NEW YORK (Reuters Health) - After one individual in a family develops symptoms of Rocky Mountain spotted fever, the diagnosis should immediately be considered in other family members with similar symptoms, according to a report in the CDC's Morbidity and Mortality Weekly Report. Three clusters of the illness, each of which began with a child who died 6 to 8 days after initial treatment, are described in the report. In each cluster the first case, involving a child between the ages of 14 months and 7 years, was seen at a clinic or emergency department with fever and other nonspecific symptoms. A patient in Oklahoma was treated in late May, while the others, in Kentucky and Arizona, were first seen in August, report C. Levy, at the Arizona Department of Health Services, and associates. In the case of the 7-year old, the rash did not appear for several days after the first symptoms began. Soon after the children became ill, close relatives were diagnosed with the same condition; all survived. None of the six patients reported a history of tick bite.

The disease occurs throughout the United States during months when ground temperatures reach 40 degrees Fahrenheit or more and ticks are active. Who gets Rocky Mountain spotted fever? Both children and adults can be affected by Rocky Mountain spotted fever. Disease incidence is directly related to exposure to tick-infested habitats or to infested pets. In spite of the disease's name, few cases have been reported from the Rocky Mountain region of the United States. How is Rocky Mountain spotted fever spread? Rocky Mountain spotted fever is spread by the bite of an infected tick (the American dog tick or the lone-star tick) or by contamination of the skin with tick blood or feces. Person-to- person transmission does not occur. What are the symptoms of Rocky Mountain spotted fever? Rocky Mountain spotted fever is characterized by a sudden onset of moderate to high fever (which can last for two or three weeks), severe headache, fatigue, deep muscle pain, chills and rash. Rocky Mountain spotted fever (RMSF) is an infectious rickettsial disease of dogs, that is characterized by severe vascular damage. Canine susceptibility to Rickettsia rickettsii was demonstrated in 1933. Recent reports emphasize that, contrary to previous literature, untreated naturally-occurring RMSF can result in death. Clinical reports suggest that RMSF is a much more common cause of disease in dogs than was previously recognized. Geographic Distribution Despite its name and original description as a disease of humans in the western United States, the majority of human cases of RMSF occur in the southeastern United States. Human cases of RMSF have been reported from nearly every state in the United States, western Canada, Mexico, and South America. Distribution of the disease is related to the distribution of the vector ticks Dermacentor variabilis, the American dog tick found in the eastern United States, and Dermacentor andersoni, the wood tick, which is the principal vector in the western United States. Canine RMSF has been recognized in most southeastern states, New York, Massachusetts, and Ohio.

Rocky Mountain spotted fever occurs mainly in the southeast and south central part of the US. It is found most often in Texas, Oklahoma, and Arkansas. It is more common in the spring and summer. The organism that causes the fever, Rickettsia rickettsii, is transmitted to humans through the bite of a tick. The species of tick involved may vary, depending on the location. What are the signs and symptoms of the infection? The time from tick bite to symptoms of Rocky Mountain spotted fever ranges from 2 days to 2 weeks. The typical symptoms include: fever headache muscle aches nausea and vomiting confusion coma a rash that begins sometime before the sixth day of illness The rash starts on the hands and feet, including the palms and soles. It then spreads to the ankles, wrists, abdomen, and trunk.

Fewer than 50 cases are reported annually in New York State. Who gets RMSF? In the eastern United States, children are infected most frequently, while in the western United States, disease incidence is highest among adult males. Disease incidence is directly related to exposure to tick-infested habitats or to infested pets. Most of the cases in New York State have occurred on Long Island. How is RMSF transmitted? RMSF is spread by the bite of an infected tick. In New York, the American dog tick (dermacentar variablis) is the most common vector. It can also be transmitted by contamination of the skin with tick blood or feces. Person-to-person spread of RMSF does not occur. What are the symptoms of RMSF? group in tick tissue RMSF, one type of spotted fever, is caused by Rickettsia rickettsii and is reported from across North America, but is most prevalent in the eastern U.S. The Rocky Mountain wood, American dog, and Pacific Coast ticks transmit the disease. The lone star ticks also transmits a spotted fever Rickettsia that causes similiar symptoms. Signs/Symptoms:
Flu-like aches and pain, headache, chills, confusion, light sensitivity, and high fever. A reddish-to-black rash (resembling measles) starts on the extremities (e.g. wrists and ankles) and may spread to the entire body. Death can occur. Doctors must take a clinical diagnosis and treat early as the main cause of death is delayed or improper treatment.

Rocky Mountain Spotted Fever, also known as spotted fever or tick fever, is a common tick-borne illness that has been overshadowed by the publicity regarding Lyme disease. Rocky Mountain Spotted Fever is primarily a western United States illness; however it is found in all states except Hawaii and Alaska. As of 2002, there have not been any reported cases in the state of Maine.     Rocky Mountain spotted fever is an illness caused by an organism called Rickettsia rickettsii , which infects ticks throughout their lifetime and is passed on to the next generation of ticks.     Rocky mountain spotted fever (RMSF) is a disease caused by a rickettsial organism. It is transmitted to humans by the bite of an infected American dog tick and frequently by other tick species. Who gets RMSF?
    In the eastern United States, children are infected most frequently, while in the western United States, adult males are the most likely to get the disease. The chance of getting RMSF is directly related to exposure to tick-infested habitats or to infested pets. How is RMSF spread?
    RMSF is spread by the bite of an infected tick (the American dog tick, the Lone-Star tick or the wood tick), or by contamination of the skin with tick blood or feces. It cannot be spread from one person to another. What are the symptoms of RMSF?
    RMSF is characterized by a sudden onset of moderate to high fever (which can last for 2 or 3 weeks), severe headache, fatigue, deep muscle pain, chills and rash. The rash begins on the legs or arms, may include the soles of the feet or palms of the hands and may spread rapidly to the trunk or rest of the body. How soon do symptoms appear?

    Rocky Mountain spotted fever is the most severe and most frequently reported rickettsial illness in the United States , and has been diagnosed throughout the Americas . Some synonyms for Rocky Mountain spotted fever in other countries include "tick typhus", "Tobia fever" (Columbia), "São Paulo fever" and "fiebre maculosa" (Brazil), and "fiebre manchada" (Mexico). The disease is caused by Rickettsia rickettsii , a species of bacteria that is spread to humans by hard ticks ( Ixodidae ). Initial signs and symptoms of the disease include sudden onset of fever, headache, and muscle pain, followed by development of rash. The disease can be difficult to diagnose in the early stages, and without prompt and appropriate treatment it can be fatal. Rocky Mountain spotted fever was first recognized in in the Snake River Valley of Idaho and was originally called " black measles " because of the characteristic rash. It was a dreaded and frequently fatal disease that affected hundreds of people in this area. By the early 1900s, the recognized geographic distribution of this disease grew to encompass parts of the United States as far north as Washington and Montana and as far south as California Arizona , and New Mexico Howard T. Ricketts was the first to establish the identity of the infectious organism that causes this disease. He and others characterized the basic
