Retinopathy of Prematurity
Retinopathy of Prematurity (ROP), also known as retrolental fibroplasia, is a potentially blinding condition affecting the retina of newborns. The factors that put infants at greatest risk of developing ROP are low birth weight (less than 3.5 pounds) and premature delivery (26-28 weeks). In babies born prematurely, the growth and development of normal blood vessels in the retina is halted and abnormal vessels may begin to develop.

Vast improvements in neonatal care over the past few decades have resulted in the saving of many premature babies who never would have survived just 20 or 30 years ago. Extremely low birth weight babies are living today at a rate of 85-90 percent and in the best nurseries that we have in the US, well in excess of 90 percent. With the exportation of modern neonatology to developing nations, many more babies are surviving there, too.
In developing countries, ROP is often not identified and treated. This is the case in much of Central and South America, so much so that at the Bascom Palmer Institute, once or twice a month and sometimes as often as three or four times we would see an 18-month or two-year-old child with stage 5 retinopathy of prematurity. No eye doctor had seen these children. In general, this is because ophthalmology has not kept pace with neonatology. In developing countries, ROP is largely ignored by ophthalmologists.
Steps are just beginning to facilitate communication among ophthalmologists around the world to identify early cases of ROP. Among them is the creation of a universal classification system that allows us to discuss ROP with ophthalmologists worldwide, which has been established already. A second step is the use of digital imaging to share images of neonates' eyes at risk for end-stage ROP. Digital imaging has provided a means for non-ophthalmologists to help screen for ROP by using a digital camera and a high-speed modem that can instantaneously send a high-quality image to an expert in this disease.

Retinopathy of Prematurity
Retinopathy of Prematurity (ROP) is an important cause of childhood blindness that can be screened for and treated with some success. The sequelae of untreated or non-responsive ROP can be devastating and may often be associated with other disabilities, and so steps to limit the condition are of vital importance for the individual children concerned, for their families and for the healthcare system that provides for them. Definition and Classification Threshold ROP is 5 contiguous clock hours of stage 3 retinopathy in zones I or II (8 clock hours or more if not contiguous), in the presence of 'plus' disease (dilated retinal arterioles and congested veins, abbreviated as '+'). If untreated, it is associated with a poor anatomical outcome in approximately 50% of cases. Pre-threshold ROP is usually defined as any retinopathy in zone 1, or Stages 2+ or 3 in zone II. Stage 3 ROP has a greater likelihood of an anatomically unfavourable outcome if present in the zone 1 (59% poor outcome)than in zone II (44% poor outcome if stage 3+

... Retina Services Retinopathy of Prematurity The last 12 weeks of gestation is a very active growth period for the fetal eye. At 16 weeks of gestation the blood supply to the retina starts at the optic nerve. Gradually vessels grow out over the surface of the retina and stop at about the time of birth. The normal retinal vessel growth is delayed in premature infants. Retinopathy of prematurity (ROP) is a spectrum of findings associated with immature retinal vasculature. ROP is more common in low weight infants (less than 1500 grams) with significant medical problems. Most babies with ROP actually "outgrow" their eye problem as the retinal vasculature matures. A minority of babies (7%) with ROP ever necessitate treatment. Laser treatment, when indicated, significantly decreases the risk of severe retinal damage in ROP. ROP screening retinal exams are routine at newborn intensive care units for low weight premature babies.

Retinopathy of prematurity (ROP) is a retinal disorder of low birth weight premature infants potentially leading to blindness in a small but significant percentage of those infants. The results of the Multicenter Trial of Cryotherapy for Retinopathy of Prematurity Cooperative Group indicated that treatment is associated with a 41% decrease in the occurrence of posterior retinal traction folds or detachments and a 19% to 24% decrease in the incidence of blindness when evaluated 5 years later. Because of the sequential nature of the progression of ROP, the proven benefits of cryotherapy and, more recently, the acceptance of at least equivalent therapeutic benefit of laser therapy for the same indications

Overview Retinopathy of Prematurity (ROP)
, also known as retrolental fibroplasia, is a potentially blinding condition affecting the retina of newborns. In the 1950's it was associated with the use of high amounts of oxygen in neonatal units. Today, modern neonatal care has curbed the incidence, yet because the survival rate of low birth weight infants is much higher, the exposure of surviving babies to required oxygen levels is increasing. The factors that put infants at greatest risk of developing ROP are low birth weight (less than 3.5 pounds) and premature delivery (26-28 weeks). In babies born prematurely, the growth and development of normal blood vessels in the retina is halted and abnormal vessels may begin to develop. The problem with abnormal vessel growth, known as neovascularization, is that it does not deliver adequate oxygen supply to the retina. In addition, it may cause many secondary problems.
ROP is classified in 5 stages, depending on the extent of the disease. Progression of the disease to later stages can lead to the formation of scar tissue in the retina and complications such as: retinal detachment

Visual disability and blindness from severe retinopathy of prematurity (ROP) may be prevented with timely treatment during the evolution of stage 3 ROP.1 Accordingly, infants at risk must be examined to discover and to follow the development of ROP.
The Canadian Ophthalmological Society, the Canadian Association of Pediatric Ophthalmologists and the Canadian Paediatric Society recommend the following guidelines for screening premature infants for ROP. Risk group
  • Infants with a birth weight of 1500 g or less
Examiners The examiner should be a skilled observer in the recognition of ROP, usually an ophthalmologist. Schedule of assessments
  • The first examination should be performed 4 to 6 weeks after birth.
  • A follow-up examination should be carried out approximately 2 weeks after every examination that reveals no ROP.5.

General Discussion
Retinopathy of prematurity (ROP) is a potentially blinding disease affecting the retinas in premature infants. The retinas are the light-sensitive linings of the insides of the eyes. In infants born prematurely, the blood vessels that supply the retinas are not yet completely developed. Although blood vessel growth continues after birth, these vessels may develop in an abnormal, disorganized pattern, known as ROP. In some affected infants, the changes associated with ROP spontaneously subside. However, in others, ROP may lead to bleeding, scarring of the retina, retinal detachment and visual loss. Even in cases in which ROP changes cease or regress spontaneously, affected children may have an increased risk of certain eye (ocular) abnormalities, including nearsightedness, misalignment of the eyes (strabismus), and/or future retinal detachment. The two major risk factors for ROP are a low birth weight and premature delivery.
