What is scoliosis?
Scoliosis is an abnormal curve of the spine (backbone). With scoliosis, the spine isn't straight. Instead, the spine is crooked and curves to the side. If the spine is very crooked, the ribs or hips may stick out more on one side than the other side. Also, one shoulder may be lower than the other. Scoliosis may begin in childhood but often is not noticed until the teenage years. In most cases, the exact cause of scoliosis isn't known. It seems to run in some families. How can I tell if my child has scoliosis?
Look at your child's spine to see if it curves or if it is straight. Also check to see if one shoulder is lower than the other. Many public schools check for scoliosis in the 5 th or 6 th grade. Your doctor may also examine your child for scoliosis at a regular check-up. Your doctor may be able to tell if your child has scoliosis just by looking at your child's back when it is in different positions. Your doctor may have your child stand and bend over to touch the toes. Sometimes x-rays help show the curve in the spine.

What is Adult Scoliosis?
The normal, healthy spine is straight when seen from the front or the back (frontal plane). When seen from the side, the healthy spine has several mild curvatures. By obtaining an X-ray of a standing person, the exact contour of the spine can be measured and then compared to normal values to determine if a significant spinal deformity is present. Scoliosis is defined as a lateral deviation of the spine. More precisely, if the spine, seen on an antero-posterior radiograph (abbreviated as "AP", ie. seen from the front), has a curvature that measures more than 10 degrees then a scoliosis is present. (figure 1)
When a curvature, or scoliosis, is mild then one may not be able to detect any abnormality without an X-ray. On the other hand, moderate or severe scoliosis can be noted due to an asymmetry of the back. A tilted waistline and skin folds may be present, as well as a hunching, or protrusion of the back. Frequently, such an asymmetry in the back is more noticeable with leaning forward.
There are many types of scoliosis, and people of all ages can be affected by scoliosis. In adults, scoliosis can result from several conditions, but most types fall into two different categories. In the first category is a person who had a scoliosis as a child/adolescent and the abnormal spine curve has increased into adult life or is becoming painful with aging. The second category of adult scoliosis is seen in patients who have never had a scoliosis as a child but begin to develop an abnormal curvature with aging. The spinal deformity in degenerative scoliosis is usually a mild side curvature involving predominantly the lower levels of the spine.

However there was not much information available, only a multitude of horrendous accounts that did more harm than good and just sent her into a frenzy of worrying. So really my purpose in creating this area is to reassure people of all ages that Scoliosis is not such a horrible thing, just something one comes to terms and learns to live with. I am sure most people go through the initial stage of 'why me?' - I know I certainly did, however what I didn't know and find out until later was that Scoliosis is quite an ordinary problem and that I wasn't one of the only people to be affected by it, just one of many in the world.

VOL 101 / NO 6 / JUNE 1997 / POSTGRADUATE MEDICINE You can't prevent it. All you can do is watch for it. It often runs in families. Girls are four times more likely than boys to have it, and it is much more likely to progress and require treatment in girls. What is this mystery problem?
Scoliosis is curvature of the spine to the side, with twisting (something like a corkscrew). The ribs on the side toward the curve become pushed together, and on the other side they're spread apart. Mild scoliosis is barely noticeable. But when it becomes severe, it can cause back pain, fatigue, nerve problems, disfigurement, and heart and lung problems if these organs are crowded into too little space. How is scoliosis diagnosed?
Scoliosis usually appears in late childhood or the early teens. It can progress rapidly during the growth spurt, so about half of US states require screening beginning in fifth or sixth grade. Curvature of the vertebrae may be visible. One shoulder blade is often more prominent than the other. One shoulder or hip may be higher. The body may tilt to one side. When the person leans forward, the ribs may form a hump. How is scoliosis treated?

What Is It?
Scoliosis is a medical term taken from a Greek word meaning curvature. This disease is known to develop in adults over the age of 18 causing the spine curve laterally (to the side) to the left or right. Adult scoliosis is caused by: 1. Progression of the disease from childhood. This usually occurs when scoliosis is not treated early or went unnoticed during childhood. The scoliotic curves may be thoracic, lumbar, or both. 2. The asymmetric degeneration of spinal elements. This may be caused by osteoporosis (porous bone), disc degeneration, compression fracture, or a combination. These conditions usually affect the lumbar spine and can affect vertebral height, shape, or basic structural integrity. 3. Combination of numbers 1 and 2. The spine's normal curves occur at the cervical, thoracic, and lumbar regions. These natural curves position the head over the pelvis and work as shock absorbers to distribute mechanical stress during movement. The normal spine viewed posteriorly (from behind) appears straight from neck to buttocks. However, a scoliotic spine bends to the left or right resembling the letter S or C.

  • HISTORY: J.G. is a ten year old female who presented to the Institute with a spinal curvature noted in a routine camp physical. She had no neurologic complaints. Her birth, medical, and developmental history were all unremarkable. Her father has a vague history of spinal curvature. PHYSICAL EXAM: Physical examination was significant for a moderate left thoracic rib prominence as well as absent abdominal reflexes in the upper and lower quadrants on the left side. XRAYS: A 24 degree left thoracic curve from T5 - Tl1 was noted on scoliosis spine films.With the asymmetric abdominal reflexes and high thoracic left curve she was sent for an MRI scan. This revealed a large syringomyelia with dilatation in the lower cervical and upper thoracic area. Also noted was an Arnold-Chiari malformation. TREATMENT: She was referred to a neurosurgeon who performed a posterior fossa decompression. She tolerated this well.

Scoliosis is a condition in which the spine takes on an abnormal curve (see figure). It can be associated with other genetic or neurological diseases, but most commonly is "idiopathic." This means that the cause is unknown. Scoliosis usually develops in early adolescence and generally affects about 1 to 2 out of every 100 people. Only a small fraction of these patients go on to need surgery. Although girls and boys initially develop scoliosis with the same frequency, girls are more likely to worsen and go on to require treatment. The condition tends to run in families. What symptoms and signs are commonly seen in scoliosis?
The signs of scoliosis depend on what part of the spine is affected. For thoracic curves (i.e. those in the chest) there is often a "hump" present where the ribs have rotated. In this case, the scapula (or shoulder blade) on the same side of the body side sticks out. In lumbar scoliosis (of the lower back) one side of the pelvis sticks out, giving the appearance of a "high hip." In general, there are no other symptoms of scoliosis until a patient reaches adulthood. At this time, back pain may develop. How can my doctor tell if I have scoliosis?
