Rickets is a vitamin deficiency disease of infancy and early childhood caused by lack of vitamin D . Rickets causes soft bones because vitamin D is not present to assist in calcium and phosphate uptake. Vitamin D is synthesised in the body on exposure to ultraviolet light and is supplemented in milk products. Some people who do not get enough sun exposure, milk products, or green vegetables may develop the disease, although this is rare today. Hereditary rickets is caused by an inherited defect which prevents the kidney from retaining phosphate. Rickets can also be caused by some other kidney and liver diseases. Rickets causes bone pain, slowed growth in children, dental problems, muscle loss and increased risk of fractures. The classic image of advanced rickets sufferers is of bow legs and a deformed chest and skull, with prominent frontal and parietal bones causing a distinctive "square headed" appearance. These deformities persist into adult life. Treatment involves increasing dietary intake of calcium, phosphate and vitamin D. Exposure to sunshine, cod liver oil , halibut-liver oil, and viosterol are all sources of vitamin D.

Pectus excavatum (PE) is defined as an abnormal formation of the rib cage where the breastbone caves in, resulting in a sunken chest appearance. Many doctors describe PE as a cosmetic defect or a congenital defect that is correctable only through surgery. But is it? What are osteomalacia and rickets?
These are disorders caused by insufficient levels of vitamin D in the body. They are really the same condition; rickets is the name used when it occurs in children whereas osteomalacia is the term used for adults.
Osteomalacia and rickets are the result of vitamin D deficiency. Where do we get vitamin D from?
There are two sources of vitamin D:
  • a person's diet
    formation in the skin through exposure to sunlight.
There are few dietary sources of vitamin D. The best ones are fatty fish such as salmon and sardines, and margarines supplemented with vitamin D. Milk contains added vitamin D in the USA but not in the UK. Most people in the UK get most of their vitamin D from exposure of the skin to sunlight. The average person has enough vitamin D stored in their body to last for two or three years.

    Q: Rickets... Is it still possible for people to suffer rickets? David Dr Rob Hicks responds Yes, this is still possible. When there's a deficiency of vitamin D the bones become weak and soft. This means they may develop an abnormal shape and are certainly at risk of breaking easily. In children vitamin D deficiency is called 'rickets', and in adults it's known as 'osteomalacia'. Bones need calcium to make them strong. However, to absorb calcium from the diet (dairy foods, fortified bread and cereals, for example) into the body vitamin D is needed too. Some of the vitamin D that we need to do this comes from our diet. Few foods actually contain vitamin D but those such as fish, eggs, and milk are quite a good source. The body also manufactures vitamin D in the skin when it is exposed to sunlight. The effects of vitamin D deficiency on the bones mean that children may not grow to their full potential and they may have knock-knees or bowed legs. Adults may suffer pain in the bones and find that they have difficulty climbing stairs or getting up from a squatting position.

    U.S. health officials say lack of sunlight and too little milk are contributing to a resurgence of rickets By Christy Oglesby CNNfyi ATLANTA, Georgia (CNN) Societal and environmental changes such as long commutes, smog and day care are contributing to the resurgence of childhood rickets a bone softening disease that health officials thought they had all but eliminated 70 years ago. Atlanta health officials have detected about 10 cases of the rare disease, said Dr. Norman Carvalho of Children's Healthcare of Atlanta, a children's hospital. But there may be more, he said, because the disease's rarity may make it unrecognizable to physicians. And if doctors do ascertain that a patient has rickets, they are not required to report it to the state health department. Carvalho was the leader in a Georgia study on malnutrition that will appear in the April issue of the journal Pediatrics. The study, conducted from 1997 to 1999, found that about one in 200,000 children in Georgia is hospitalized with rickets.

Significant past medical history included the following: She was adopted from Russia and according the Russian medical documents was born to a mother who suffered from alcohol abuse. She was born prematurely (gestational age not available) at 2000gms, 44cm, with an apgar of 5/6. Furthermore, she had previously been diagnosed with encephalopathy, anemia, fetal alcohol syndrome and rickets. - Since arriving here in the U.S., the adopted mother had her on vitamin supplements for 7 weeks. The mother states that the child began walking at the age of 27 months. She was seen by both orthopaedics and pediatrics and her physical exam was as follows: - She was well below the 5th percentile for both height and weight - There was delayed psycho-motor and speech development - Multiple rachitic deformities of the skeleton to include severe bowing of the tibias bilaterally (45 degrees), pigeon chest (pectus carinatum), thickened/widened wrists bilaterally and a trendelenberg gait on the left side.

    Vitamin D is necessary for proper bone growth, but it is not found in many foods naturally. It is synthesized in the skin with exposure to sunlight, but many babies do not get much exposure to the sun for a number of reasons, such as not spending time outdoors and wearing clothes, hats, or sunscreen. The darker an infant's skin, the more sun exposure is needed to synthesize vitamin D. Although breastfeeding is the ideal form of nutrition for infants, it does not supply the vitamin D needed for healthy bone growth. Nutritional rickets is a condition in which children's bones are too soft and do not develop properly. Thanks largely to vitamin D supplementation of infant formula and recognition of the need for sunlight, rickets had become largely a thing of the past in the United States. However, cases of nutritional rickets among babies have been on the rise recently.
