Rosacea is a facial rash that occurs in middle-aged men and women. Approximately 13 million people in the United States have Rosacea. The cause of Rosacea is unknown. It affects people mainly in the 30s and 40s, especially those with fair-skin, blue eyes and of Celtic origin. Famous individuals with Rosacea include WC. Fields, Rembrandt and President Bill Clinton.
Rosacea used to be called "acne rosacea" but it is quite different from acne. The red spots and pustules are dome-shaped rather than pointed and there are no blackheads, whiteheads, deep cysts, or lumps. Sometimes the affected skin is swollen and hot. Rosacea affects the cheeks, nose and forehead - rarely it involves the trunk and upper limbs. Facial creams or oils, and topical steroids may aggravate Rosacea.
Rosacea is often accompanied by other features:
· A red face due to flushing and telangiectasia (persistent red veins).
· Red, sore or gritty eyelids. (Blepharitis)
· Sun exposure and hot and spicy food, drinks containing caffeine and alcohol often aggravate symptoms.

DISEASE ENTITY Acne rosacea is a chronic acneiform disorder affecting both the skin and the eye. It is a syndrome of undetermined etiology characterized by both vascular and papulopustular components involving the face and occasionally the neck and upper trunk. Clinical findings are usually limited to the sun exposed areas of the face and chest and include mid facial erythema, telangiectasias, papules and pustules, and sebaceous gland hypertrophy. Rosacea is characterized by episodic flushing of affected areas, which may be associated with consumption of alcohol, hot drinks, or spicy foods. During inflammatory episodes, affected areas of the skin, primarily the convexities of the face, develop swelling, papules, and pustules. The skin lesions are notable for the absence of comedones, which distinguishes this disorder from acne vulgaris. Rhinophyma is a late finding. Ocular rosacea is a term used to describe the spectrum of eye findings associated with the skin involvement. Ocular involvement may include meibomian gland dysfunction and/or chronic staphylococcal lid disease, recurrent chalazia, chronic conjunctivitis, peripheral corneal neovascularization, marginal corneal infiltrates with or without ulceration, episcleritis and iritis. Occasionally, the ocular manifestations may precede skin involvement, delaying the diagnosis. KEY CLINICAL POINTS Definition

In the first stage the face gets red. This redness sometimes merges into the normal variants of ruddy or blushy complexion types. The redness often persists after cold exposure or after exposure to irritants like soap. With time, small blood vessels develop making the redness more noticeable. Many patients will develop stinging or burning sensations and the skin will often feel tight. The skin feels like a mild sunburn with smiling, frowning, or squinting. Sometimes this progresses to the point that everything the patient puts on his or her face stings, burns, and irritates. In the second stage of rosacea, the redness covers a larger area of the face. Slight swelling, pimples, and pustules develop. This is especially noticeable on the nose, mid-forehead, and chin. As the condition progresses, prominent facial pores can develop.
