Study Design: Treatment, Safety/Efficacy Study Further Study Details: Two concerns emerged from the CRYO-ROP extensive study on the natural history of ROP and treatment of threshold ROP. The first of these is failure of peripheral retinal ablation to eliminate all, or nearly all cases, of retinal detachment due to ROP. In the CRYO-ROP Study, 26 percent of eyes with threshold disease in zone II and 78 percent of eyes with zone I threshold disease had an unfavorable structural outcome despite treatment. The second concern is that most children who developed threshold ROP disease had visual acuity worse than 20/40 even if the eye had a favorable structural outcome. Since no other treatment has yet been shown to be effective in preventing blindness from ROP, retinal ablation remains the treatment of choice. The ETROP Study will test whether earlier treatment is more effective than treatment at threshold in improving functional (visual acuity) outcome following ROP, as well as determining whether earlier treatment decreases the probability of an unfavorable structural outcome.

The Low Vision Centers of Indiana Retinopathy of Prematurity (ROP), originally called retrolental fibroplasia, was the leading cause of blindness in children in the 1940s and 1950s. It was first described in the medical literature in 1942 by Terry. In 1952, Campbell theorized that the condition was caused by the use of oxygen therapy to treat the immature lungs in premature infants. Today, we realize that oxygen is not the only factor in developing ROP. High levels of oxygen have been associated with ROP, but lower levels of oxygen may lead to more respiratory complications and death in premature infants. Better oxygen level monitoring has led to better control of the oxygen given to premature infants. Today, however, there is an increase in ROP due to the fact that neonatal care advances mean more low weight premature infants are surviving. Statistics ROP occurs in over 16% of all premature births. In babies weighing less than 1,700 grams at birth, over 50% will develop ROP. In the United States , over 2,100 children annually experience the complications of ROP. Of those estimates of 500 to 1,200 cases of new blindness or severe complications are reported.

Today, ROP is second-leading cause of vision loss and blindness in children of developed countries. Retinopathy of prematurity occurs when blood vessels that normally would develop in the womb begin to grow in the wrong places in the eye after premature delivery. Early examination by an ophthalmologist is necessary to catch ROP and treat it in its early stages. At Texas Children's, ophthalmologists examine premature babies for ROP at five weeks after birth. Ophthalmologists at Texas Children's Hospital are currently participating in a study on the early treatment of retinopathy of prematurity.

Premature infants are not born with retinopathy but with incomplete development of the blood supply to the retina.  The retina is the light sensitive internal lining of the eye.  Infants at risk for developing ROP are infants who are extremely premature, i.e. a birthweight of less than 1500 grams or a gestational age of less than 32 weeks. Blood vessel development is complex in the eye, one system of blood flow is present early in development, then at about 16 weeks, new tissues grow from the optic disc to stimulate growth of blood vessels in the retina.  This is completed 2-3 months after full term birth.  ROP occurs when the development of the blood vessels is abnormal. Retinopathy is a progressive disease; it starts around the fourth to tenth week after birth. It has several stages.  Not all infants will progress through all the stages.  Stage one is when there is a clear demarcation between the mature and immature retina.  Stage 2 displays a rolled edge of scar tissue as a demarcation line.  Stage 3 is when there are new and abnormal blood vessels growing into the scar tissue.  In Stage 4 the scar tissue causes the retina to pull away from the back wall of the eyeball.

Retinopathy of Prematurity (ROP), formerly known as Retrolental Fibroplasia (RLF), is an eye condition in which abnormal blood vessels grow in the eye, causing bleeding and scarring. This scarring may pull the retina out of position or even make it detach, which could result in vision impairment or blindness.
There are five stages of ROP. Stages one and two usually disappear on their own. These stages involve mild to moderate abnormal vessel growth. Stage three, involving severe abnormal vessel growth, may also go away without treatment, but close attention should be paid, for it may evolve into stage four, meaning part of the retina detaches. This is because scarring remains even when ROP has gone away, so it can still pull the retina out of position or cause retinal detachment. Stage four involves partial retinal detachment and stage five means the retina has been completely detached, meaning a person is totally blind.
The exaact cause of ROP is unknown. It used to be thought that an excessive amount of oxygen during the NICU time caused ROP. However, a lack of oxygen is also associated with ROP. Generally, the greatest risk factors are being born before the thirtieth week gestation or weighing less than 1250 grammes.

Purpose
To test the efficacy, safety, and costs of providing supplemental oxygen in moderately severe retinopathy of prematurity (prethreshold ROP).
Background
ROP remains one of the important morbidities among extremely premature infant survivors who are otherwise experiencing great gains in survival and in pulmonary and neurologic sequelae. While the use of cryo- or laser ablation of peripheral retina during the severe stages of ROP reduces the proportion of infants who progress to retinal detachments, less destructive treatment would be desirable.
Description
Infants who develop moderately severe ROP are recruited to participate in STOP-ROP. Following informed consent, eligible infants are randomized to oxygen administration with continuous saturation monitoring at conventional levels (target pulse oximetry, 89-94 percent saturation) versus supplemental levels (target pulse oximetry, 96-99 percent saturation). Pulse oximetry is monitored continuously, and feedback to the bedside nurses is provided in a variety of formats on a laptop computer screen. Compliance with study targets is recorded systematically. Exact severity of ROP is confirmed by two independent, masked ophthalmologists at study entry and again if severe ROP (threshold) occurs. Infants remain on study-assigned oxygen saturation ranges for at least 2 weeks and until both eyes have reached study end points.

Background: Retinopathy of prematurity (ROP) causes vision loss in many premature infants each year, despite the advances being made in treatment. In the search for ways to prevent the disease altogether, the exposure of the retina to bright ambient light following premature birth has been a natural hypothesis, since the premature infant normally would be in the dark in-utero environment. Several controlled studies have now addressed this theory. Objectives: To answer the question: "Among very low birth weight infants, what is the effect of reducing early environmental light exposure on the incidence of any "Acute ROP", or "Poor ROP Outcomes"? Search strategy: Searches were made of the Cochrane Neonatal Group Register of Controlled Trials, Medline, EMBASE, the Cochrane Library, previous reviews including cross references, abstracts, conference and symposia proceedings, and expert informants. The search terms used were [retrolental fibroplasia or retinopathy of prematurity] and [light or light/ae or lighting or lighting/ae or light/tu or lighting/st].
