Rubeola (Measles)
What is rubeola?
Rubeola, also called 10-day measles, red measles, or measles, is a viral illness that results in a viral exanthem. Exanthem is another name for a rash or skin eruption. Rubeola has a distinct rash that helps aid in the diagnosis. It is spread from one child to another through direct contact with discharge from the nose and throat. Sometimes, it is spread through air-borne droplets from an infected child. This is a very contagious disease that usually consists of a rash, fever, and cough.

Rubeola Reminder Vaccinate! Vaccinate! Vaccinate! On July 19, a Juneau pediatrician notified the Section of Epidemiology of a possible case of measles (rubeola) in a 16-year-old female California resident who was visiting relatives in Juneau.
    The girl had presented to the pediatrician with a fever of 103°F., a generalized maculopapular rash, a sore throat, cough, and conjunctivitis. Physical examination revealed the presence of Koplik spots. Measles was diagnosed and was serologically confirmed on August 2. Serum obtained from the patient on 7/19/89 had an anti-rubeola IgG antibody titer of less than 1:8 by immune adherence hemagglutination (IAHA); the anti-rubeola IgG antibody titer of serum collected on 7/28/89 was 1:128, a greater than four-fold titer rise.
    The patient's history of vaccination against measles could not be confirmed. She had traveled by plane from California to Juneau 12 days prior to the onset of her rash. The patient's 15-year-old sister, who had remained in California, developed a rash on the same day as her sister and was also diagnosed on July 19 as having measles. Her history of measles vaccination also could not be verified.
Only one possible secondary case of measles was identified. This was an unimmunized 18-month old child exposed to the index case in the physician's waiting room on July 19 who received MMR vaccine on July 21. She developed fever and a facial rash during the second week after exposure. On August 4 the child had a cough, lethargy, and anorexia; examination revealed a fever of 103°F., a generalized maculopapular rash, and Koplik spots. The diagnosis of measles in this child cannot be serologically confirmed, since vaccination with measles vaccine elicits the same antibody response as measles disease.

T. Zecca, D.Graffino. M. Lania-Howarth, M. Passannante, J. Oleske NJMS, Children's Hospital of New Jersey, Newark, NJ The syndromie of autism is a clinical entity affecting 20 out of 10,000 children. We have evaluated the possible role of MMR in the Pathogenesis of Autism by comparing Rubeola titers in autistic and normal children. Among 16 children diagnosed with Autism followed in our clinical practice, we noted these children to have a 3 fold increase in their rubeola titers over expected normal range. A Wilcoxon Kruskal Wallas test comparing 13 rubeola titers from normal children reveals a statistically significant P-value of 0.0050. Subjectively, parents have stated that their children's developmental milestones deteriorated following MMR vaccination. Neurological sequelae following MMR are widely reported . MMR therefore may play a role in the pathogenesis of Autism . The elevated titers of anti-measles antibodies in Autistic children may signify a chronic activation of the immune system against this neurotropic virus.

The infection is spread by droplets from the nose, mouth or throat of an infected person. The incubation period is 10 to 14 days before symptoms generally appear. An immunity to the disease occurs after vaccination, after active infection, and passive immunity of an infant whose mother is immune lasts most of the first year of life. Before widespread immunization, measles was so common during childhood that 90% of the population had been infected by age 20. Measles cases have dropped by 99% in the U.S. and Canada after widespread immunization. Only 9,600 cases were reported in the U.S. in 1991.
