There are several types of scoliosis. Fortunately, most are rare. The common type is "idiopathic scoliosis," so called because the cause is unknown. It affects about 4% of the population, but is more common among females. Idiopathic scoliosis usually starts between the ages of 10 and 12. As the child grows, there is a chance the curve can progress (worsen). Most curves will not worsen during adolescence and worsening after growth is complete is unusual. Other types of scoliosis include congenital (caused by an abnormally shaped bone that is present at birth); neuromuscular (due to a neuromuscular disease such as cerebral palsy, muscular dystrophy, etc.); acquired (following a fracture, radiation therapy for cancer, etc.); and juvenile. Juvenile scoliosis is similar to adolescent scoliosis, but it is much more likely to progress. Before considering treatments for scoliosis, it is important to know the natural history of the disease (what happens if left untreated). Curves that measure 10 degrees or less are considered "normal" and do not interfere with strength, joint mobility, endurance, or any other body function. They are not true scoliosis, almost never progress, and do not increase the likelihood of developing back pain, arthritis, disc herniation, or any other musculoskeletal problem. Treatment of scoliotic curves of 10 degrees or less is therefore unnecessary. Sometimes these minimally curved spines even get straighter on their own.

What is scoliosis?
Scoliosis is a complex twisting of the spine that affects the alignment of the bones from nearly every angle. While one in ten persons will have some curvature, only two or three out of a thousand persons need treatment. Some cases of scoliosis are due to an underlying muscle or nerve problem, or a defect in bone formation. Most scoliosis, however, is called "idiopathic" meaning that the cause is not known. It is known, however, that idiopathic scoliosis:
  • usually develops in young people aged ten to twelve years old;
  • has a tendency to run in families;
  • can usually be prevented from worsening in a growing adolescent by wearing a brace.
    What is a scoliosis brace?
    The brace (orthosis) is a device to hold the spine in a straighter position during the growing years. The brace is prescribed by an orthopaedic specialist and is usually worn nearly full-time until bone growth has stopped. The Boston-style brace ( photo ) is a plastic body jacket that fits snugly around the body to exert pressure on the ribs and back, pushing the spine into a straighter position.     Scoloiosis May Result in Back Pain

    I have scoliosis of the spine with a curvature of more than 35 degrees. I had no problems as a young girl. Over the last 7 to 8 years, my posture has become more exaggerated and I’ve had increasing pain. I’ve been to several orthopedists, a chiropractor and a pain management center. My bone density is normal and I’m otherwise quite healthy. Do you have any suggestions for a youthful appearing senior citizen in otherwise good health with a lot of back and leg pain?
    A: Scoliosis, or curvature of the spine, is a condition most commonly discovered in young women, that if uncorrected by the onset of menstruation, can result in a permanent deformity. Your experience would suggest that while you were symptom free, your spine was developing arthritis that is now most likely the source of your pain, either from local irritation, nerve root compression, muscle spasm or a combination. You mentioned that you had seen a number of orthopedists with no real answers. Did these physicians specialize in diseases of the spine? Another option is an evaluation by a neurosurgeon with a similar area of expertise. I would suggest that you contact the medical staff office of your local hospital or call the Departments of Orthopedic Surgery or Neurosurgery at the Medical College of Wisconsin asking for an appointment with their physicians who specialize in diseases of the spine.

    Spine Impairment and Spinal Disability A Disability Rating addresses the issue of the patients inability to complete any substantial gainful activity successfully that the patient was previously able to complete before their injury and subsequent impairment. For example, a fireman whose main job task is lifting has had an injury to his low back and has a Spinal Impairment Rating of . His Spinal Disability Rating is for the job task of lifting. The Disability Rating is also used with the Impairment Rating to help third parties understand the patients disability in the form of a numerical percentage(15%, 40%, 85% etc.). An Impairment Rating helps third parties such as attorneys, judges, courts and insurance companies understand the extent of a patients injuries. The Impairment Rating is a medical assessment of a patients physical or anatomical deficit or loss use of function, represented by a percentage value for each deficit or functional loss, expressed in terms of the whole person. The Impairment Rating also assists to translate a medical injury or condition into a numerical percentage(25%, 60%, 75% etc.) so that non-medical personnel can understand how much of the patients body is impaired.

In scoliosis, the spine curves to the side when viewed from the front, and each vertebra also twists on the next one in a corkscrew fashion. Our spine is an engineering marvel that supports our weight and ties our body together.
