Is Sciatica a Disorder or a Symptom?
The term "sciatica" is commonly used to describe pain traveling in the distribution of the sciatic nerve. Sciatica is a symptom caused by a disorder occurring in the lumbar spine. The sciatic nerve is the largest nerve in the human body, about the diameter of a finger.
Sciatica Fig 1 Sciatic nerve fibers begin at the 4th and 5th lumbar vertebra (L4, L5) and the first few segments of the sacrum. The nerve passes through the sciatic foramen just below the Piriformis muscle (rotates the thigh laterally), to the back of the extension of the hip and to the lower part of the Gluteus Maximus (muscle in the buttock, thigh extension). The sciatic nerve then runs vertically downward into the back of the thigh, behind the knee branching into the hamstring muscles (calf) and further downward to the feet.

Red Structures = Arteries Blue Structures = Veins Sciatic Symptoms Usually sciatica affects one side of the body. The pain may be dull, sharp, burning, or accompanied by intermittent shocks of shooting pain beginning in the buttock traveling downward into the back or side of the thigh and/or leg. Sciatica then extends below the knee and may be felt in the feet. Sometimes symptoms include tingling and numbness. Sitting and trying to stand up may be painful and difficult. Coughing and sneezing can intensify the pain.

Back pain is very common and a major cause of disability and days off work. It is usually non-inflammatory ("mechanical") in origin. It is often due simply to bad posture, incorrect lifting of heavy loads or sometimes previous injury. Often this type of back pain can be helped by exercises to strengthen back muscles, improvement of bad posture and lifting technique, and sometimes a lumbar support corset.   • Slipped Disk
The bones which make up the spine are separated by soft discs which help the bones to move on one another when you bend your back. Somtimes the centre of a disc pops out of position, referred to as a slipped disc. This causes pain and limited movement, and if the disc presses on a nerve it may cause weakness of a muscle (usually in one leg) and numbness or a tingling sensation.

Sciatica describes a pain which spreads down one leg from the buttock on one side. It originates in the lower back where the sciatic nerve is pinched as it leaves the spine, and this causes pain to be felt all the way down the leg.

  • A syndrome characterised by pain radiating from the back into the buttock and into the lower extremity along its posterior or lateral aspect and most commonly caused by prolapse of the intervertebral disk, the term is also used to refer to pain anywhere along the course of the sciatic nerve.
  • Pain radiating (travelling) into the buttock, back of the thigh, and often into the calf and foot. This is usually caused by irritation of a nerve root of the sciatic nerve, often from compression by a disc or degenerative disease.

Overview The longest nerve in your body, the sciatic nerve runs from your pelvis through your hip area and buttocks and down each leg. It divides into the tibial and peroneal nerves at the level of your knees. The sciatic nerve controls many of the muscles in your lower legs and provides feeling to your thighs, legs and feet. The term sciatica Sciatic pain usually goes away on its own in six weeks or so. In the meantime, hot and cold packs, over-the-counter pain relievers, and exercise or physical therapy can help ease discomfort and speed recovery. Surgery to relieve pressure on the nerve may be an option when symptoms don't respond to conservative treatment and pain is chronic or disabling.

The management of each of these types of pain is different and should be understood to be such. Radicular (root) pain caused by herniated or ruptured disc. Pain is localized in the back but travels down the legs to the foot. Principles of Management The goals of pain management are basic: 1) pain relief; 2) physical rehabilitation; and, 3) control of psychological and emotional problems. Pain Relief Pain relief can be achieved by: 1) medications; 2) nerve blocks; and, 3) neuroaugmentation.

Introduction; my own experience with an Oriental sciatica treatment
My very first experience with traditional Chinese medicine came when I had some sciatica pain during a kung fu workout. I asked my kung fu teacher about it. He showed me some acupoints on my leg to press at home that night and the next day. They weren't too hard to get to, but it was nice for him to press them for me this first time. To my utter amazement, the pain was 95% gone the very next day. The sciatica pain that began in my lower back and radiated down to my little toe was gone after one session of acupressure. In my experience, the pain would peak for about three days, and then take about two weeks to completely fade away. But in this case it was completely gone in two days. Not all sciatica patients respond to just acupressure. Cases such as mine did because, even though it wasn't a pleasant situation for me, it was just a minor ache.
