Brachycephaly Severity Assessment Assessing the severity of plagiocephaly can be very subjective. Severity is dependant upon many factors including flattening of the back of the head, prominence of the forehead, ear shift, facial asymmetry and torticollis. In an attempt to standardize the classification of plagiocephaly, Cranial Technologies has developed the assessment tools provided below. These tools are currently used across the country to document the improvement achieved with repositioning. A child's severity may be documented at a visit to the pediatrician and then re-evaluated using the same form after 4 - 6 weeks of repositioning. A multi-center study is currently under way to determine the validity and reliability of these forms. Results of this study will be shared when they become available.

You may have been a little startled to notice your baby's head developing an unusual shape. The condition is not uncommon, it's called plagiocephaly (pla'je-o-sef'ah-le), and often results from external forces applied to the soft infant skull. It might sound complicated, but plagiocephaly literally means "oblique head" (from Greek: plagio = oblique, and cephale = head) It's called a parallelogram deformity because, if you look down on the infant's head (bird's-eye-view), this is the shape you will see. From this angle it looks as if half of the head has been pushed forward, often accompanied by misalignment of the ears, facial asymmetry and bulging forehead.
Brachycephaly
Something as simple as sleeping on the back can cause brachycephaly . The head flattens uniformally, causing a wider and shorter shape. Increased head height is also common in children with brachycephaly.

Primary Craniosynostosis is a rare disorder of the skull that may be inherited as an autosomal dominant or autosomal recessive genetic trait. Premature closure of the bones (sutures) in the skull result in an abnormally shaped head. The severity of symptoms and shape of the skull depend on which skull bones are prematurely closed. This disorder is present at birth.

    SCAPHOCEPHALY applies to premature fusion of the sagittal suture. The sagittal suture joins together the two parietal bones of skull. Scaphocephaly is the most common of the craniostenoses and is characterized by a long, narrow head. It is one of a group of less common cephalic disorders that are called the CRANIOSTENOSES. Craniostenoses are deformities of the skull caused by the premature fusion or joining together of the cranial sutures. Cranial sutures are fibrous joints that join the bones of the skull together. The nature of these deformities depends on which sutures are affected.

    Outcome Analysis of Dynamic Spring Mediated Cranioplasty for Scaphocephaly
    Lisa David, MD , Stephen Glazier, MD, and Louis Argenta, MD. Introduction: Spring mediated cranioplasty, first introduced by Lauritzen in 1999, has demonstrated promising outcome results for the treatment of craniosynostosis. The purpose of this study is to assess the clinical outcome of this treatment modality, in comparison to historical controls, treated with standard cranial expansion techniques. Material and Methods: Children, between the age of 3 and 6 months, diagnosed and undergoing treatment for scaphocephaly were considered for this study. This is an IRB approved study requiring parental consent for enrollment. Eight children, 3 females and 8 males, have been enrolled in this study to date. All study patients underwent cranial surgery including the removal of a 1.5 cm strip of midline calvarium including the sagittal suture. After this limited craniectomy, two 1.2 mm thick stainless steel omega shaped expanders (springs) were placed. All patients had preoperative and postoperative skull films to measure cranial expansion. Indices calculated include cephalic index, length ratio, width ratio, and height ratio. Results: All children successfully underwent spring-mediated cranioplasty. There were no perioperative morbidities or mortalities associated with any of the surgical procedures. No secondary surgeries for infection or skin breakdown were required. The mean age at initial operation was 4.1 months. No transfusions were required for any of the study patients and the mean operative time was 45 minutes for spring placement. The mean hospital stay for the initial procedure was 36 hours. The second outpatient operative procedure to remove the springs was done at a mean age of 8 months, without any peri-operative squeal. The mean operative time for the second procedure was 25 minutes with a mean hospital stay of 21 hours. Serial follow-up skull films showed a mean maximal spring expansion of 7.1 cm. The mean cephalic index postoperatively was 75%, which compares favorably to published normal age matched historical controls treated with cranial expansion techniques.
