Vitamin C and Scurvy
Vitamin C (ascorbic acid), a reducing agent, is necessary to maintain the enzyme prolyl hydroxylase in an active form, most likely by keeping its iron atom in a reduced state. The precursor molecule to the protein collagen, procollagen, contains an unusual amino acid sequence in that every third amino acid is a glycine and contains a high frequency of two amino acids not found in any other proteins - hydroxyproline and hydroxylysine. These latter two amino acids are converted from proline and lysine, respectively, after the procollagen molecule has been synthesized. The hydroxylation of proline and lysine in procollagen is carried out by the enzyme prolyl hydroxylase using vitamin C as a cofactor. A vitamin C deficiency results in an underhydroxylation of proline and lysine in collagen which results in a lower melting temperature of the resulting collagen fibers which causes a breakdown of the protein collagen needed for connective tissue, bones and dentin, the major portion of teeth. Collagen is a cementing material that binds cells together, and is an essential connective tissue protein in the body. Whenever the body is wounded, collagen glues the separated tissues together to form a scar. A lack of collagen causes the walls of the body's blood capillaries to break down and hemorrhaging occurs in cells throughout the body. When capillaries lose the "glue" that holds them together, symptoms of scurvy appear.

A Simple Experiment on Scurvy
I n 1747 as the HMS Salisbury sailed from England to the Plymouth Colony, the ship's physician, James Lind, performed a simple experiment to determine what might be effective as a cure for scurvy. Scurvy was described as early as 1500 BC in the Ebers papyrus and other descriptions appeared in Greek and Roman writings. It struck the crew of Magellan's around the world journey in 1519-22 and a British report in 1600 indicated that in the previous 20 years some 10,000 mariners had been destroyed by the disease. O n May 20, 1747, Lind divided his 12 ill men into six groups of two each. All 12 shared a common diet for breakfast, lunch and dinner but each group received a different supplement as follows:
  • quart of apple juice daily 25 drops of elixir vitriol (sulfuric acid and aromatics) two spoonfuls of vinegar three times a day concoction of herbs and spices half-pint of sea water daily two oranges and one lemon daily
  • T he two men who ate the oranges and lemon recovered immediately. One was fit for duty in six days and the other was also well in six days and was appointed as nurse to all the others. The two men who drank apple cider improved but were not well enough to work. None of the others showed any improvement. M r. Lind concluded that

Scurvy does not occur in most animals because they can synthesize their own vitamin C, but humans, other primates, guinea pigs, and a few other species lack an enzyme necessary for such synthesis and must obtain vitamin C through their diet. Vitamin C is widespread in plant tissues, with particularly high concentrations occurring in citrus fruits (oranges, lemons, limes, grapefruits); tomatoes, potatoes, cabbages, and green peppers are also good sources of this vitamin.

It is caused by a dietary lack of vitamin C (ascorbic acid), a nutrient found in many fresh fruits and vegetables, particularly the citrus fruits. Ascorbic acid is important in the formation of collagen (an element of normal tissues), and any deficiency interferes with normal tissue synthesis.
      How can I tell if I have scurvy?
    Scurvy is characterized by swollen and bleeding gums with loosened teeth, soreness and stiffness of the joints and lower extremities, bleeding under the skin and in deep tissues, slow wound healing, and anemia.     Background Scurvy is Vitamin C deficiency. It occupies an interesting place in the history of the world when English sailors became progressively ill after long voyages at sea. It was only with the observation that the consumption of fruit helped to ward off the devastating effects of scurvy did the sailors begin to transport limes with them. Hence, the term Limeys came to refer to English sailors. Scurvy results because humans cannot synthesize vitamin C (ascorbic acid) in our bodies. We lack the enzyme L-gluconolactone which converts L-gluconogammalactone to L-ascorbic acid (vitamin C). Curiously, guinea pigs are the only other mammal lacking this enzyme. Vitamin C is essential for collagen synthesis. Any deficiency will affect any organ or structure that requires collagen. Patients may present with hyperkeratotic papules centered in follicles. Hair may grow in a corkscrew-like pattern and may be associated with hemorrhage, often with minor trauma. In chronic disease, ecchymoses, usually over pressure points such as the shins, may occur. The mouth may show gingival hypertrophy. Hemorrhage may occur in any mucosal surface including conjunctiva, subungua, and skin. In addition, hemorrhage in the skeletal muscle, intra-articular sites, and subperiosteal region of the epiphyses of the long bones may occur.     Scurvy likely cause of Afghanistan outbreak
    KABUL, Afghanistan (CNN) Officials with the World Health Organization said Friday that they believed a vitamin C deficiency is at the root of an outbreak of disease in a remote, central Afghan province. A spokeswoman for the U.N.-affiliated group said doctors were still not completely clear about the outbreak, but now believe it to be scurvy with a secondary infection. Spokeswoman Loretta Hieber-Girardet in Kabul said WHO is ready to send doctors with supplies of the vitamin to the area, but the region is difficult to access without helicopters, which are in short supply. The lack of access to the region has also hindered identification of the disease. Forty people in Ghowr province have died from the disease, and aid workers have seen symptoms in at least 80 people in a dozen villages. Two staff members of the Action Against Hunger humanitarian group also have shown signs of the disease and are under observation. The group said it wants to evacuate its staff from the area.
