Reyes Syndrome Alaska reported its first case of Reyes Syndrome occurring in a young boy. Reyes Syndrome is an extremely serious illness characterized by encephalopathy, metabolic abnormalities and fatty infiltration of the liver. This syndrome usually is preceded by a non-specific viral illness. Mortality is 50% and the cause of the syndrome is unknown. Generally this disease strikes young children. Fortunately, the child survived. In this particular case, Adenovirus, Type 14, was isolated from the cerebral spinal fluid. (Reported by Dion Roberts, M.D.) Hand, Foot and Mouth Disease Hand, foot and mouth disease is caused by a Coxsackie Virus, usually Type A-l6. The disease is usually characterized by vesicular stomatitis with oral lesions occurring on the buccal surface of the cheeks and gums. Papulovesicular lesions also occur on the hands, feet and legs and may persist for 7 to 10 days. Hence the name hand, foot and mouth disease. Herpangina, also caused by a Coxsackie Virus (usually Type 3) differs from hand, foot and mouth disease in that vesicular lesions are generally confined to the soft palate and anterior pharynx. Treatment is symptomatic for both of the above diseases.

Swelling can also develop in the brain and cause such emergency symptoms as seizures or convulsions and eventually lead to coma and brain death. After Reyes Syndrome was identified in 1963, the number of recognized cases began to increase during the 1970's. In 1980 the number of cases peeked in the U.S. at 555. After that the figure began a sharp decline. In 1984-94 there were no more than 36 cases of Reye's syndrome per year, and since then only a few cases have been reported each year.

    What is Reyes Syndrome?
    Aspirin - Reyes Syndrome
    The cause of Reyes Syndrome remains a mystery. However studies have shown that using aspirin or salicylate-containing medications to treat viral illnesses increases the risk of developing Reyes Syndrome. A physician should be consulted before giving a child any aspirin or anti-nausea medicines during a viral illness, which can mask the symptoms of Reyes Syndrome.     Is there any treatment for Reyes Syndrome?
    There is no cure for Reyes Syndrome. Successful management, which depends on early diagnosis, is primarily aimed at protecting the brain against irreversible damage by reducing brain swelling, reversing the metabolic injury, preventing complications in the lungs, and anticipating cardiac arrest. It has been learned that several inborn errors of metabolism mimic Reyes Syndrome in that the first manifestation of these errors may be an encephalopathy with liver dysfunction. These disorders must be considered in all suspected cases of Reyes Syndrome. Some evidence suggests that treatment in the end stages of Reyes Syndrome with hypertonic IV glucose solutions may prevent progression of the syndrome.

    Reyes Syndrome By Dr Rob Hicks Reye's syndrome is a rare illness that affects the brain and liver. It affects children when they appear to be recovering from a viral infection, for example, the flu, diarrhoea or chickenpox. It was first described in 1963 by an Australian pathologist called R. Douglas Reye. The cause of Reye's syndrome is not fully understood. Taking aspirin to treat a viral illness with fever may trigger or aggravate the development of Reye's syndrome. Symptoms of Reye's syndrome develop as a result of swelling of the brain and of the liver. Common symptoms include the personality of an affected child changing with them becoming irritable, aggressive and confused. Vomiting is common. A child may become drowsy and lose consciousness. Reye's syndrome mainly occurs in children under five years of age. It becomes less likely to develop as a child gets older and it rarely occurs over the age of 19 years old.

    From the Desk of Kathy Clark, RN As the school year moves on students with viral illness symptoms are being seen in each of the building clinics in increasing numbers. With this in mind, I want to take the opportunity to remind all parents that occasionally following a viral illness or chicken pox, some young people develop a disorder called Reye’s Syndrome. Reye’s generally occurs when someone is recovering from any viral illness, but it can develop 3 to 5 days after the onset of the illness. Early diagnosis is crucial. If the following symptoms occur 2 to 3 weeks after a viral illness seek medical care:
    • Relentless or continuous vomiting Listlessness Drowsiness Personality changes Disorientation or confusion Combativeness Delirium, convulsions or loss of consciousness
    Studies have shown that using aspirin or aspirin containing products to treat the symptoms of viral illness increases the chance of developing Reye’s Syndrome. Products labeled aspirin, acetylsalicylate, salicylate, or salicylic acid should NOT be used.

    Reye's Syndrome is a very serious disease that often develops during cold, flu and chickenpox seasons. The disease occurs after a viral illness begins to wane, although it can occur within three to five days of the onset and can affect any age group from infants to adults. This disease affects all organs of the body but its main targets are the liver and brain. It is a two-phase illness because it is thought to be connected to a previous viral infection. One danger of Reye's is that it is often difficult to diagnose and is often misdiagnosed as meningitis, encephalitis, drug overdose, Sudden Infant Death Syndrome, diabetes, or even psychiatric illness. In Reye's Syndrome, abnormal concentrations of fat begin to develop in the liver and other organs of the body along with a severe increase of pressure on the brain. Quick and accurate diagnosis is essential in this life-threatening disease. While the cause is unknown, it is known that Reye's is not contagious. However, it is life-threatening and staff should be aware of the symptoms. If you have a child in your center recovering from a virus, watch for these symptoms during the 2 to 3 weeks following the illness:
    • Constant vomiting
    • Loss of energy and drowsiness
    • Personality changes such as slurred speech, irritability, aggressive behavior or sensitivity to touch
