Epidural Injections: No Significant Functional Benefit for Sciatica
Some Short-Term Pain Relief, but Does Not Reduce Need for Surgery
Results of a randomized, double-blind study, "Epidural Corticosteroid Injections for Sciatica Due to Herniated Nucleus Pulposus," were recently published in the New England Journal of Medicine. The study's working premise was that while epidural corticosteroid injections are commonly used for sciatica, their efficacy has not been established. The trail tested the effects of epidural injections of a corticosteroid (methylprednisolone acetate), and isotonic saline (placebo group.). The authors compared the epidural steroid and placebo groups at three weeks, six weeks and three months: Three weeks after treatment, the two groups showed no statistical difference in their visual-analogue scores for leg pain; McGill Pain Questionnaire; the Sickness Impact Profile scores; the number of days of limited activity; and the physical examination measures. The only significant difference between the two groups was that the methylprednisolone group "had greater improvement in the finger-to-floor distance (P=0.006), and a smaller proportion of patients in this group had sensory deficits (P=0.03)." At six weeks, the results were similar. The only difference "was in the degree of improvement in leg pain, as assessed by the visual-analogue pain scale, which was greater in the methylprednisolone group (difference in mean change, -11.0; 95 percent confidence interval, -21.1 to - 0.9; P=0.03)."

  • Sciatica refers to pain in the passage and distribution region of the sciatica nerve.
  • It's main manifestation is radiating pain in the lower back and legs. It usually radiates from the lumbar region on just one side from the lumbar region or buttock to the posterior side of the thigh, popliteal fossa, lateral side of the leg and lateral side of the dorsum of the foot.
  • Most sciatica is due to prolapse of a lumbar intervertebral disc.
    Differential Diagnosis:
    Wind-cold-damp.
    As a Bi syndrome, it is fixed, severe and cold pain in the lumbar region and legs. Movement is limited when bending forward and backward as well as turning. It is aggravated on cloudy, rainy and cold days. Tongue: White, greasy coat
    Pulse: Wiry and floating Kidney Jing Defeciency.
    Lingering soreness and weakness of the loins and knees, lassitude and listlessness. Tongue: Pale with white coating.
    Pulse: Deep, thin, weak pulse. Blood stasis obstructing the collaterals.
    Severe fixed prickling pain in the lumbar region and leg, aversion to pressure. Tongue: Dark purplish, sometimes with purple spots.

    Putting it into context
    For patients with chronic disease, and in this case chronic painful disease, interventions may be attractive even if their success rate is far lower than would be acceptable in, say, the management of postoperative pain. The art of clinical practice will then come into play, as patient and doctor juggle the risk and benefit of the alternatives, albeit with better data than we have at present.
    For long-term benefit, the only study that was itself significantly different from control was a comparison with saline. There was no significant long-term benefit for local anaesthetic plus steroid over local anaesthetic.
