Sciatica exercises
Sciatica is a commonly used term for pain originating from the back that is felt along one of several nerve distributions in the buttock, hip, thigh, leg, ankle or foot. Sciatica involves compression or irritation of one or more nerves exiting the lower spine that make up the sciatic nerve. Sciatica
Sciatica is the term given to pain down the leg, which is caused by irritation of the main nerve into the leg, the sciatic nerve . This pain tends to be caused where the nerves pass through and emerge from the lower bones of the spine (lumbar vertebrae). Much of the advice regarding this condition is the same as that for low back pain
Symptoms
In sciatica, there is a pain down into the leg, which travels below the knee, and may involve the foot. There may be numbness and there may be weakness of the lower leg muscles. These symptoms may come on their own, but are often combined with low back pain . The order in which the symptoms appear may vary. Sometimes the back pain comes before the sciatica, and sometimes will follow.
Red Flags
As with low back pain, there is a group of features (the Red Flags ) which doctors use to highlight the need to act more quickly, because they may suggest a problem which is not as straightforward as simple back pain or sciatica. If you fit into one of these groups or are concerned, you should see your doctor soon:
  • You are younger than 20 or older than 55 when you get the problem for the first time.

They start in the lower spine, pass behind the hip joint, and go down the buttock and back of the leg to the foot. Pain from this nerve, called sciatica, may be felt from the hip to the big toe. What causes sciatica?
Sometimes the nerve is pinched between vertebrae, or its passageways are narrowed by arthritis or swelling of a sprained ligament in the area, and it becomes irritated and tender. A disk (the cushion between the vertebrae) may bulge out of place and press on the nerve. Rarely, an abscess, blood clot, or growth may push on the nerve. At times, no exact cause is found, but a movement that is normally harmless (such as bending over) suddenly brings on violent pain. Is sciatica different from back pain?
Yes, although low back pain often comes with it. Usually, sciatica affects only one side. Pain often radiates down the buttock or leg. It can range from a tingling, burning, pins-and-needles feeling to excruciating, shooting pain that makes standing up nearly impossible. Coughing, sneezing, and sitting make the pain worse. Your leg or foot may be numb or weak and have odd sensations in it. You may be unable to move your foot or bend your knee. What can I do for sciatica?

B ACK SURGERY BETTER FOR SEVERE SCIATICA AND SPINAL STENOSIS Surgical treatment for patients with certain types of lower back pain provides better relief than nonsurgical treatment, according to studies by researchers from the MGH, the Maine Medical Assessment Foundation and the University of Washington at Seattle. The studies, reported in the August Spine , looked at patients treated for two common causes of low back and leg pain sciatica, severe lower back pain that radiates into a leg, and spinal stenosis, a narrowing of the canal carrying the spinal cord that causes back and leg pain in the elderly. In both groups, surgically treated patients reported greater levels of improvement after one year than those treated by such nonsurgical means as rest, medication, exercise, physical therapy and spinal manipulation. Bed rest for sciatica is just a waste of time
February 23, 1999 Add sciatica to the sorts of back pain that shouldn't keep you in bed. A week or two in bed used to be routine treatment for any sort of aching back. But over the past 15 years, doctors have learned that many types of backaches get better just as fast if people go about their normal activities. Until now, that hadn't been tested with pain from the sciatic nerve pain that's usually limited to the buttocks and thigh but sometimes goes all the way down to the foot. Dr. Patrick C.A.J. Vroomen of Maastricht University Hospital in the Netherlands, where two weeks of bed rest is a common treatment for sciatica, reported his findings in last week's New England Journal of Medicine. Doctors randomly assigned 183 patients either to bed rest or "watchful waiting" getting up and about whenever possible without provoking pain. After two weeks, 70 percent of the 92 patients who got out of bed only to bathe or use the toilet felt better. So did 65 percent of those who were up and about. Within statistical error, those are the same results.

But now, a study published in the New England Journal of Medicine finds saying in bed for weeks may be a waste of time. Treatment with anti-inflammatory drugs coupled with a decrease in activity may be just as effective, researchers say. The sciatic nerve is actually a cluster of nerves that run from the lower back in the buttock area down the back of the leg.
