This is a discussion of congenital scoliosis, NOT idiopathic scoliosis. I was born with hemi-vertebra and vertebra which failed to segment on one side. This problem occurred in the developing fetus. That's what congenital scoliosis is. It was nothing my mother did to cause it. It was nothing I did to cause it. It just happened. The picture below is of two t-spine x-rays taken three years apart. I held the x-rays up to a window and then snapped a picture. On the left is an x-ray of 1997 when I was age 51. On the right is an x-ray of 1994 when I was age 48. The major curve is about 65-degrees in 1994 and about 85-degrees in 1997. The other compensating curves are not pictured. This is the way I have appeared for my entire life. There were no treatments for congenital scoliosis when I was a child. My scoliosis was first detected by a casual observer when I was 5 years old. It was already as bad as it appears in these x-rays. The congenital malformations are most obvious at the T4-T8 levels. The vertebra in this area are not distinctly visible on the x-ray. Notice the ribs just to the right of the spine. It appears as if these were joined as one rib which eventually split into individual ribs further out from the right side of the spine. Without these abnormalities, there is no scoliosis at all. Congenital scoliosis is about five percent of all scoliosis cases. Congenital scoliosis is really bad developmental luck. No one is to blame. I can't stress this enough. I have read e-mail from parents who thought they were to blame for their child's scoliosis. No one is to blame for this bad developmental luck. Could have been much worse luck with more to overcome. Could have been blind, could have been deaf, could have been any number of worse things in addition to scoliosis.

At the present time this technique has been used on over seventy children/adolescents and three adults. So that we could gather our results the last case done was approximately October 2000.
Dr. Sanders in San Antonio, TX and Dr. Rieger in New Jersey continue to do this technique. The disparity in numbers of children compared to adults is due to the nature of our practice and that our hospital is a childrens hospital. If the opportunity arose, we have a number of local "adult type" hospitals available. Adults have the same anatomy. It's just that our practice is mainly with children. We do not feel this technique is useful at the present for anyone over 50 nor do we think it is useful for anyone who has a low curve ( in the lumbar area.) We are able to do the procedure below the diaphragm now to reach to L2 or L3 . In Janury 99 we did our first and second cases of "thoracolumbar scoliosis". They were quite successful
..so far, so good. So we now know we can get below the diaphragm with minimally invasive surgery but our limit is to the second lumbar vertebra....maybe the third.

There are many causes of a structural scoliosis, but by far the most common (80 to 90 per cent) is the unknown, or idiopathic. This idiopathic scoliosis develops usually as the spine is growing rapidly. The earliest form, or infantile form, occurs in the first three years of life, and it usually resolves with time. The juvenile form occurs up to the age of nine, and has a high familial relationship. It can often be a progressive scoliosis. The adolescent form is the most common, and occurs from nine to fourteen years, and the most severe cases involve females. It is not known for sure why some children get a scoliosis. It appears that genetic inheritance is a major contributor to a scoliosis.
When should screening for scoliosis occur?
The most important part of the management of scoliosis is the early detection of its presence, as early management may prevent the need for surgery. The Spine Society of Australia recommends that screening for scoliosis should be done on Year 7 and Year 9 schoolgirls.
Management of Scoliosis
The management of a scoliosis is determined by the extent of the scoliosis. A number of methods are used to decide upon the most appropriate treatment. In most instances a mild scoliosis requires no specific treatment. Advice in regard to posture and exercises may be offered. If the scoliosis is more severe it must be treated.

    Overview
    Scoliosis is defined as a lateral or rotary curvature of the spine. Scoliosis is common in most connective tissue disorders including Marfan syndrome, homocystinuria, Ehlers-Danlos, osteogenesis imperfecta, the MASS phenotype and mitral valve prolapse syndrome. Scoliosis is a feature of rickets, a condition in which bones become soft and decalcified. Scoliosis with no known cause is called "idiopathic" scoliosis. The term "idiopathic" scoliosis may becoming dated, as recent studies show a clear link between scoliosis and lowered bone densities.

    There are many causes for scoliosis. About 80%-85% of the patients have a type called IDIOPATHIC scoliosis. This means "no known cause." Idiopathic scoliosis often runs in families and appears to be due to genetic or hereditary factors. It is not known what "triggers" the development of the curve, or why some curves progress more than others. Scoliosis may occur in children who are otherwise perfectly healthy.
    Also, scoliosis may be caused by NERVE AND MUSCLE DISORDERS , such as cerebral palsy, muscular dystrophy and polio. Other known causes of scoliosis include: CONGENITAL which is caused by abnormalities in the formation of the vertebrae present at birth, CONNECTIVE TISSUE DISORDERS such as Marfan's Syndrome, and CHROMOSOMAL ABNORMALITIES , such as Down's Syndrome. Early diagnosis of the cause of scoliosis can aid in the proper treatment.

Causes, incidence, and risk factors Return to top There are three general causes of scoliosis: congenital, usually related to a problem with the formation of vertebrae or fused ribs during prenatal development; neuromuscular (poor muscle control or muscular weakness or paralysis due to diseases like cerebral palsy muscular dystrophy , spina bifida and polio ); and idiopathic (of unknown cause), which appears in a previously straight spine.
    The idiopathic form in adolescents is the most common and may have a genetic predisposition. Most cases occur in girls and curves generally worsen during growth spurts. There are also infantile and juvenile forms that are less common and affect a similar number of boys and girls. Scoliosis may be suspected when one shoulder appears to be higher than the other, or the pelvis appears to be tilted. It is often unnoticeable to an untrained observer, however.
