Retinopathy of Prematurity
ROP is a disorder of the developing retinal vasculature that occurs with interruption of the forming retinal vessels. Constriction and obliteration of the advancing capillary bed are followed by neovascularization of the retina, which can extend into the vitreous. The most serious and feared complication of ROP is retinal detachment. ROP has previously been known as RLF- Retrolental Fibroplasia, a very advanced form or ROP with end stage fibrosis and scarring behind the lens.
Development of ROP: It is postulated that there are two events that occur
  • Vasoconstriction and obliteration of the capillary network in response to a vascular insult ( possibly high supplemental O2 concentration) Vasoproliferation- possibly a response by the hypoxic retina to an angiogenic factor released by the insult — thought that hypoxia can cause an overexpression of VEGF that can induce abnormal retinal angiogenesis.
  • Etiology of ROP: multifactorial and still unclear
  • Oxygen administration originally was thought to contribute to the development of ROP. This is now being debated. The STOP ROP- Supplemental Therapeutic Oxygen to Prevent ROP study investigated whether supplemental therapeutic oxygen for premature infants reduces the proportion of infants that progress to threshold ROP. The study found that the more liberal use of oxygen actually decreased the risk of progression to threshold ROP in these infants from 48% to 41%. Threshold ROP is defined as disease progression to the point of necessitating peripheral retinal ablation therapy.

    What is retinopathy of prematurity? The eye is often compared to a camera. The front of the eye contains a lens that focuses images on the inside of the back of the eye. This area, called the retina, is covered with special nerve cells that react to light. Underneath the retina is a network of blood vessels. These blood vessels normally grow quickly in the last few weeks before a baby is born. If the baby is born prematurely, there can be a problem with this growth. In some premature babies, the blood vessels grow into parts of the eye where they do not belong. This can cause scar tissue to form inside the eye. The scar tissue can damage the retina and cause a significant loss of vision. This condition is called retinopathy of prematurity. Why do babies get retinopathy of prematurity?

    RETINOPATHY OF PREMATURITY Retinopathy of prematurity (ROP) is an eye disease that occurs in some premature babies. The last 12 weeks of a full-term delivery, from 28-40 weeks gestation, are particularly active for the growth of the fetal eye. The blood supply to the retina starts at the optic nerve at 16 weeks of gestation, and the vessels gradually grow out over the surface of the retina. The vessels reach the anterior edge of the retina and stop their progression at about the time of birth. classification of ROP has been developed to standardize evaluation and research in this disease. This classification is not just a way to pigeonhole patients; learning about this classification is very helpful in understanding the disease process and rationale for treatment. Premature children are at risk for developing ROP because they have been taken out of the protective environment of the uterus and are exposed to many things, including medications, high levels of oxygen, and variations in light and temperature. Some or all of these factors may have an effect on the development of ROP. A number of risk factors for ROP have been identified.

    What is Retinopathy of Prematurity (ROP)?
    The internal surface of the back of the eye is lined by a tissue, called the retina. This tissue is equivalent to the film in a camera and is responsible for the initial formation of the visual image. This is then transmitted to the brain. In order to function, the retina requires a blood supply. The normal development of this blood supply for the retina starts at 16 weeks into a pregnancy and is completed by 36 weeks. If an infant is born prematurely, with the retinal blood vessel development incomplete, problems occur. Abnormal blood vessels may develop which can subsequently lead to bleeding and scar tissue formation. This may then stretch the retina pulling it out of position. Visual loss may result. Inherited ROP There does not appear to be any hereditary basis for this condition, i.e. it is association with premature birth and not specific families. What Causes ROP? The lack of oxygen in areas of retina which have not developed blood vessels, results in the release of chemicals that promote the growth of new blood vessels. These blood vessels often grow in an irregular manner, for reasons that are not completely understood. The presence of large amounts of oxygen (often needed for the baby's survival) are thought to be possibly toxic to the developing blood vessels. As premature babies have underdeveloped lungs they usually require additional oxygen therapy. There are other undetermined factors also contributing.

    Retinopathy of prematurity: A systematic review of the literature
    The Fetus and Newborn Committee, Canadian Paediatric Society OBJECTIVE: To assist in the diagnosis of retinopathy of prematurity (ROP) to facilitate treatment in a timely manner to help prevent blindness.
    DATA SOURCES: Systematic review using MEDLINE including the following key words, “retinopathy of prematurity”, “retrolental fibroplasia”, “blind”, “blindness”, “vision screening”, “cryotherapy”, “cryosurgery”, “laser” and “ablative therapy”. The bibliographies of the references found using the above techniques were scanned for references missed in the primary search.
    DATA SELECTION: Eight population-based studies examining the incidence and severity of ROP were identified. Other studies of ROP were included because they contributed to an understanding of the natural history, treatment or long term outcome of ROP.

    Retinopathy of Prematurity
    Retinopathy of prematurity is a neonatal complication, primarily related to exposure to elevated oxygen tension during the period in which the retina is maturing. Embryologically, the retina begins vascularization at 16 weeks gestational age, and is complete on the nasal side by 8 months, and entirely complete at term. The oxygen causes constriction of the capillaries growing in the forming retina, and results in obliteration of the capillary bed. This is followed by neovascularization of the vitreous and retinal edema. Fibrosis then occurs and is followed by traction and detachment of the retina. Classification : ROP is classified by stage, the number of circumferential clock hours of involved retina, and by zone. Zone I is defined as 30 degrees around the macula, zone II extends to the ora serrata nasally, and to the anatomic equator temporally, and Zone III is the small crescent of remaining temporal territory.
    Stage I : A demarcation line between the vascularized and still avascular retina. Stage II : The line develops into a raised ridge, projecting into the vitreous. Histologically, there is evidence of an AV shunt in this tissue.

    Pediatric Visual Diagnosis Fact Sheet TM RETINOPATHY OF PREMATURITY DEFINITION Retinopathy of Prematurity (ROP) is an eye disorder affecting premature infants. This disorder was called Retrolental Fibroplasia in the past. ROP affects immature blood vessels of the retina. It occurs weeks after birth. Once development of blood vessels is complete, a child is no longer a candidate for this disorder. As pictured in the graphic of the retina:
    Zone One is centered on the Macula (which is the area of central vision needed for reading).
    Zone Two refers to an area that is doughnut shaped that extends to the edge closest to the nose.
    Zone Three
    refers to the crescent shaped area toward the ear. If disease appears in Zone One (as can happen with extremely low birthweight infants), damage to the retina tends to be more severe. If Zone Three is affected, the premature infant will tend to have mild involvement, leading to useful vision. CAUSES The suggested causes of Retinopathy of Prematurity are those conditions which stop the orderly growth of retinal blood vessels and stimulate their wild overgrowth.

    Vast improvements in neonatal care over the past few decades have resulted in the saving of many premature babies who never would have survived just 20 or 30 years ago. Extremely low birth weight babies are living today at a rate of 85-90 percent and in the best nurseries that we have in the US, well in excess of 90 percent. With the exportation of modern neonatology to developing nations, many more babies are surviving there, too. In developing countries, ROP is often not identified and treated. This is the case in much of Central and South America, so much so that at the Bascom Palmer Institute, once or twice a month and sometimes as often as three or four times we would see an 18-month or two-year-old child with stage 5 retinopathy of prematurity. No eye doctor had seen these children. In general, this is because ophthalmology has not kept pace with neonatology. In developing countries, ROP is largely ignored by ophthalmologists. Steps are just beginning to facilitate communication among ophthalmologists around the world to identify early cases of ROP. Among them is the creation of a universal classification system that allows us to discuss ROP with ophthalmologists worldwide, which has been established already. A second step is the use of digital imaging to share images of neonates' eyes at risk for end-stage ROP. Digital imaging has provided a means for non-ophthalmologists to help screen for ROP by using a digital camera and a high-speed modem that can instantaneously send a high-quality image to an expert in this disease.

    Michael X. Repka, MD Clinical trials for retinopathy of prematurity (ROP) are not new. Clinical research into the disease at least its ocular manifestations dates back more than 50 years. Research has led to enhanced prevention of the disease as well as improved outcomes of treatment. Improvements in the neonatal intensive care unit (NICU), in the training of NICU staff, and developments in the fields of neonatology and nutrition all of which have evolved over the last two decades have boosted prevention. And improved treatment includes all examinations and the latest interventions performed by an ophthalmologist. ROP research methods, like those of any disease, include observational studies, in which its natural history is monitored; and case series, in which an assessment is made of the impact of changes in nursery care, the impact of a nutritional change or the impact of an intervention. These studies utilize patient populations drawn from two different time periods, and cannot account for other changes in care. Thus, such studies cannot prove the beneficial or deleterious effect of other changes. Randomized clinical trials are the most effective in proving the value of an intervention. This article examines clinical trials that have been part of research efforts to cure ROP.
