WHAT IS SCHIZOAFFECTIVE DISORDER?
Some psychiatric disorders are very difficult to diagnose accurately. One of the most confusing conditions is schizoaffective disorder. This relatively rare disorder is defined as "the presence of psychotic symptoms in the absence of mood changes for at least two weeks in a patient who has a mood disorder." The diagnosis is used when an individual does not fit diagnostic standards for either schizophrenia or "affective" (mood) disorders such as depression and bipolar disorder (manic depression). Some people may have symptoms of both a depressive disorder and schizophrenia at the same time, or they may have symptoms of schizophrenia without mood symptoms. Many individuals with schizoaffective disorder are originally diagnosed with manic depression. If the person experiences delusions or hallucinations that go away in less than two weeks when the mood is "normal," bipolar disorder may be the proper diagnosis. Someone who experiences psychosis for three or four weeks while in a manic phase does not have schizoaffective disorder. However, if delusions or hallucinations continue after the mood has stabilized and are accompanied by other symptoms of schizophrenia such as catatonia, paranoia, bizarre behavior, or thought disorders, a diagnosis of schizoaffective disorder may be appropriate. Accurate diagnosis is easier once the acute psychotic episode is under control.

Usually the diagnosis of schizoaffective disorder is given when the schizophrenic symptoms are more pronounced; if the mood disorder is more pronounced the condition is more often called depressive or manic psychosis. There are many different definitions of these conditions, and it can be difficult for the doctors to settle on a diagnosis. About one in every two hundred people (1/2%) develops schizoaffective disorder at some time during his or her life. Some people believe that this low level of diagnosis does not represent the true level of the condition, and that many people are given a mis-diagnosis instead of a diagnosis of schizoaffective disorder. More recently, in another study, a quarter of all psychotic patients were eventually diagnosed with schizoaffective disorder. Schizoaffective disorder usually begins in late adolescence or early adulthood, often between the ages of sixteen and thirty. More women than men tend to suffer from schizoaffective disorder.

Schizoaffective disorder is often accompanied by unemployment, poverty and homelessness and as such drug therapy alone usually is insufficient. Drug therapy usually can stop the patient's psychosis, but often only social and occupational rehabilitation therapies can overcome the associated unemployment , poverty and homelessness.

Thus the medical management of schizoaffective disorder often requires a combination of antipsychotic, antidepressant and antianxiety medication. Unfortunately, after the first year of treatment, only a minority of schizoaffective outpatients remain on their oral medications. Thus, long-acting, depot antipsychotic medications are usually required to overcome this non-compliance. However, the newer (serotonergic) antidepressants have dramatically benefitted many apathetic or depressed schizoaffective patients. This disorder is characterized by the presence of one of the following as well as the presence of at least two of the following symptoms, for at least one month:
  • delusions
  • hallucinations
  • disorganized speech (e.g., frequent derailment or incoherence)
  • grossly disorganized or catatonic behavior
  • negative symptoms (e.g., affective flattening, alogia, avolition)
(Only one symptom is required if delusions are bizarre or hallucinations consist of a voice keeping up a running commentary on the person's behavior or thoughts, or two or more voices conversing with each other.) The occurrence of the delusions or hallucinations must be in the absence of any serious mood symptoms for at least 2 weeks. The mood disorder, however, must be present for a significant minority of the time. The symptoms of this disorder also can not be better explained by the use or abuse of a substance (alcohol, drugs, medications) or a general medical condition (stroke).

Disease - Schizoaffective disorder
Schizoaffective disorder Definition: A disorder that includes features of both schizophrenia (hallucinations, delusions, and deteriorating function) and a mood disorder (either bipolar disorder "manic depression" or major depressive disorder) in which the symptoms are so intertwined that a distinction between the two problems cannot be made. Causes And Risk: People with this condition often seek treatment for mood problems (mania, depression, symptoms of both simultaneously or rapidly alternating) which shifts towards symptoms of schizophrenia or may display symptoms of both conditions simultaneously, although the schizophrenic symptoms dominate. To be diagnosed with this disorder, someone must experience psychotic symptoms (hallucinations and delusions) for at least 2 weeks in the absence of mood disorder. The exact cause of the condition is unknown, but factors that affect the development of both schizophrenia and affective disorders may play a role, including a strong genetic component and other biochemical factors. Risk factors include a family history of schizophrenia or affective disorder.
