularity and peripheral metaphyseal spurring (Pelken spur). These changes are most prominent at sites of active endochondral bone growth such as distal femur, proximal and distal tibia/fibula, proximal humerus and distal radius/ulna. The epiphyses tend to be osteopenic with sparing of the rim causing a relatively dense peripheral ring (see Wimbergers sign ). Extensive new bone formation occurs along the shafts of bones in response to subperiosteal haemorrhage.

Scurvy in the past
Scurvy was a serious problem in the past, when fresh fruitsand vegetables were not available during the winter in many parts of the world.It was especially common among sailors in the days when only nonperishable foods could be stocked aboard ship. More than half the crew of Vasco da Gama died fromscurvy on his first trip (1497-99) around the Cape of Good Hope. In 1747 the Scottish naval surgeon James Lind treated scurvy-ridden sailors with lemons and oranges and obtained dramatic cures. In 1795 the British navy began to distribute regular rations of lime juice during long sea voyages (hence the name limeys for British sailors), a measure that was largely successful in preventing scurvy. It was probably the first disease to be definitely associated with a dietary deficiency.

One malady that killed sailors by the score was caused not by what was in the food but by what was not. Scurvy, caused by a lack of Vitamin C, was the great killer of mariners on long oceanic voyages. During his voyage to India in 1497-98, Vasco da Gama lost two-thirds of his men to scurvy. Two hundred and fifty years later, Commodore Anson lost over half of his nearly two thousand men during a four-year voyage. After a full week of exposure to the news and entertainment provided by our modern culture, I've decided I've had enough. I'm going to shuck it all and pursue a simpler life. Herewith, a list of a few things I'll be happy to get shut of:
  • Bennifer: Get married or don't, break up or don't. And stop whining about the press "hounding" you when you set up so danged many obvious photo ops. You don't want press? Star in a Kevin Costner movie. Hurricane hysteria: For the 300 million or so of us who were in no way affected by Isabel, the coverage was a little much. A Houston station sent their favorite pudgy little stormchaser to Virginia Beach so they could get local only footage of him being blown back and forth. The only problem was, the bodies of all the other local TV personalities doing the same thing were creating a windbreak.

    Home A Short History of Scury I think it's particularly amazing that the information on the cure of scurvy was available a long time before it was acted upon. Hippocrates described Scurvy: bleeding gums, hemorrhaging and death, as early as the 5th c. BC. During the Crusades the disease became widespread. In 1250 it forced the retreat and capture of St. Louis with all his knights. It didn't become a major problem, however, until the age of exploration. Long sea voyages lacking in fresh food led to vitamin C deficiency. There is a legend that during one of Christopher Columbus's voyages some Portuguese sailors had scurvy and wanted to be dropped off at one of the nearby islands and die there rather then dying on board and being fed to the fish. While the men were on the island they ate some of the island's fresh fruits and plants and to their amazement began to recover. When Columbus's ships passed by several months later, the pilot was surprised to see the men waving from land alive and healthy. The island was named Curacao, meaning Cure. During Jacques Cartier's second voyage (1535-36), he used two indian interpreter-guides to pilot him up the St. Lawrence River to Stadacona (the site of modern Quebec). Wintering in Stadacona, 25 members of his crew died from scurvy before the indians taught them to drink a brew of white cedar which saved the rest.
