This common condition, called sciatica, is named after the sciatic nerve. Most people with sciatica improve without treatment within days or weeks. However, in 10% to 25% of people, the pain is so severe that it interferes with daily activities and requires treatment. What is sciatica?
It is an irritation or inflammation of the sciatic nerve, which is the longest nerve in the body, beginning at the spinal cord in the lower back and extending through the buttock and down the legs to the heels. Usually, the pain is felt on only one side of the body. What causes sciatica?
It can be caused by sitting for long periods of time, injury, infection, diabetes or, rarely, tumors. It can also result from prolonged pressure on the nerves in your lower back from arthritis or a slipped disk. Disks normally act as shock absorbers for your spine. When a disk slips out of place or ruptures, it can trap the sciatic nerve between the disk and bone. What are the symptoms?

Sciatica / Slipped disc / Prolapsed disc What is sciatica? Sciatica is pain that starts in the back and radiates down one of the legs. It is quite a common complaint. It is caused by pressure on the sciatic nerve which runs down into the leg. The cause of pressure can be varied from a Slipped (Prolapsed) disk to muscle tension ( Piriformis syndrome ) or something less common such as a tumours, bony growths and infections. Depending on where the sciatic nerve is pinched will determine where you feel the pain which can radiate to the front of the knee or right down the back of the leg to the foot. On the illustration opposite, L4 and L5 are the bottom two Lumbar vertibrae at the base of the spine. S1 is the top of the Sacrum. What can you do about it?
The shaded regions indicate where pain may be felt depending on which part of the spine is involved.
  • Rest (in bed if necessary, you may find that the psoas position is most comfortable).

What is sciatica? Sciatica is pain resulting from irritation of the sciatic nerve. Sciatica pain is typically felt from the low back to behind the thigh and radiating down below the knee. The sciatic nerve is the largest nerve in the body and begins from nerve roots in the lumbar spinal cord in the low back and extends through the buttock area to send nerve endings down the lower limb. What are causes of sciatica? While sciatica is most commonly a result of a disc herniation directly pressing on the nerve, any cause of irritation or inflammation of this nerve can reproduce the symptoms of sciatica. These cause include irritation of the nerve from adjacent bone, tumors, muscle, internal bleeding, infections, injury, and other causes. How is sciatica treated? Bed rest has been traditionally advocated for the treatment of sciatica. But how useful is it?

The vertebrae are the bony building blocks of the spine. Between each of the largest part of the vertebrae are the discs. Ligaments are situated around the spine and discs. The spine has seven vertebrae in the neck (cervical vertebrae), twelve vertebrae in the mid-back (thoracic vertebrae), and five vertebrae in the low back (lumbar vertebrae). What is the purpose of the spine and its discs?
The bony spine is designed so that vertebrae "stacked" together can provide a movable support structure while also protecting the spinal cord (nervous tissue that extends down the spinal column from the brain) from injury. Each vertebrae has a spinous process, which is a bony prominence behind the spinal cord that shields the cord's nerve tissue. The vertebrae also have a strong bony "body" in front of the spinal cord to provide a platform suitable for weight-bearing. The discs are pads that serve as "cushions" between each vertebral body which minimize the impact of movement on the spinal column. Each disc is designed like a jelly donut with a central softer component (nucleus pulposus). This softer component can rupture (herniate) through the surrounding outer ring (annulus fibrosus) and irritate adjacent nervous tissue. Ligaments are strong fibrous soft tissues that firmly attach bones to bones. Ligaments attach each of the vertebrae and surround each of the discs. When ligaments are injured as the disc degenerates, pain in the area affected can result.

    Sciatica The sciatic nerve may become irritated, and sciatic pain and symptoms can occur, whenever unusual pressure is applied to the sciatic nerve at some point along it's length. This happens most commonly when the shock absorbing discs between the vertebrae in the spinal column bulge out into the space where the sciatic nerve starts. In some cases, sciatic pain comes infrequently and lasts only a few days (caused by a temporary bulging of the disc because of muscle spasms or sitting in an awkward position). In other cases, when the disc has been permanently damaged, the sciatic pain will become chronic (lasting form longer periods of time and return over and over again) unless surgery is done to repair the bulging disc. Some men are bothered by sciatic pain that is caused by sitting on their wallet if they carry it in their back pocket. While it is much less likely, pressure on the sciatic nerve can also be caused by a blood clot, a tumor or an abscess. When sciatic pain is caused by pressure on the sciatic nerve in the lower spine, it is often made worse by bending, straining, coughing or sneezing. The lower part of the back may also feel stiff, lose its inward curve and the muscles along each side of the spine may go into painful spasm. In most cases you can prevent sciatic pain or reduce existing sciatic pain by taking steps to care for your back:
    • brace yourself if you feel an unusually forceful cough or sneeze coming on
    • practice safe lifting techniques
    • ask for help and use the proper equipment when it comes to lifting heavy or awkward loads

    Sciatica Sciatica refers to pain that begins in the hip and buttocks and continues all the way down the leg. This condition is often accompanied by low back pain, which can be more or less severe than the leg pain. The term "sciatica" indicates that the sciatic nerve, which travels from the lower back through the buttocks and into the leg, is thought to be the cause of the pain in this condition. True sciatica is a condition that occurs when a herniated lumbar disc compresses one of the contributing roots of the sciatic nerve. This type of low back pain is less common than other causes and conditions that produce back pain . For instance, sporting activities, recreational activities, and heavy labor can cause back and leg pain, which is commonly misdiagnosed as sciatica. The challenge for a physician is to distinguish between radicular pain , which is caused by an inflamed nerve root, and referred pain, which is a result of a musculoskeletal sprain or strain. Symptoms
    The most common symptom of true sciatica is posterior thigh, lower leg or foot pain that can be much worse than the accompanying lower back pain. Usually a patient will experience moderate to severe pain, which begins in the buttocks and runs down through the leg or foot. It is important to know that true sciatica will produce pain that radiates beyond the knee. Often a patient will have a previous history of lower back pain beginning a few days or weeks before the leg pain occurs, then the leg pain becomes worse than the back pain, and in some cases the back pain will completely disappear.
