When my Grandmother was diagnosed with RA a few decades ago, the milder cases were simply put on aspirin, over 20 a day! If they didn't die of a bleeding ulcer, they were quickly disabled from the disease. The more severe cases might be put on prednisone or gold but was pretty much it. Not a lot to choose from.
As much as I hate having RA, I am glad I have it now. Never before have we had so many treatment options. In the last two years we've been bombarded with new drugs and treatments. We have so much to choose from. Most of us will live full productive lives, unlike those a generation ago.
There are a host of new drugs in development, and maybe even a cure in the works. The next generation of RAers may never even have to consider a joint replacement or take a liver damaging drug, what a wonderful world that will be! Disease Description

What is rheumatoid arthritis?
Rheumatoid arthritis is a disease that causes pain, stiffness, swelling, and sometimes joint deformity. It occurs most commonly in the fingers, wrists, elbows, shoulders, jaw, hips, knees, and toes. It usually affects the same joint on both sides of the body (for example, both hips). Rheumatoid arthritis often appears first in early adulthood or middle age. However, sometimes it does not occur until the later years. There may be one single attack, but more often the condition comes and goes in repeated episodes. The disease cannot be cured, but medicine can reduce the frequency and severity of attacks.

HAND SURGERY FOR RHEUMATOID ARTHRITIS
Rheumatoid arthritis is the commonest disorder of connective tissues. It has an incidence of 1-3% of the population. It is three times more common in women. The principal target tissue is synovium which lines both joints and tendons. 90% of affected patients have involvement of the upper limb. Early disease usually presents as polyarthritis (arthritis of many joints) characterised by pain and swelling. If the disease remains active, irreversible damage can occur. This causes stiffness, deformity and instability.
To see typical radiographic changes, click on the screen (75KB file) In the early stages, treatment is supervised by your Rheumatologist. Management consists mainly of anti-inflammatory drugs and sometimes more specific anti-rheumatoid drugs. During this time you may be referred to a Hand Therapist. She will give you advice about protecting your joints. She may also provide you with splints to prevent deformity or to rest inflamed joints. Occasionally, surgery is needed in the early stages to correct:
  • Nerve compression
  • Tendon rupture (snap)
(see X-ray) . These are associated with functional problems such as loss of movement and poor grip. The need for surgery is assessed individually, often jointly by Surgeon, Rheumatologist and Hand Therapist. Factors taken into account are:

The authors review the latest drug therapy options and treatment plans, with an emphasis on patient education, communication, and involvement. N umerous drugs and other measures have been used to treat rheumatoid arthritis. Throughout history, purveyors of antirheumatic therapies have enjoyed lucrative careers catering to the 2% of the world's population affected by this debilitating disease. Unfortunately for Pierre August Renoir, Thomas Jefferson, Theodore Roosevelt, and the countless millions who have suffered the ravages of rheumatoid arthritis, treatments improving long-term outcome have only recently become available. Rheumatoid arthritis is not a benign disease. A 25-year prospective study (1) showed that median life expectancy is shortened by 7 years in males and 3 years in females. A significant decline in functional status also occurs over time. About 50% of patients with rheumatoid arthritis cannot function in their jobs within 10 years of disease onset. According to one study, (2) 48% of patients with a recent diagnosis were highly functional compared with 17% who had had the disease for 12 years. An important discovery was that joint destruction occurs early in disease (3). Within 3 years of onset, 70% of patients show radiographic damage, which is more pronounced during the first year of disease than during the second or third year.

One in three adult Americans suffer from some form of Arthritis and the disease affects about twice as many women as men. Arthritic diseases include rheumatoid and psoriatic arthritis, which are autoimmune diseases ; septic arthritis, caused by joint infection; and the more common osteoarthritis, or degenerative joint disease. Arthritis can be caused from strains and injuries caused by repetitive motion, sports, overexertion, and falls. Unlike the autoimmune diseases, osteoarthritis largely affects older people and results from the degeneration of joint cartilage. Arthritis may also be caused by gout Arthritic joints can be sensitive to weather changes. Few trials likely to be bias free (randomised, blind), and valid (longer duration) exist. The results are mixed, but a cautious interpretation may be that there is no evidence of lack of effect, while what evidence we have suggests some benefit. Clinical observation has been that diet may improve the symptoms of rheumatoid arthritis. The biology underlying any dietary influences is not exact. Some food additives are taken by patients with RA. It has also become traditional in some centres for patients to fast, followed by a vegan or vegetarian diet with add-back of foods not associated with symptom worsening. This may be accompanied by holistic approaches involving physiotherapy, exercise, and psychotherapy. A systematic review has examined the evidence-base for fasting and vegetarian diet.
Reference
Review
Only MEDLINE was searched (up to 1997), and reference lists checked. Criteria for inclusion were trials with follow-up information for at least three months, and full journal publication.
Results
The review detected 31 reports. Only one was randomised. A further randomised report with just under three months duration was also included. The other reports were observational studies, or controlled studies. Most were very small, with few having more than 30 patients in total, or 30 patients per group. The main findings in each study are reported, including clinical and biochemical or immunological findings.
